    One Side of Body Larger Than the Other
    Dear Dr. Warren: My daughter is 7 years old. When she was three I noticed while she was sleeping that her one leg was slightly larger than the other. After the years passed it turned out her entire one side was larger including her ear. We've seen various specialist which say she doesn't have any symtoms as being any type of syndrome. We now see a lymph specialist annually to have a bone scan to measure out the difference in growth. The specialist now says there's not much we can do but have her growth in the large leg stunted so the other catches up. She's very tiny for her age. She presently weighs only 37 pounds and is 45 inches tall. She is extremely bright and she is academically excelling in all subjects in school. I'm just afraid that we haven't done enough to find out what this really is. Could you give me any advice or should I just wait it out til she is ten for possible surgery to stunt the growth of her leg?

    Thank you for enquiring about hemihypertrophy, which is a rare congenital condition that is found either as an isolated feature, or associated with one of a number of so called 'overgrowth syndromes'.
    Hemihypertrophy is an example of Hemi-asymmetry, where part or all of one half of the body is enlarged, affecting multiple tissues on a primary basis. Additional features vary according to which overgrowth syndrome is present. One such example is Beckwith-Wiedemann syndrome, a congenital overgrowth syndrome with variable expression, that occurs with an incidence of 1:13,700 births. Most cases occur sporadically, but approximately 15% are familial, hemihypertrophy occuring in around 24% of cases. The major features are abdominal wall defects, enlarged tongue (macroglossia), and large birth weight or postnatal growth and less commonly low blood sugar in infancy, organ enlargement, congenital kidney anomalies and a predisposition to the development of tumours affecting embryonic tissue.

    Answer
    In some children there is only partial enlargement of, for example, the foot or below the knee, but in others there is an overgrowth of the whole of one side of the body involving the leg, abdomen, chest, arm and face.
    Often no cause for the condition can be found, but because in some cases the overgrowth of the body tissues can be due to some kind of tumour, children with hemihypertrophy will usually have investigations to try and detect any such underlying problem. Hemihypertrophy is sometimes associated with benign (non-cancerous) tumours of blood vessels or lymph vessels or with a Wilm's tumour which is a form of kidney cancer found in children.
    As your son is already eight, and 90 per cent of Wilm's tumours occur in children younger than seven, this is unlikely to be the underlying cause of his hemihypertrophy. You ask what the outlook is for him. It is always difficult to predict the future but apart from being larger than usual the tissues on the overgrown side of his body should be structurally normal and should continue to function in a normal way throughout his life. The difference in size between the two sides of his body may well remain constant as he grows, although in some children it may become more marked when they go through puberty.

The distinction from normal is important as some children with congenital hemihypertrophy have an increased risk of malignant abdominal tumours, most commonly Wilms. Other tumours include adrenal carcinoma and hepatoblastoma. Acquired hemihypertrophy associated with injury, infection, radiation or inflammation is not associated with an increased risk of malignancy. Hemihypertrophy is rarely apparent at birth but develops as the child grows. Total hypertrophy involves all systems: muscular, vascular, skeletal, neurologic and lymphatic. Ipsilateral paired internal organs are also affected. Limited forms only affect certain somatic structures. Non-syndromic hemihypertrophy is associated with anomalies, particularly genitourinary abnormalities. The presence of cutaneous and vascular lesions or other clinical features suggests a syndrome such as neurofibromatosis Fig.1

It is a congenital malformation, presumably related to an underlying genetic mutation. An increased risk of childhood tumours is seen in patients with hemihypertrophy. This increased risk is also thought to be genetically mediated. These patients have an increased risk of Wilms tumour , adrenal cortical carcinoma and hepatoblastoma . Of note, the side of hypertrophy and the side of the tumour are not necessarily related. Hemihypertrophy may be detected on excretory urography, ultrasound, CT, and MRI. The renal anatomy is preserved, the number of calyces is not increased, and excretion is normal. An enlarged kidney is often the only radiological manifestation in the absence of associated tumours.
