Frequently Asked Questions - Viral Hemorrhagic Fevers
What are viral hemorrhagic fevers (VHF)?
They are infections caused by a viruses (germs) that are common in Africa and South America, but very rare in the United States.
Is VHF spread from person-to-person?
Yes. These infections are commonly spread from person to person by contact with infected blood and other infected body fluids such as urine, feces, vomitus, and droplets coughed into the air by the infected person.
How soon will symptoms develop (incubation period)?
Normally symptoms start 5 days or longer after exposure to the germ. Not all persons exposed to the germ will develop infection.
What are the symptoms of VHF?
Symptoms include high fever, sore muscles and extreme weakness. The eyes may become red and the skin may appear to be red (flushed). In the advanced stages of the infection there may be bleeding from the nose, mouth, bowel or bladder.
How is the infection treated?

What is Ebola hemorrhagic fever? Electron micrograph of Ebola virus. Ebola hemorrhagic fever (Ebola HF) is a severe, often-fatal disease in humans and nonhuman primates (monkeys, gorillas, and chimpanzees) that has appeared sporadically since its initial recognition in 1976. The disease is caused by infection with Ebola virus, named after a river in the Democratic Republic of the Congo (formerly Zaire) in Africa, where it was first recognized. The virus is one of two members of a family of RNA viruses called the Filoviridae. There are four identified subtypes of Ebola virus. Three of the four have caused disease in humans: Ebola-Zaire, Ebola-Sudan, and Ebola-Ivory Coast. The fourth, Ebola-Reston, has caused disease in nonhuman primates, but not in humans. Where is Ebola virus found in nature?

It causes shock and severe bleeding abnormalities. Causes, incidence, and risk factors Return to top Ebola hemorrhagic fever (Ebola fever) is a viral disease that is limited to parts of Africa. It has been newsworthy worldwide because of its destructive potential. The exact mode of transmission is not understood. The incubation period appears to be up to 1 week, at which time the patient develops fatigue malaise , headache, backache, vomiting, and diarrhea. Within a week, a raised (papular) rash appears over the entire body. The rash is often hemorrhagic (contains blood). Hemorrhaging generally occurs from the gastrointestinal tract, causing the patient to bleed from both the mouth and rectum. Mortality is high, reaching 90%. Patients usually die from shock rather than blood loss.

Ebola Hemorrhagic Fever
  • Ebola hemorrhagic fever is caused by several Ebola viruses. The source of these viruses in nature is not known. People can get Ebola hemorrhagic fever by direct contact with virus-infected blood, body fluids, organs, or semen. There is no known cure or treatment. Recent outbreaks in humans have occurred in areas where medical supplies and care were inadequate. The outbreaks were controlled by using barrier nursing techniques. Under normal circumstances, travelers are at low risk of getting the disease. To eliminate the risk, travelers should avoid areas where Ebola outbreaks are occurring.
What is Ebola hemorrhagic fever Ebola hemorrhagic fever is one of the deadliest of a group of diseases called viral hemorrhagic fevers. They range in seriousness from relatively mild illnesses to severe and potentially fatal diseases. All forms of viral hemorrhagic fever begin with fever and muscle aches. Depending on the virus, the disease can get worse until the patient becomes very ill with breathing problems, severe bleeding (hemorrhage), kidney problems, and shock. Viral hemorrhagic fevers are caused by viruses from four families: filoviruses, arenaviruses, flaviviruses, and bunyaviruses. The usual hosts for most of these viruses are rodents or arthropods (such as ticks and mosquitoes). In some cases, the natural host for the virus is not known.

The clinical presentations of these diseases are nonspecific and variable, making diagnosis difficult. It is noteworthy that not all patients will develop hemorrhagic manifestations. Even a significant proportion of patients with Ebola virus infections may not demonstrate clinical signs of hemorrhage.83 Laboratory abnormalities include leukopenia (except in some cases of Lassa fever), anemia or hemoconcentration, and elevated liver enzymes; DIC with associated coagulation abnormalities and thrombocytopenia are common. Several different viruses can cause a hemorrhagic fever syndrome and hence are designated as hemorrhagic fever viruses.
  • All possess single-stranded RNA (which requires reverse transcriptase for multiplication or amplification by polymerase chain reaction [PCR]) All possess a lipid envelope
Hemorrhagic fever viruses belong to four taxonomic families:
  • Filoviridae Arenaviridae Bunyaviridae Flaviviridae

Ebola hemorrhagic fever Ebola hemorrhagic fever is a life-threatening viral infection that is generally acquired by direct contact with the blood or body fluids of infected persons. Outbreaks have been reported from Uganda, Gabon, the Sudan, and the Democratic Republic of the Congo. A single case has also been reported from Cote d'Ivoire. A related virus, called Ebola-Reston, has been isolated from monkeys imported from the Philippines to the United States, but has not been shown to cause clinical illness in humans, i.e. blood tests show that humans have been infected with Ebola-Reston, but they don't become sick. The incubation period of Ebola hemorrhagic fever ranges from 2 to 21 days, most often 4 to 10 days. Initial symptoms include fever, muscle aches, headaches, fatigue, and sore throat, followed by rash, vomiting and diarrhea. Complications include hemorrhage, dehydration, dangerously low blood pressure, liver impairment, and kidney failure. There is no treatment except for aggressive supportive measures, such as intravenous fluids, transfusions, etc. Suspected cases should be strictly isolated, including both respiratory and body fluid precautions. Except for health care personnel and relief workers, travelers are generally at low risk for infection, because close contact with those who are infected or their remains is usually necessary for transmission. On March 21, 2005, the Centers for Disease Control and Prevention (CDC) in Atlanta confirmed the presence of Marburg virus in 9 of 12 patient samples of tissue and blood sent for analysis by health authorities in Angola. The samples came from a growing number of patients, almost all of them linked to a single pediatric ward in the main hospital in Uige Province, who were rapidly dying from an unknown disease with hemorrhagic manifestations. The identity of the causative agent came as a surprise: Marburg hemorrhagic fever is an exceedingly rare disease, and there had never been a case before in Angola. Diagnostic confirmation set in motion a large-scale international response that began the day after the CDC's finding. Both the features of Marburg hemorrhagic fever and the conditions in Angola have made this response an extreme test of the national and international capacity to hold a dangerous emerging disease at bay.
