Peer Review Status: Internally Peer Reviewed Blood in the urine should never be ignored. This could be the first sign of a serious condition. In order to help your doctor with the correct diagnosis, you may try to provide details such as: Was it associated with pain? Did you see blood clots? What shape did the clots have? The color of the blood (brown, cherry, bright red, pink or orange) At what time during urination did you see blood in the urine (beginning or end of stream or during entire stream)? When passing blood in urine is associated with pain, especially abdominal pain, it could be associated with kidney or ureteral stones. The pain could be localized to the back, to the side, to the groin, or the penis in men and the labia in women. If you are passing different-shaped clots in your stream, they could represent bleeding from the urethra or prostate (in men). Clots can be wormlike, and if associated with pain it could represent clots coming from your ureters(tubes from your kidneys to your bladder). The color of the urine can be also affected by certain foods or medications. However, you should always consult your doctor if this develops.

"What does blood in the urine from the urinalysis indicate? You can't really see in urine, but it is detected through the urine test. Background is a 42 year old, diabetic, who gets very sick when ill because of diabetes" J.E. Blood in the urine that cannot be seen with the naked eye and is only detected through urine dip strip chemical tests or microscopic analysis is called microscopic hematuria. The most common cause of microscopic hematuria is simply unknown and not a cause for concern if major problems have been ruled out. It is estimated that possibly up to 20% of adult men and postmenopausal women have microscopic hematuria ( ). The cause for the microscopic bleeding may only be found in as few as 30-40% of patients undergoing a full urologic work up ( Microscopic hematuria is defined as more than 3 red blood cells per high power microscopic field in a centrifuged urine specimen ( ). Routine urinalysis to check for hematuria in an asymptomatic patient during an annual routine exam is not recommended. This is because of the very high incidence of false positives for serious problems and because the work up includes several expensive and slightly risky procedures (

Hematuria (Blood in Urine)
What is hematuria?
Hematuria means blood in the urine. Microscopic hematuria means that the blood is seen only when the urine is examined under a microscope. Gross hematuria means that there is enough blood in the urine to be seen without a microscope. It causes the urine to look pink, red, or sometimes brown. Certain kinds of foods, such as beets or blackberries, may give the urine a reddish tint. This should last only for a day or so after eating these foods. A few medicines may also turn the urine reddish. If you have started a new medicine and notice a color change in your urine, call your pharmacist to see if that is normal. If the redness persists and cannot be explained by food or medicine, consult your health care provider promptly.

    • Initial diagnosis is by microscopic examination of a properly collected urinalysis. Proper collection is defined as a mid stream specimen in a patient who has avoided strenuous physical exercise or instrumentation for at least 48hrs. Women should not be menstruating. Chemical evaluation of hematuria (dipstick Urinalysis) correlates poorly with quantitative evaluations of formed elements (RBC/HPF) and can detect hematuria down to physiologic normal limits. A properly performed urinalysis is one that has been performed by a standardized technique that gives reproducible results.
