  • Gastroesophageal reflux disease (GERD) is a digestive disorder. It is also called heartburn and acid indigestion. It is the backward flow of acid from the stomach up into the esophagus.
What causes heartburn?
  • The digestive system turns food into energy and removes waste from the body. It is made up of the mouth, esophagus (tube to the stomach), the small and large intestine, and the rectum. In normal digestion, a muscle called the LES (see figure below) blocks the esophagus so acid from the stomach can't go back up into the throat. Heartburn happens when the muscle doesn't work right and lets acid from the stomach back up into the throat. Infants and children can get heartburn if their digestive system is not working right yet.

Heartburn
Overview
Signs and Symptoms
  • Burning pain or discomfort in your upper abdominal area or behind your breast bone. Sour, bitter or acid taste in your mouth (especially after belching). Nausea/vomiting in more severe cases. Chronic cough or wheezing, particularly at night.
Self-Care Measures
  • Take OTC antacids liquids or tablets before and after meals and at bedtime. Take an OTC acid reducer. Wait 2 to 3 hours after you eat to go to bed or lie down. Avoid acidic foods like onions and citrus fruit/juices, foods known to relax the esophageal sphincter such as peppermint, caffeinated foods/drinks, and spicy foods. For fever or pain relief, use an OTC analgesic with acetaminophen only to minimize stomach irritation. Minimize or eliminate the use of nicotine and alcohol. Eat smaller, more frequent meals or snacks versus 2 or 3 large meals.

Feel The Burn: The Lowdown On Heartburn
By Joshua Levine
Health Correspondent - Every 2nd Saturday
The game is over and the boys have just left. As you get up to clear the empty buckets of Buffalo wings from the coffee table, it hits a sharp burning sensation up and down your chest and a sour taste at the back of your throat. Well say hello to heartburn; the post-meal killer that can floor you for hours. You may have enjoyed dinner, but now you have to pay the price and squirm in bed for most of the night. what is heartburn?
Frequently experienced after eating heavy, spicy meals, heartburn is often characterized by discomfort or pain in the upper abdomen or right below the ribs, accompanied by a fiery feeling in the chest and acidic taste in the back of the throat. It is usually caused by the regurgitation or reflux of stomach contents (acid and bile) through the esophagus because the sphincter doesn't close completely. The esophagus is not able to withstand the stomach acid that comes back up, hence the burning sensation. Note that contrary to the name, heartburn actually has nothing to do with your heart; it's a digestive problem. It is important to differentiate heartburn from ulcers and a rupture in the esophagus. The pain felt is relatively milder than the latter two. If your suspected heartburn occurs more than twice a week, seek medical attention for a proper diagnosis; keep in mind that heartburn can be a sign of something more serious.

AUSTRALIAN GASTROENTEROLOGY INSTITUTE
(educational arm of the Gastroenterological Society of Australia)
Website: www.gesa.org.au ... Copies reviewed November 2001 HEARTBURN - A Guide to Reflux Disease What is reflux disease? Gastro-oesophageal reflux disease is a common cause of indigestion. It is caused by the washing back (reflux) of food and stomach acid into the gullet (oesophagus ). This occurs because a muscular valve (sphincter) at the junction of the oesophagus and stomach fails to work properly. What are the symptoms? The stomach contents irritate the oesophagus and cause a painful burning sensation in the chest rising up to the throat (heartburn). Sometimes this is accompanied by a bitter taste in the mouth. These symptoms typically occur after food. Large meals and fatty or spicy foods are most likely to cause problems. Lying down or bending over may cause or worsen symptoms. Stomach contents may rise as far as the throat (often described as repeating) and be experienced as regurgitation. When severe, it may resemble vomiting. Occasionally they may irritate the breathing passages and cause cough, hoarseness, sore throat and asthma. If this happens at night it may cause awakening with choking attacks.

General Health Information

There are two kinds of peptic ulcers: gastric ulcers and duodenal ulcers. Gastric ulcers occur in the stomach, and duodenal ulcers occur in the first part of the small intestine. Even though these ulcers occur in two different places, they are both caused by the same thing…damage to the protective lining of the stomach or small intestine. Ulcer pain is caused by stomach acid coming in direct contact with unprotected tissue. When stomach acid is suppressed, ulcer pain diminishes, and the ulcer heals. It is important to keep the lining of the gastrointestinal tract healthy and intact. Stress, smoking, alcohol, aspirin and NSAIDS can all bring on the formation of ulcers. Stress-management techniques, a healthy lifestyle, and the avoidance of irritants all help keep the mucosal lining of the gastrointestinal system healthy and intact. Vitamins A and E and the amino acid glutamine are important in maintaining the integrity of the mucosal lining. Licorice, especially in its DGL form, also supports mucosal protective factors. Ulcers may also be caused by the bacteria helicobacter pylori. Predisposing factors for infection are low acidity and low antioxidant content of the gastrointestinal lining. H. pylori can also infect a pre-existing ulcer site. Bioflavonoids can inhibit the growth of H. pylori.

The Burning Truth About Heartburn Certain foods and beverages, as well as lifestyle factors such as too much stress and not enough exercise can cause heartburn. According to the American Gastroenterological Association (AGA), more than 60 million American adults experience heartburn at least once a month, and about 25 million adults suffer daily from heartburn. Heartburn is caused by acids backing up from the stomach into the esophagus - the tube that food passes through on its way from the throat to the stomach. The "burn" in heartburn is caused by the irritation of the acids on esophagus tissues. Unlike the stomach, the esophagus is not protected from these acids by a thick lining of mucous. A muscle, the esophageal sphincter, which closes to prevent the back up of stomach acids, protects it. When this muscle relaxes, digestive juices and stomach acids may travel into the esophagus. Usually these acids flow back into the stomach after a short time, aided by the flow of saliva (a natural antacid), gravity and the regular contractions of the esophagus. If the juices remain pooled in the esophagus, heartburn occurs. (AGA/Gallup Survey, Understanding Heartburn In America, 5/00)

Natural Remedies for Heartburn Supplements to Ease the Burning
We all get heartburn now and then. But for some 25 million Americans, heartburn is a regular occurrence that is more than a daily annoyance. Researchers at the Karolinska Institute in Sweden report in the New England Journal of Medicine (3/18/99) that if left untreated, chronic heartburn increases the risk of esophageal cancer eightfold; this is an especially disturbing finding given the deadliness of that form of cancer. A number of potent prescription and over-the-counter heartburn medications are available, and they may be effective. Safe natural remedies and simple changes in diet and lifestyle can also provide welcome relief. Helpful supplements
The supplements below can be very effective for relieving heartburn. Some (calcium carbonate, licorice, aloe vera juice, for example) are recommended for immediate relief of symptoms. To improve the entire digestive process, try either gamma-oryzanol, a natural extract from rice bran oil, or a combination of choline, pantothenic acid, and thiamin. It may take a month or so for these longer-acting supplements to take effect. Calcium carbonate
A well-known heartburn reliever, calcium carbonate is the principal active ingredient in antacid tablets such as Tums. It can provide immediate relief and may be particularly effective for treating sporadic heartburn. However, avoid brands that include peppermint, which has been shown to relax the low esophageal sphincter (LES), the muscle at the base of the esophagus that normally keeps food and digestive acids in the stomach. A weakened LES can lead to heartburn.

Dealing With Heartburn
For example, FDA says some heartburn drugs interact with other medications. (Read about " Drug Interaction Precautions ") Others should be avoided by those with certain medical conditions, such as high blood pressure. (Read about " Hypertension: High Blood Pressure ") Therefore, it's important to read the label and consult with your healthcare provider before taking these medications, especially if you find the symptoms of heartburn are recurring regularly. Constant heartburn can be an indication of a much more serious condition called gastroesophageal reflux disease or GERD. (Read about " GERD ") Types of Medications According to the National Digestive Diseases Information Clearinghouse (NDDIC), heartburn medications generally work by either reducing the amount of stomach acid produced by your body or by neutralizing it. Your doctor may recommend over-the-counter antacids, which you can buy without a prescription, or medications that stop acid production or help the muscles that empty your stomach. They can include any of the following:
  • Antacids are usually the first drugs recommended to relieve heartburn, according to NDDIC. Many brands on the market use different combinations of three basic salts - magnesium, calcium, and aluminum - with hydroxide or bicarbonate ions to neutralize the acid in your stomach. Antacids, however, have side effects. Magnesium salt can lead to diarrhea, and aluminum salts can cause constipation. Aluminum and magnesium salts are often combined in a single product to balance these effects. Calcium carbonate antacids can also be a supplemental source of calcium. (Read about "

Pregnancy and Heartburn
Heartburn is a common complaint during pregnancy. Although it has nothing to do with the heart, heartburn involves a burning sensation in the center of the chest.
What causes heartburn during pregnancy?
Heartburn occurs when the valve between the stomach and the food pipe (esophagus) are unable to prevent the stomach acids from passing back into the esophagus. Pregnancy has the tendency to increase the frequency of heartburn because the hormone progesterone causes the valve to relax. This allows the stomach acid to pass into the esophagus and irritate the lining. Heartburn and indigestion are more common during the third trimester because the developing baby tends to put pressure on the intestines and the stomach. The pressure on the stomach may push contents back up into the esophagus.
