Cutaneous Indications for HIV Testing Oral Hairy Leukoplakia (OHL) Oral lesions may provide the only early indication of HIV infection and are important in the classification of HIV disease. In an otherwise asymptomatic individual, recognition of oral lesions, such as oral candidiasis or oral hairy leukoplakia, may give the first indication of HIV disease. For patients who have documented HIV infection, NIH clinical guidelines suggest that oral examinations should be performed by the primary care provider during every physical examination. Routine oral examinations should include careful inspection of the soft tissues, with particular attention paid to the soft palate and lateral margins of the tongue. Oral hairy leukoplakia (OHL) is a benign oral lesion caused by Epstein Barr virus (EBV). Recognition of OHL in a previously healthy patient should prompt HIV testing. This lesion may be present in more than 50% of HIV-positive homosexuals and bisexuals in the United States. Often, the appearance of this lesion is a harbinger of progressive disease; the average CD4+ lymphocyte count is typically 400 at the time of diagnosis of OHL. Clinically, OHL appears as asymptomatic, hyperplastic, verrucous, white to grayish epithelial plaques on lateral tongue. The patient may report that the lesion changes from day to day, but does not rub off.

Epstein-Barr virus (EBV) is a ubiquitous human herpesvirus that infects nearly every human being by young adulthood. Most EBV infections occur asymptomatically during childhood, but EBV is associated with a wide variety of human diseases including the infectious mononucleosis syndrome, Hodgkin's lymphoma, African Burkitt's lymphoma and nasopharyngeal carcinoma. EBV is also an opportunistic pathogen in immunocompromised patients, associated with lymphoproliferative diseases and oral hairy leukoplakia.

A 32-year-old white man who had tested positive for human immunodeficiency virus (HIV) presented with a two-year history of white papules and plaques on the lateral borders of his tongue (Figures 1 and 2). There were no lesions on the palate, the inner aspect of the cheeks, or the lips. The patient reported a history of smoking for several years. He denied a history of diabetes and was not taking any medications. Attempts to rub off these lesions with a tongue blade were unsuccessful, a potassium hydroxide preparation was negative for hyphae, and a rapid plasma reagin test was nonreactive.

Overview Symptoms Treatment Prevention Definition: A precancerous lesion that develops on the tongue or the inside of the cheek as a response to chronic irritation. Occasionally, leukoplakia patches develop on the female external genitalia.
Alternative Names: Hairy leukoplakia; Smoker's keratosis
Causes, incidence, and risk factors: Leukoplakia is primarily a disorder of the mucous membranes of the mouth, caused by irritation. Lesions usually develop on the tongue, but they may appear on the insides of the cheek.
Irritation may result from rough teeth or rough places on dentures, fillings, crown, and so on. It may also result from chronic smoking or other tobacco use (smoker's keratosis). Pipe smoking involves a high risk for development of leukoplakia, as does holding chewing tobacco or snuff in the mouth for a prolonged period of time.

Hairy Tongue or Hairy Leukoplakia?

This is better known as "black (or brown) hairy tongue," or simply "hairy tongue." This is a descriptive name: Individuals with hairy tongue look like they have been licking Furbies. (That's a children's toy, for you folks out there who don't have kids!)
So ... why does your tongue look hairy? Keratin. Keratin is one of the chief protein constituents of skin, hair, toenails and fingernails. Keratin is found in all epithelial tissues (i.e., not just skin, but also the mucous membranes that line your nose, mouth, tongue, esophagus, stomach and so forth). Hairy tongue is due to an accumulation of keratin within the taste buds on the top surface of the tongue.

Treating oral hairy leukoplakia with high dose valacyclovir

A study of 19 people (73% on HAART) with HIV-associated oral hairy leukoplakia (OHL) and Epstein-Barr virus (EBV) replication found that treatment with high dose oral valacyclovir (Valtrex), one gram every eight hours for a month, could inhibit productive replication of EBV. Walling and colleagues at the University of Texas report that in the majority of cases OHL was resolved and EBV replication was halted. OHL was cleared in 89% of subjects. OHL and EBV replication recurred in two subjects and EBV replication alone was detected in two more subjects. The researchers theorise that either the virus developed resistance and/or subjects did not take their medication as directed. Ref: Walling DM, Flaitz CM and Nichols CM. Glick and co-workers (1) have demonstrated that the presence of OHL in patients infected with the AIDS virus is a trustable marker for the progression of the HIV infection. Greenspan and co-workers (2) as well as Moniaci and coworkers (3) have shown than 40 to 60% of patients infected with HIV convert to AIDS within 2 to 3 years after the development of OHL. Brandwein et al (7) using standard DNA in situ hybridization (ISH) to detect Epstein Barr virus (EBV) in lingual hairy leukoplakia, found EBV in all 11 specimens that were investigated. EBV was found in the basal and parabasal layers and strongly localized in the upper epithelial layers. Additionally these authors reported that in the same 11 specimes there was no evidence of human papilloma virus (HPV) or cytomegalovirus DNA. The authors concluded that there is no evidence to suggest that HPV is involved in the development of hairy leukoplakia. Miranda and Lozada-Nur (8) as well as other authors (9,10) have reported the presence of lingual hairy leukoplakia in HIV-negative immunosuppressed and in immunocompetent patients. These authors comment on the importance of a thorough examination of the oral tissues in patients who are undergoing immunosuppressive therapy.

Oral Hairy Leukoplakia (OHL)
OHL tends to manifest in patients who's CD4+ cell count falls below 300 cells per cubic millimiter. There have been several studies which have demonstrated the importance of OHL as a determinant of the prognosis of AIDS. These studies have shown that patients with OHL have a shorter life span than those that do not present this lesion. This might implicate that the status of the immune system in those patients with OHL is at a lower level than those that do not have OHL. OHL is generally not treated unless it is requested by the patient because of aesthetics, masticatory or phonetic reasons. OHL resolve in patients which undergo treatment with AZT (zidovudine). This 24 year-old man presented these barely visible, vertical striations on both sides of his tongue. Note their almost parallel orientation. These lesions did not rub off. The clinical diagnosis of OHL was established. Additionally, the patient had a large tonsilar abscess and a weight loss of 15 pounds in the last 2 months. He attibuted the loss of weight to change in his diet. The patient denied belonging to any AIDS risk group but a Western blot and ELISA test, when performed, were positive and his CD4+ cell count was 280 cells per cubic millimeter. Then, the diagnosis of AIDS was established.
