... The Office of the Vice President for Student Affairs After food leaves the stomach, it enters the small intestine, the beginning of the lower gastrointestinal (GI) tract. As it passes through, the major portion of digestion and absorption takes place. Undigested food residue, debris, and some water then pass into the large intestine, or colon, where almost all of the water absorbed, and the solids are compacted as feces. This fecal mass is stored until distention causes a reflex muscle movement that propels the mass into the rectum for defecation and excretion through the anus. Normally, ingested food passes through this system and its waste products are excreted within 35 – 70 hours, resulting in a normal bowel movement 3-12X/week. However, the defecation reflex is rather weak, and easy to ignore. We ignore the reflex for many reasons, including inconvenience or lack of private, clean, or comfortable bathroom facilities. After a few minutes, the reflex goes away, and will not begin again for several hours. During this time, the fecal mass becomes drier and harder, resulting in constipation. Constipation Constipation describes bowel movements that involve excessive straining, difficulty, or discomfort; or that occur less than twice a week. Although occasional constipation is common, chronic or persistent constipation and associated straining at bowel movements can lead to more serious problems. Complications of chronic constipation may include hemorrhoids, anal fissures, and damage to the muscles and supportive tissues of the anus and perineum. The most common cause of constipation is a diet lacking fluid and fiber; in combination with poor bowel habits, such as routinely delaying defecation. Other causes of constipation may include:

ON HEMORRHOIDS
The disease of the hemorrhoids is formed in this way: if bile or phlegm be determined to the veins in the rectum, it heats the blood in the veins; and these veins becoming heated attract blood from the nearest veins, and being gorged the inside of the gut swells outwardly, and the heads of the veins are raised up, and being at the same time bruised by the faeces passing out, and injured by the blood collected in them, they squirt out blood, most frequently along with the faeces, but sometimes without faeces. It is to be cured thus: 3. Another method of cure:-Having got the anus to protrude as much as possible, foment with hot water, and then cut off the extremities of the hemorrhoids. But this medicine should be prepared beforehand, as an application to the wound:-Having put urine into a bronze vessel, sprinkle upon the urine the flower of bronze calcined and finely triturated; then, when it is moistened, shake the vessel and dry in the sun. When it becomes dry, let it be scraped down and levigated, and apply with the finger to the part, and having oiled compresses, apply them, and bind a sponge above. 7. An hemorrhoid in a woman may be thus cured. Having fomented with plenty of hot water, boil in the water certain of the fragrant medicines, add pounded tamarisk, roasted litharge and galls, and pour on them white wine, and oil, and the grease of a goose, pounding all together. Give to use after fomenting. In fomenting the anus is to be made to protrude as much as possible.

Hemorrhoids are swollen veins around the anus and rectum. The anus is the opening at the end of the rectum. Bowel movements pass through the anus. Hemorrhoids can be annoying. They can be itchy and painful, or may crack and bleed. Even though hemorrhoids can be quite uncomfortable, they are not a serious problem. Common causes of hemorrhoids are:
  • chronic constipation or diarrhea sitting for a long while pregnancy and childbirth anal intercourse and heredity when parents or other family members have had hemorrhoids
There are two types of hemorrhoids, internal and external. Internal hemorrhoids are inside the anus. The most common symptom of internal hemorrhoids is bright red bleeding. There may be bright red blood on the toilet paper or in the toilet. An internal hemorrhoid may become pushed out of the anus while having a bowel movement. This will usually go back by itself. If it happens often, it may be necessary to push it back. After a long time of this happening, it can come out, and stay out. When this happens, there may be a constant dull ache and itching or bleeding. External hemorrhoids are outside the anus. They may be very painful. Sometimes a hard lump forms near the anus called a thrombosed hemorrhoid. It is very painful. Often the healthcare provider can make a small cut in the hemorrhoid, and drain it. This can be done in the office. Once this is done, the pain goes away.

Hemorrhoids
Description Hemorrhoids are dilated or bulging veins in the rectum and/or anus caused by increased pressure in the rectal veins. Factors contributing to this increased pressure include normal upright posture, straining during bowel movements, prolonged sitting, constipation, diarrhea, pregnancy, trauma, and obesity. Internal hemorrhoids form inside the rectal canal and may or may not protrude through the anus. External hemorrhoids form in the mucus membrane lining just beyond the rectal canal at the anus. Symptoms The most common symptoms of hemorrhoids are pain with bowel movements, incomplete bowel movements, rectal itching, a soft lump felt at the anal opening, or a small amount of blood on the stool or toilet paper after wiping. Occasionally, hemorrhoids are completely without discomfort and are found during a routine examination by a clinician. Occasionally, hemorrhoids may become thrombosed (contain clotted blood) causing increased pain, difficulty sitting and may necessitate opening the hemorrhoid and removing the clot. Bleeding from the rectum should be reported to your clinician at all times. Most often, it is a sign of hemorrhoids and/or cracks in the anal mucosa called a "fissure." There are, however, several more serious causes of rectal bleeding including inflammation of the intestines, rectal polyps, and cancer. Rectal or large intestine cancer is most common in the elderly, but 1% of the cases occur in individuals under the age of 30.

Hemorrhoids may also become inflamed or may develop a blood clot (thrombus). Both internal and external hemorrhoids may remain in the anus or protrude outside the anus. Increased pressure in the veins of the anorectal area leads to hemorrhoids. This pressure may result from pregnancy, from frequent heavy lifting, or from repeated straining during bowel movements (defecation). Constipation may contribute to straining. In a few people, hemorrhoids develop from increased blood pressure in the portal vein. A doctor can distinguish the dilated, twisted veins that occur in this condition from common hemorrhoids. Hemorrhoids can bleed, typically after a bowel movement, producing blood-streaked stool or toilet paper. The blood may turn water in the toilet bowl red. However, the amount of blood is usually small, and hemorrhoids rarely lead to severe blood loss or anemia.
