Hemolytic uremic syndrome: Events of the past decade PN McLaine This article describes the birth of the Canadian Pediatric Kidney Disease Research Centre (CPKDRC) in 1985 and the activities that have transpired as a result of collaborative research at paediatric centres across Canada. These include the National Retrospective Study of Childhood Hemolytic Uremic Syndrome (HUS), National Prospective Study of Risk Factors for Developing Escherichia coli O157:H7 Infection, and Intervention Studies for the Prevention of HUS. A look to the future describes possible studies to determine potential factors (surrogate markers) to identify children who are at risk for developing HUS following verotoxin-producing E coli gastroenteritis, other intervention studies and a more accurate understanding of permanent renal insufficiency in children who have had HUS.

Barbara G. Delano, MD Dr. Delano is Professor of Clinical Medicine at the State University of New York (SUNY)-Health Science Center at Brooklyn, Brooklyn, New York. KEY WORDS: hemolytic uremic syndrome, infectious diseases The hemolytic uremic syndrome is a severe renal disease associated with hemolytic anemia, thrombocytopenia, and uremia. In this condition, aggregates of platelets reversibly obstruct the arterioles and capillaries, producing ischemia and sometimes infarction predominately, but not always exclusively, in the kidneys.2 This results in acute renal failure. The causes of HUS are shown in Table I .3 It has been estimated that 90% of children with HUS have evidence of the toxin-associated type, and this review will be limited to this entity. The syndrome is typically associated with diarrhea and occurs most frequently in early childhood. The usual clinical picture is one of initial gastrointestinal symptoms consisting of abdominal pain, vomiting, and diarrhea, which is usually bloody. After a prodroma of 1 to 14 days, oliguria occurs with azotemia. The patients also develop microangiopathic hemolytic uremia, with hemoglobin concentrations as low as 3 g/dl. Thrombocytopenia occurs but is usually not severe. The children may also have neurologic symptoms. In general, the longer and more severe the diarrheal phase, the worse the outcome. The acute mortality rate is under 10%.4 Perhaps the most newsworthy outbreak of this disease was reported in Morbidity and Mortality Weekly Report (MMWR) in 1993,5 when there was an outbreak of more than 600 cases of acute gastroenteritis and 501 cases of culture-confirmed E. coli 0157:H7. Three hundred and seventy-one cases were in children, 11% of whom developed HUS. Fourteen of those patients had significant gastrointestinal complications, and the acute mortality rate was 8%. Most of the cases (89%) were associated with eating in a fast-food restaurant chain, where E. coli was isolated from hamburger patties. Within 5 days, the Washington State Department of Health had identified the origin of the outbreak and removed the unsold contaminated meat. Eleven lots of ground beef produced in a single meat-processing plant in California were implicated as the source of the infection.6

Hemolytic-uremic syndrome (HUS) and thrombotic thrombocytopenic purpura (TTP) are microangiopathic disorders — that is, they are characterized by abnormalities (chiefly blood clots) that occur within the small blood vessels of the body. Both HUS and TTP are distinguished by blood clots within the capillaries and arterioles of many organs. Such clotting is associated with hemolytic anemia (low red blood cell count due to cell rupture) and low numbers of platelets (cell-like bodies responsible for blood coagulation). Hemolytic anemia results from the fragmentation of the red blood cells when they pass through areas of thrombi (masses or clots) or turbulence in the circulation. Such forces shear the cells in half, producing cell remnants that appear as helmets and other odd shapes when viewed under a microscope. In fact, the diagnosis of HUS-TTP is aided by microscopic examination of the blood for sheared red blood cells.

Causes, incidence, and risk factors: This disorder is most common in children. It frequently occurs after a gastrointestinal (enteric) infection, often one caused by a specific E. coli bacteria (Escherichia coli O157:H7). It has also been associated with other enteric infections including Shigella and Salmonella and some non-enteric infections.
The incidence of HUS, once relatively rare, is increasing in children. It is the most common cause of acute renal failure in children. Several large outbreaks in 1992 and 1993 were attributed to undercooked hamburger contaminated with E. coli. Because of this association new labeling will appear on supermarket hamburger and temperature guidelines have been published for cooked hamburger at fast food chains and restaurants.

Hemolytic-uremic syndrome (HUS)
Definition
HUS is a disorder marked by kidney failure hemolytic anemia thrombocytopenia (platelet deficiency), coagulation defects, and variable nervous system signs.

Causes
This disorder is most common in children. It frequently occurs after a gastrointestinal (enteric) infection, often one caused by a specific E. coli bacteria ( Escherichia coli O157:H7). It has also been associated with other enteric infections including Shigella and Salmonella and some non-enteric infections.
HUS, once relatively rare, is increasing in children. It is the most common cause of acute kidney failure in children. Several large outbreaks in 1992 and 1993 were attributed to undercooked hamburger contaminated with E. coli . Because of this association, supermarket hamburger has new labeling, and there are new temperature guidelines for hamburger cooked at fast-food chains and restaurants.
HUS is less common in adults. Most adult cases have been reported in patients with cancer who have received the chemotherapy drug 5-FU.
