The Tyler Medical Clinic Assisted Conception - Affordable Infertility Treatments - List of Diseases - Hereditary Spastic Paraplegia Hereditary Spastic Paraplegia (HSP) refers to a group of degenerative spinal cord disorders characterized by gradual, progressive weakness and stiffness (spasticity) of the legs. Approximately 10,000 people in the United States are afflicted with HSP. In HSP, the motor nerves that innervate the lower limbs start to deteriorate. This results in difficulty with balance, weakness and stiffness of the legs, muscle spasms, and dragging of toes while walking. Onset is generally during childhood but occasionally symptoms do not appear until later in life. The severity and progression of the disease also varies, even among families. Other symptoms include shortened Achilles tendon, leg cramps, hyperactive reflexes, and high arch feet (pes cavus). HSP is classified as either uncomplicated or complicated. Complicated HSP is rare and differs from uncomplicated HSP by the presence of other neurological disorders such as ataxia, epilepsy, optic and retinal neuropathies, mental retardation, ichthyosis and many others. There is no specific test to diagnose HSP. HSP is diagnosed only through careful elimination of other diseases that share common symptoms. Treatment aims to alleviate the symptoms. Physical therapy is important to maintain muscle strength and prevent further muscle weakness. Although the disease is progressive, rarely do patients ever become completely immobile. Assisted devices such as a cane or walker, however, are usually needed.

General Discussion
HSP may be classified into two major subtypes: "uncomplicated" or "complicated" HSP. In individuals with uncomplicated (or "pure") HSP, progressive spastic paraplegia occurs as an isolated, primary finding. In those with complicated HSP, additional neurologic abnormalities are present. Some individuals with uncomplicated HSP may develop muscle spasms and difficulties with bladder control. In those with complicated HSP, associated symptoms and findings may include visual and/or hearing impairment, mental retardation, impaired control of voluntary movements (ataxia), and/or other abnormalities.
According to researchers, changes (mutations) of many different genes may cause HSP. In most cases, such mutations appear to be transmitted as an autosomal dominant trait. More rarely, mutations for HSP may be inherited as an autosomal recessive or X-linked recessive trait. The basic underlying defect or defects in HSP are unknown. However, associated symptoms appear to result from progressive degenerative changes of regions of the spinal cord (corticospinal tracts) that convey motor impulses from the brain to muscles involved in controlling certain voluntary movements

Hereditary spastic paraplegia (HSP), also called familial spastic paraparesis (FSP), refers to a group of inherited disorders that are characterized by progressive weakness and stiffness of the legs. Though the primary feature of HSP is severe, progressive, lower extremity spasticity , in more complicated forms it can be accompanied by other neurological symptoms. These include optic neuropathy retinopathy (diseases of the retina), dementia ataxia (lack of muscle control), icthyosis (a skin disorder resulting in dry, rough, scaly skin), mental retardation peripheral neuropathy , and deafness . Diagnosis is primarily by neurological examination and testing to exclude other disorders. Specialized genetic testing and diagnosis are available at some medical centers. Is there any treatment? There are no specific treatments to prevent, slow, or reverse HSP. Symptomatic treatments used for other forms of chronic paraplegia are sometimes helpful. Regular physical therapy is important for improving muscle strength and preserving range of motion.

    Hereditary spastic paraplegia (HSP) is a clinically and genetically heterogeneous group of inherited disorders characterized by insidiously progressive, often severe, lower extremity weakness and spasticity. Symptom severity, age of onset and rate of disease progression may vary widely among patients. It is estimated that three in every 100,000 people are affected with HSP in the U.S. HSPs are classified by clinical presentation, as either uncomplicated or complicated, and by inheritance pattern. Uncomplicated (or pure) HSP is the most common form and is characterized by progressive lower extremity spastic weakness, accompanied by hypertonic urinary bladder disturbances, and often, mildly impaired vibration sensation. Uncomplicated HSP can be disabling, but it does not shorten life span. HSP is classified as complicated if the impairments of the uncomplicated form are accompanied by additional neurologic abnormalities that are not attributable to other co-existing disorders. Researchers estimate that uncomplicated HSP represents 90% of all HSP cases, while complicated HSP accounts for 10%.
    HSP's inheritance pattern may be autosomal dominant (the most common), autosomal recessive or X-linked. Recent advances in molecular genetic testing now enable the identification of at least 50% of uncomplicated autosomal dominant hereditary spastic paraplegia (ADHSP) cases.

There are at least 20 known types of hereditary spastic paraplegia, which is estimated to affect 20,000 people in the U.S. An inherited disease is caused by a gene mutation passed down from either the father or mother. With hereditary spastic paraplegia, these gene mutations alter the code for proteins involving the nerves in the brain and spinal cord, thus disrupting their proper function. There are three different modes of inheritance for hereditary spastic paraplegia: autosomal dominant, autosomal recessive and X-linked. Each mode has a different risk factor, ranging from almost zero to 50 percent. The most common form of hereditary spastic paraplegia, autosomol dominant, carries a 50 percent risk of inheritance.
