R Hunt, ABR Thomson, Consensus Conference participants These guidelines were created to dispel confusion and provide guidance about how the isolation of Helicobacter pylori infection has led to new opportunities and initiatives to improve patient care. The guidelines are designed for practical application in management decisions, but must remain flexible and amenable to change with new information. Updated versions of the recommendations are anticipated. What is a peptic ulcer?
A peptic ulcer is a sore on the lining of the stomach or duodenum, which is the beginning of the small intestine. Peptic ulcers are common: One in 10 Americans develops an ulcer at some time in his or her life. One cause of peptic ulcer is bacterial infection, but some ulcers are caused by long-term use of nonsteroidal anti-inflammatory agents (NSAIDs), like aspirin and ibuprofen. In a few cases, cancerous tumors in the stomach or pancreas can cause ulcers. Peptic ulcers are not caused by stress or eating spicy food, but these can make ulcers worse. Top

Focus on H. pylori
Helicobacter pylori is a bug in the news! It has been implicated as one causative agent in gastric and duodenal ulcer formation, and as one factor involved in gastric cancer. New diagnostic tests for H. pylori antibodies are becoming available, and drug therapies for eradication are being extensively tested.
Bandolier does not have the resources to cover all these issues in depth, but the focus on H. pylori this month will give the reader a strong feel of what is coming.
H. pylori and ulcers
In 1975 it was estimated at about 4,000,000 Americans had gastric or duodenal ulcers with a total estimated cost of $3.2 billion per year. While gastric ulcer, duodenal ulcer and gastrointestinal inflammation are clearly separate disease entities, they share a common pathophysiology - an imbalance between mucosal aggressive and defensive factors.
In the late '70s a pathologist (J R Warren) from the Royal Perth Hospital in Western Australia described finding spiral bacilli in biopsy specimens from inflamed gastric mucosa. In 1982 the organism was isolated by Barry Marshall, also at Perth. Marshall elegantly fulfilled Koch's postulates for the role of H. pylori in antral gastritis with self administration of H. pylori. The last decade has seen a considerable effort, not without controversy, which has brought H. pylori to the forefront as a major causative agent in gastric and duodenal ulceration.

For an upset stomach, I have found that taking acid, not an antacid, is helpful now that I'm in my older years (I am 85). To quote an early issue of Prevention magazine, "Research has shown that we produce less and less hydrochloric acid as we age. One of the biggest downsides of this is that protein requires a lot of hydrochloric acid to be broken down, and if protein isn't broken down completely, you will have burping and gas after eating; bloating, and heartburn." Additionally, I read that an important cause of calcium deficiency is inadequate absorption of calcium and that calcium needs acid to be absorbed. Since older people have less stomach acid, isn't it counterproductive to take calcium-containing antacids as a calcium supplement? I take 1 teaspoon of cider vinegar after meals, and I have had great success. Why isn't this approach promoted?

August 2, 2002 Scientists have discovered a protein that plays a key role in one of the most common bacterial infections worldwide and that may be a candidate for a vaccine. The protein, called SabA, allows the bacterium H. pylori to use the body's immune system to its own advantage. Its presence may explain how the pathogen causes such a persistent infection.

More than half of the world's population is infected by H. pylori , which is a leading cause of ulcers and a risk factor for stomach cancer. Especially prevalent in developing countries, infections typically start in infancy and can last for decades. A team of researchers from Sweden, Estonia, and the United States reports in Science that the H. pylori pathogen uses SabA to microscopically 'grab' onto special sugars on stomach cells. Cells bring these sugars to their surfaces to attract immune cells. Their purpose, explains researcher Thomas Borén, is "to signal to the body that part of the tissue is in need of help." Because of SabA, however, the sugars actually make the infection worse.
