Introduction Local anaesthesia may be employed in hernia operations, either on its own or combined with general anaesthesia. The choice of technique will be influenced not only by local resources and skills, but also by patient preference. Advantages of Local Anaesthesia for Hernia Repair With a careful technique, local anaesthesia causes minimal physiological disturbance. This may be particularly useful for patients with cardiovascular or respiratory disease for whom there may be advantages in avoiding a general anaesthetic. The absence of postoperative sedation or drowsiness allows early ambulation and diminishes the requirement for recovery facilities. Local anaesthesia provides postoperative analgesia for up to four hours and may be administered by the surgeon. When adrenaline is mixed with the local anaesthetic (normally in a dilution of 1:200,000) useful vasoconstriction is produced resulting in a relatively bloodless field.
Disadvantages of Local Anaesthesia for Hernia Repair Surgery on the awake patient under local anaesthesia must be carried out gently. Although pain sensation is usually blocked by the anaesthetic, traction on certain tissues, particularly the peritoneum, is uncomfortable. The patient should be warned that some sensation may be experienced during the operation but that it will not be painful. Larger hernias, particularly those with incarcerated bowel may prove unsuitable for local anaesthesia.

A hernia is the protrusion of an organ or tissue out of the body cavity in which it normally lies. By far the most common hernias develop in the abdomen , when a weakness in the abdominal wall evolves into a localized hole, or "defect", through which the protrusion occurs. These may present either with pain at the site, a visible or palpable lump, or in some cases by more vague symptoms resulting from pressure on an intraabdominal organ which has become "stuck" in the hernia. Fatty tissue usually enters a hernia first, but it may be followed by or accompanied by a segment of intestine or other intraabdominal organ. It is generally advisable to repair hernias in a timely fashion, in order to prevent complications which include intestinal blockage, gangrene , and death. Most hernias can be surgically repaired. Recovery rarely requires long-term changes in lifestyle. By far the most common abdominal hernias are the so-called inguinal hernias . Named for the Latin word for "groin", these involve the bulging of abdominal contents into a defect in the

Many are performed by the conventional "open" method. Some hernia repairs are performed using a small telescope known as a laparoscope. If your surgeon has recommended a laparoscopic repair, this brochure can help you understand what a hernia is and about the treatment. WHAT IS A HERNIA? A hernia occurs when the inside layers of the abdominal muscle have weakened, resulting in a bulge or tear. In the same way that an inner tube pushes through a damaged tire, the inner lining of the abdomen pushes through the weakened area of the abdominal wall to form a small balloon like sac. This can allow a loop of intestine or abdominal tissue to push into the sac. The hernia can cause severe pain and other potentially serious problems that could require emergency surgery.

LAPAROSCOPIC VENTRAL HERNIA REPAIR Laparoscopic hernia repair is a technique to fix tears or openings in the abdominal wall using small incisions, laparoscopes (small telescopes inserted into the abdomen) and a patch (screen or mesh) to reinforce the abdominal wall. It may offer a quicker return to work and normal activities with decreased pain for some patients. WHAT IS A VENTRAL HERNIA? Other sites that ventral hernias can develop are the belly button (umbilicus) or any other area of the abdominal wall. A hernia does not get better over time, nor will it go away by itself. HOW DO I KNOW IF I HAVE A HERNIA? A hernia is usually recognized as a bulge under your skin. Occasionally, it causes no discomfort at all, but you may feel pain when you lift heavy objects, cough, strain during urination or bowel movements or with prolonged standing or sitting. The discomfort may be sharp or a dull ache that gets worse towards the end of the day. Any continuous or severe discomfort, redness, nausea or vomiting associated with the bulge are signs that the hernia may be entrapped or strangulated. These symptoms are cause for concern and immediate contact of your physician or surgeon is recommended. WHAT CAUSES A VENTRAL HERNIA?
