Breast Reduction in Men With Gynecomastia
What is gynecomastia?
Gynecomastia is a condition in which the male's breast tissue enlarges. Gynecomastia literally means "woman breast." This increase in tissue usually occurs at times when the male is having hormonal changes, such as during infancy, adolescence, and old age.
What causes gynecomastia?
This is usually a benign (non-cancerous) condition that may be caused by many different hormonal changes, or the cause may be unknown. The changes in hormones usually include an increase in estrogens or a decrease in androgens (testosterone). Sometimes, this condition may be associated with other diseases that must be ruled out in order to make a diagnosis. In some conditions, including breast cancer, liver diseases, lung cancer, testicular cancer, tumors of the adrenal glands or pituitary gland, drug use, certain congenital (present at birth) syndromes, or thyroid disorders or trauma, gynecomastia presents itself as a symptom. However, this is not true in all cases.

Gynecomastia is defined as benign, excessive enlargement of the male mammary gland (breast). HIV-related gynecomastia develops mainly in men with preserved immunological status after years of HAART. Photographs of men affected by this syndrome are posted on the Internet at www.gynecomastia.org
In a recently published Spanish study , researchers describe the clinical features and treatment of 34 HIV-positive men with gynecomastia in what they call "the largest series of gynecomastia reported in HIV-infected patients."
The patients came from a cohort of 1400 HIV-positive men. Of these, 900 were receiving antiretroviral therapy, the authors report, for an incidence of 2.4 cases per 100 patients receiving HAART per year.
In a recent observational longitudinal study , investigators report on five patients diagnosed with gynecomastia associated with efavirenz ( Sustiva )-based HAART regimens. In all five cases, gynecomastia regressed after efavirenz withdrawal (mean period of 5 months).

Your body is changing voice, hormones, overall growth and just when you thought it couldn't get any more awkward, some boys sprout breasts. The medical term is gynecomastia, when a male develops breast tissue, reported WNBC-TV in New York. "Kids, they could be really, like, cruel," said Kyle Rich, 14. "You know, it really bothered me. Yeah, they were really, really not cool at all. And after a while, I started getting an attitude. And I was ready to start swinging. ... They were being really mean, saying some nasty things." The condition can run the gamut from small nubs to moderate, like Rich's, to large, pendulous breasts. While the exact cause is uncertain, it is related to hormones. "Patients who have gynecomastia, it is believed, have tissue which is abnormally sensitive to the small amount of estrogen that they will normally produce during their pubescent years," said Dr. Elliot Jacobs, a New York plastic surgeon.

    Gynecomastia
    Most breast lumps in men are actually a disorder called gynecomastia, which literally means 'female breast'. Usually, a man has very little breast tissue. With gynecomastia, a button-like growth under the nipple and areola area can be felt and is sometimes visible to the naked eye. Occasionally it can be quite tender.
    This increase in the amount of breast tissue is not a tumour and occurs due to changes in hormone balances at times like adolescence and in old age, when the overall level of androgen gradually decreases. It is extremely rare for this condition to progress into a cancer.
    Gynecomastia can also occur in situations where the male body produces more oestrogen, such as diseases of certain endocrine glands.
    Diseases of the liver (important for hormone metabolism) can also alter a man's hormone balance and lead to gynecomastia.
    In some cases, commonly prescribed medications can cause it - such as certain drugs prescribed to treat ulcers, heartburn, high blood pressure and heart failure.

Gynecomastia Gynecomastia may be physiologic, familial, pathologic, drug-induced or, in many cases, of unknown etiology. Breast enlargement is usually unilateral and asymptomatic. Mechanisms of gynecomastia involve increased estrogen stimulation, decreased testosterone levels or a decreased androgen/estrogen ratio. Hyperprolactinemia is not a cause. Treatment of gynecomastia should be directed at the underlying cause when one can be identified. Most cases are benign and can be managed by explanation, reassurance and observation. Gynecomastia may occur normally in the neonatal period, during puberty and in old age. It may also develop in conjunction with certain familial syndromes, various pathologic states and numerous drugs. In many cases, however, the cause of the breast enlargement cannot be determined. This article reviews the histopathology, etiology and treatment of gynecomastia.

    Gynecomastia, a benign enlargement of the male breast due to proliferation of the glandular component, is a common clinical condition. It may be an incidental finding on routine examination or may present as an acute unilateral or bilateral painful tender mass beneath the areolar region or as a progressive painless enlargement of the breast.     To the Editor: In his review of gynecomastia (Feb. 18 issue), Dr. Braunstein includes a long and detailed list of agents that have the potential to induce the condition. Human growth hormone was not listed. Although not widely known, it has been reported that growth hormone can induce gynecomastia . The underlying mechanisms are not known, but may be related to the lactogenic activity of growth hormone. Recombinant techniques have provided an unlimited supply of growth hormone, and many patients without growth hormone deficiency are being treated with the hormone.     Gynecomastia
    Gynecomastia is the development of abnormal breast tissue on men, small or large, and normally on both sides. The term comes from the Greek gyne meaning "woman" and mastos meaning "breast." The condition is common in adolescent boys, though in 90% of such cases the symptoms disappear in a matter of months to a few years at most; it may also occur in males of advancing years. It is generally a result of an imbalance in hormones, though there are many different possible root causes. Gynecomastia is not simply a buildup of adipose tissue but includes the development of glandular tissue as well. It may take a softer form or include a form of lump-like gland, which should not be confused for cancer.
    Causes
    Among the various potential causes of gynecomastia are
    • Puberty Steroid abuse Obesity Tumors Chronic liver disease Side effects of various medications including those with hormonal effects Castration Aging Genetic disorders such as Klinefelter Syndrome and Gilbert's Syndrome.
