Each time you swallow food, it travels down the esophagus to the stomach where it is mixed with stomach acid and other powerful digestive enzymes. The stomach is lined with cells that secrete mucus to protect it from this potentially destructive cocktail. A muscular ring at the base of the esophagus, called the lower esophageal sphincter (LES), relaxes to allow food to pass into the stomach, and then tightens to prevent backflow or reflux of acidic stomach contents into the more vulnerable esophagus. Unfortunately, a number of factors can interfere with this protective mechanism. Reflux is common, and often occurs without causing any symptoms because the body responds by quickly neutralizing and clearing the acidic contents from the esophagus. In some individuals, however, the protective mechanisms are not adequate to meet the challenge for reasons that we don't understand well. In these individuals, inflammation of the esophagus (called esophagitis) will occur and produce the classic symptoms of GERD. What are the symptoms of GERD?

Heartburn, Hiatal Hernia, and Gastroesophageal Reflux Disease (GERD)
On this page: Gastroesophageal reflux disease, or GERD, occurs when the lower esophageal sphincter (LES) does not close properly and stomach contents leak back, or reflux, into the esophagus. The LES is a ring of muscle at the bottom of the esophagus that acts like a valve between the esophagus and stomach. The esophagus carries food from the mouth to the stomach. When refluxed stomach acid touches the lining of the esophagus, it causes a burning sensation in the chest or throat called heartburn. The fluid may even be tasted in the back of the mouth, and this is called acid indigestion. Occasional heartburn is common but does not necessarily mean one has GERD. Heartburn that occurs more than twice a week may be considered GERD, and it can eventually lead to more serious health problems. Anyone, including infants, children, and pregnant women, can have GERD.

Page 2 Page 3 What is the risk of developing esophageal cancer from reflux disease? In the last twenty years the incidence of this cancer has increased faster than any other malignancy. Many experts agree that it is no accident that this has happened since the advent of strong acid suppression drugs. More people are taking more of these drugs and more people are turning up with cancer. Despite this phenomenon the pharmaceutical industry has continued to produce more and stronger acid lowering medications. Scientists in the research of the esophagus have discovered that the longer a patient with GERD has the disease and takes these medications the more likely he is to get cancer. This fact was demonstrated in a study published in the New England Journal of Medicine. We now recognize that acid is not the chemical that causes cancer of the esophagus. This is the reason why acid-lowering medications may make you feel better but may not protect you from the complications of the disease, even cancer.

New warning for heartburn drug Propulsid

Web posted at: 11:15 a.m. EDT (1515 GMT) WASHINGTON (CNN) The Food and Drug Administration is urging doctors to try everything else before prescribing Propulsid, a heartburn drug taken by President Clinton, out of concern the drug may cause serious heart problems. The FDA ordered strengthened warnings for Propulsid Monday after reports of heart rhythm abnormalities, including 38 deaths, since the pill hit the market in 1993. The FDA said it cannot prove the drug caused any deaths but noted that Propulsid already was known to cause irregular heartbeat rhythm, known as heart arrhythmia, when taken together with certain other medicines. Monday's action strengthens those warnings, adding newly discovered drug interactions and a list of other illnesses that increase the risk. Q: What is the best treatment for heartburn? Rose Kassardjian in Whittier, California A: Well, Rose, according to WebMD, 20 percent of adult Americans have heartburn at least once a month. There are many different approaches to treating the feeling of burning, heat or pain in the upper abdomen and throat. For mild heartburn, simple lifestyle changes such as eating smaller meals, limiting spicy or acidic food intake, decreasing pressure on your stomach, losing weight and decreasing stress may help relieve heartburn. For more serious heartburn cases, try nonprescription antacids and acid reducers. Liquid or dissolving antacids usually work faster than tablet forms. Antacids do work faster than acid reducers, but their effect does not last more than one to two hours.

BOSTON (CNN) People who suffer from chronic heartburn and acid reflux are substantially more likely to develop a deadly cancer of the esophagus, according to a new study. Researchers also found that people who take antacids and other medications to battle heartburn and acid reflux do not reduce their cancer risk. Those treatments only mask symptoms and don't prevent or cure the underlying condition. The results of the Swedish study are reported in the latest issue of the New England Journal of Medicine.

Gastroesophageal refers to the stomach and esophagus (or "food pipe"). Reflux means to flow back or return. Therefore, gastroesophageal reflux is the return of the stomach's contents back up into the esophagus. Back to top Q. What causes gastroesophageal reflux disease (GERD)?

In normal digestion, the lower esophageal sphincter (the valve between the esophagus and the stomach) opens to allow food to pass into the stomach and closes to prevent food and acidic stomach juices from flowing back into the esophagus. Gastroesophageal reflux occurs when the lower esophageal sphincter (LES) is weak or relaxes inappropriately allowing the stomach's contents to flow up into the esophagus. The severity of GERD depends on LES dysfunction, as well as the type and amount of fluid brought up from the stomach and the neutralizing effect of saliva.

Heartburn
Heartburn is a discomfort or pain caused by the stomach contents regurgitating from the stomach up into the gullet (oesophagus). The oesophagus is not made to withstand acid and is thus irritated and inflamed when acid from the stomach refluxes into it. This is known as reflux oesophagitis.
Symptoms
The pain caused by the irritation of the oesophagus is a burning pain which you feel in the centre of the chest. There is often an associated awareness of acid and stomach contents in the back of the throat or mouth. Lying down or bending (eg in the garden) tend to increase the symptoms.
Causes
The whole of the intestinal tract is in the form of a tube with muscles in the wall which move the food along (peristalsis). At the bottom of the oesophagus the muscles encircling the tube work rather like a valve to stop food going backwards. This is not a perfect valve, but in some people works less well than others:
  • Obesity leads to a higher pressure in the abdomen.
  • Pregnancy causes a higher pressure in the abdomen and there is also evidence that the changing hormone levels cause relaxation of the muscle valve.
