Halitosis Halitosis, also called bad breath, foul breath, malodor, foetor ex ore, and fetor oris is defined as an offensive odor emanating from the oral cavity. Bad breath is a common pet odor complaint. Causes may be oral (most common) or extraoral (rare). The sour milk odor accompanying periodontal disease may result from the bacterial population associated with plaque, calculus, unhealthy tissues, decomposing food particles retained within the oral cavity, or the rotten meat odor emanating from tissue necrosis. Contrary to common belief, neither normal lung air or stomach aroma contributes to halitosis. The most common cause of halitosis is periodontal disease caused by plaque (bacteria). Bacteria is attracted to the pellicle (an acellular film formed from the precipitation of salivary glycoproteins). In the freshly cleaned and polished tooth a glycoprotein layer forms over the tooth as soon as the patient starts to salivate. Bacteria attaches to the pellicle within 6-8 hours. Within days, the plaque becomes mineralized producing calculus. As plaque ages and gingivitis develops into periodontitis (bone loss), the bacterial flora changes from a predominantly non-motile gram-positive aerobic coccoid flora to a more motile, gram-negative anaerobic population including: Bacteroides, Fusobacterium, and Actinomyces species. Most patients suffering from halitosis have oral causes, the remaining are caused by, dermatologic, metabolic, respiratory, or gastrointestinal disease.

What is bad breath?
The complaint of bad breath is unusual in children and the cause should be uncovered and dealt with directly. When your child's breath has an unpleasant odor it could be caused by food, dental problems, or by another underlying problem. The problem may be recent or a long-standing one.

What happens?
Friends and colleagues usually notice the condition before the individual does. Most sufferers are oblivious to the fact that they have bad breath until a good friend points it out to them. Once aware that they have bad breath they'll often obsessively check their breath, going to extremes to ensure it doesn't smell, even after they've solved the problem.

Halitosis is very common and is usually caused by simple things you can do something about. Some illnesses can cause bad breath, such as kidney or liver failure (in fact, doctors are taught to smell their patients' breath to look out for these), but if you're otherwise well it's unlikely that these are causing your problem. If you're worried, though, check with your doctor. People often laugh at bad breath, but it's a serious problem and has lead to social isolation, divorce and even suicide. Causes of bad breath The bad smell is usually caused by chemical molecules containing sulphur. These are produced when bacteria that live in the mouth break down protein in the diet. Other factors are important too, including:

Brushing and flossing are two of the most crucial elements for attacking bad breath. Bad breath is caused by bacteria which live on our teeth and gums. Halitosis Treatment in the Dental Office
Halitosis, and the treatment of this problem, is quickly becoming the growth area of the 90's in dental practices throughout North America. Oral malodor is a very pervasive problem, and can be simply an embarassment to some, or the sign of potentially serious systemic problems for others. Most of us as dental professionals have received little, if any, formal training in the management of this problem. Professionals are being drawn to establish this treatment service within their offices for several reasons. Perhaps the primary reason is the power of this concept as a marketing tool. It is an excellent way of attracting new patients, particularly those who are not regular users of dental services. The treatment of halitosis is often viewed as a "non-dental" procedure by the public, and so does not carry the psychological impact of other dental treatment. This allows us to get these people comfortable with the dental environment in a non-threatening fashion. Amongst our existing patients it is a strong motivator for the acceptance of dental treatment, particularly in the area of periodontal disease, which is known to be a major source of halitosis. Finally, we now have available the means to quantify this problem, and some definitive means to treat the problem, once properly diagnosed. It is generally known and agreed that VSC's(Volatile Sulfur Compounds) are responsible for the odor, and that 80-90% of these come from oral sources. VSC's are produced by bacterial and cellular degradation, and include hydrogen sulphide, methyl mercaptan, and dimethyl sulphide. The fact that oral odors can originate from sources other than the oral cavity, demands that a proper diagnosis be done, requiring a thorough exam and history. It is known that systemic disorders, medications, and ENT problems can all contribute to halitosis.
