What is a hernia?
A hernia occurs when a portion of an organ within the abdomen, such as the intestines, protrudes through a weakness in the muscles of the abdomen. A soft bulge is seen underneath the skin where the hernia is located.
What is an inguinal hernia?
A hernia that occurs in the groin (inguinal) area is called an inguinal hernia.
What causes it?
Between 12 to 14 weeks of fetal development, the testicles or ovaries form in the abdomen near the kidneys. They gradually move down into the lower part of the abdomen as the baby continues to develop. As they move down, a portion of the peritoneum (a thin layer of tissue that lines the inside of the abdomen) that attaches to the testicle is drawn with it into the scrotum, forming a pouch or sac. A similar process occurs in girls as the round ligament of the uterus descends into the groin at the labia. This sac is known as the processus vaginalis and normally closes shortly after birth. This eliminates any connection between the abdominal cavity and the scrotum or groin. When closure of the processus vaginalis is delayed or incomplete, it may stretch and eventually become a hernia. The stretching of the processus vaginalis creates an inguinal sac, allowing organs to extend from the abdomen and enter the sac. If fluid, rather than organs, builds up and remains in the sac, the child has a

Hiatus Hernia ... see also Heartburn What is a hiatus hernia? A hiatus hernia occurs when the upper part of the stomach, which is joined to the oesophagus (gullet), moves up into the chest through the hole (called a hiatus) in the diaphragm. It is common and occurs in about 10 per cent of people. What are the symptoms? Most people are not troubled by their hiatus hernia, but if reflux of the acid contents of the stomach occurs (called gastro-oesophageal reflux), you get heartburn. This is a painful burning sensation in the chest, which can sometimes be felt in the throat. Sudden regurgitation of acid fluid into the mouth can occur, especially when you lie down or bend forward. These symptoms are a problem when you go to bed and can wake you up. Other symptoms include belching, pain on swallowing hot fluids and a feeling of food sticking in the oesophagus. Who gets a hiatus hernia? It is most common in overweight middle-aged women and elderly people. It can occur during pregnancy. The diagnosis is confirmed by barium meal X-rays or by passing a tube with a camera on the end into the stomach (gastroscopy). What are the risks?

Hernia van de rug
Versiedatum: april 2005

Hernia
Definition
Hernia is a general term used to describe a bulge or protrusion of an organ through the structure or muscle that usually contains it. Description
There are many different types of hernias. The most familiar type are those that occur in the abdomen, in which part of the intestines protrude through the abdominal wall. This may occur in different areas and, depending on the location, the hernia is given a different name. An inguinal hernia appears as a bulge in the groin and may come and go depending on the position of the person or their level of physical activity. It can occur with or without pain . In men, the protrusion may descend into the scrotum. Inguinal hernias account for 80% of all hernias and are more common in men. Femoral hernias are similar to inguinal hernias but appear as a bulge slightly lower. They are more common in women due to the strain of pregnancy A ventral hernia is also called an incisional hernia because it generally occurs as a bulge in the abdomen at the site of an old surgical scar. It is caused by thinning or stretching of the scar tissue, and occurs more frequently in people who are obese or pregnant. An umbilical hernia appears as a soft bulge at the navel (umbilicus). It is caused by a weakening of the area or an imperfect closure of the area in infants. This type of hernia is more common in women due to pregnancy, and in Chinese and black infants. Some umbilical hernias in infants disappear without treatment within the first year.

        Hiatal Hernia
        What is a hiatal hernia?
A hernia is a protrusion of part of an organ through the muscle wall that surrounds it. A hiatal hernia occurs when the upper part of the stomach pushes up into the chest through a small opening in the diaphragm, the muscle that separates the stomach from the chest. A hiatal hernia results in retention of acid and other contents above this opening. These acids and other substances can easily back up (reflux) into the esophagus. Many people age 50 and over have small hiatal hernias; however, hiatal hernias may affect people of all ages.

Local Anesthesia for Inguinal Hernia Repair (Originally posted 22 September 1997 on About Anesthesiology) INTRODUCTION
Many practitioners and institutions routinely and successfully employ local anesthesia for inguinal hernia repairs. Although there are certainly other options (including general anesthesia and regionl anesthesia such as spinal or epidural), local offers some unique advantages to the patient. As such, here is a short summary about the use of local for these operations including a discusssion of advantages and disadvantages. Obviously, the choice of anesthesia will be influenced by patient preferences and needs. In addition, to complete any operation successfully under local anesthesia requires a surgeon who is comfortable with the technique and willing to stop surgery and place additional local anesthesia should this become necessary. Interestingly with inguinal hernia repair it is rarely, if ever, necessary to supplement with additional injections once the initial infiltration is complete. Reported experiences with it have been uniformly positive. Advantages of Local Anesthesia
Minimal physiological disturbance which may be an advantage in the patient in whom you wish to avoid a general anesthetic and in whom a regional technique may be contraindicated. This could include patient's with serious cardiac or respiratory diseases which could tolerate other types of anesthesia but would be at reduced risk if given only local anesthesia.
