Cochrane reviews are regularly checked and updated if necessary. Background: Helicobacter pylori (H pylori) is the main cause of peptic ulcer disease. The role of H pylori in non-ulcer dyspepsia is less clear. Objectives: To determine the effect of H pylori eradication on dyspepsia symptoms in patients with non-ulcer dyspepsia. Search strategy: Trials were identified through electronic searches of the Cochrane Controlled Trials Register (CCTR), MEDLINE, EMBASE, CINAHL and SIGLE, using appropriate subject headings and keywords, searching bibliographies of retrieved articles, and through contacts with experts in the fields of dyspepsia and with pharmaceutical companies. These searches were updated in October 2004. Selection criteria: All parallel group randomised controlled trials (RCTs) comparing drugs to eradicate H pylori with placebo or other drugs known not to eradicate H pylori for patients with non-ulcer dyspepsia.

Back to main page Helicobacter pylori is stop-the-presses, front-page headline news for smokers, and a disaster in the making for the anti-smoking movement. It is a bacterial infection of the stomach lining beneath the gastric mucosa. The National Institutes of Health (NIH) 1994 Consensus Statement declared that most ulcers are caused by H pylori. The International Agency for Research on Cancer (IARC) now considers HP a major cause of stomach cancer. H pylori is also linked to primary gastric lymphoma (Bayerdorffer E, et al. Lancet 1995 Jun 24;345(8965):1591-1594), and to acne rosacea (Parish LC, Witkowski JA. Int J Dermatology 1995 Apr;34(4):236-237). H pylori infection is more common in the lower socioeconomic class in developed countries. Nearly all of those infected acquired it in very early childhood, before they were smokers, which invalidates speculations that smoking increases susceptibility to it. Smokers are more likely to be in the lower socioeconomic classes, and thus are more likely to be infected, and to suffer from HP-associated diseases. The anti-smoking movement exploited this association for 30 years to falsely attribute diseases to smoking which were actually caused by Helicobacter pylori.

Panorama Helicobacter pylori Gene Array Helicobacter pylori is a gram-negative, micro-aerophilic bacterium, that has been shown to be associated with gastric inflammation and peptic ulcer disease. As a human pathogen, H. pylori’s presence in the gastric mucosa is associated with gastritis and is often implicated in peptic ulceration and mucosa-associated lymphoid tissue lymphomas. H. pylori infects approximately half of the world’s population, possibly distinguishing it as the most prevalent chronic bacterial pathogen in humans. What is Helicobacter pylori?
Helicobacter pylori (H. pylori) is a bacterium (germ) that lives in the inner lining of the stomach. It produces a number of chemicals that damage the lining of the stomach. Once the infection is present, it persists for many years, if not for life.
How common is Helicobacter pylori infection in Australia?
About 40% of persons over 40 years of age are infected with this germ compared to less than 10% of children. Most people are infected in childhood and in Australia the risk of becoming infected is now much less than in the past. There is no difference in infection rate between men and women.
H. pylori is more common in older people and in certain ethnic populations (eg. Middle Eastern, Asian and Eastern European). In general the germ is more common in people from a poorer background, people living in institutions (eg. mentally handicapped) and those who have migrated from countries where infection is more common, especially the developing world. However, any individual may be infected.

5. Summary of Data Reported and Evaluation
5.1 Exposure data
Helicobacter are spiral, flagellated, gram-negative bacteria that colonize the gastrointestinal tract of human beings and animals. H. pylori is restricted to human gastric mucosa and can infect some other primates. H. pylori strains are genetically heterogeneous, and this attribute is useful in studies of transmission. H. pylori can be cultured, is sensitive to most antibiotics in vitro and is characterized by very strong urease activity. Colonization of the gastric mucosa and subsequent development of gastritis are dependent on bacterial factors, including motility, potent urease activity and specific adherence to gastric epithelium. H. pylori can be detected in gastric biopsy specimens and indirectly by serology and analysis of breath after ingestion of labelled urea. Standard histological and bacteriological techniques, the polymerase chain reaction and indirect tests are highly sensitive. The rapid urease test on biopsy specimens is practical but less sensitive. Epidemiological studies currently involve use of serological tests and mainly commercially available enzyme-linked immunosorbent assay kits. H. pylori
