Most people do not expect to become paralyzed during the course of their lives. Barring injury to the nervous system or debilitating disease, one does not expect to lose motor function. In spite of these expectations, people of all races, sexes, ages, and classes can be afflicted with a debilitating syndrome that can lead to difficulty in walking or even to temporary paralysis in the most severe cases. This syndrome is known commonly as Guillain-Barre Syndrome, or GBS. GBS is an inflammatory disorder of the peripheral nerves. When the syndrome occurs, the body's peripheral nerves become inflamed and cease to work due to an unknown cause. Around 50% of the cases of GBS appear after a bacterial or viral infection.

Causes, incidence, and risk factors: Guillain-Barre syndrome is an acute type of nerve inflammation. The inflammation damages portions of the nerve cell, resulting in muscle weakness or paralysis and sensory loss. The damage usually includes loss of the myelin sheath of the nerve (demyelination ), which slows the conduction of impulses through the nerve. The damage can also cause denervation (kill the axon part of the nerve cell), which stops nerve function entirely. Without the axon, messages cannot be transferred from one nerve cell to another.
The exact cause of this disorder is unknown. It may occur at any age but is most common in people of both sexes between the ages 30 and 50. It often follows a minor infection, usually a respiratory (lung) infection or gastrointestinal (gut) infection. Signs of the original infection have usually disappeared before the signs of Guillain-Barre begin.

Causes
Typically, Guillain-Barre develops as an autoimmune reaction following an acute infection. It's not inherited, although it's thought genetic factors may make some people more likely to develop autoimmune conditions.
Symptoms
The paralysis is often progressive and ascending An initial episode of headache, vomiting, fever and back and limb pain is followed by paralysis, which starts as tingling and numbness followed by increasing weakness. The paralysis is often progressive and ascending (starting with the feet and moving upwards), but the condition may come on suddenly and affect all four limbs at once. The degree of paralysis varies. There may also be a loss of sensation, sometimes with pain. In about one in four cases the paralysis spreads to the respiratory nerves controlling breathing and the person needs to be put on a ventilator.

Guillain-Barré Syndrome
Synonym: acute inflammatory polyneuritis. An autoimmune disorder causing demyelination and axonal degeneration, causing an acute, ascending and progressive neuropathy characterised by weakness, paraesthesiae, and hyporeflexia. In severe cases, muscle weakness may lead to respiratory failure. Severe autonomic dysfunction may also occur. Epidemiology This is the most common acute neuromuscular paralytic syndrome. The annual incidence of Guillain-Barré Syndrome is 1-3/100 000 population . Increased incident in males. Peak ages are 15-35 years and 50-75 years.

  • An 8-year old girl presents with one week of progressive ascending motor weakness, subsequently diagnosed as Guillain-Barre syndrome. Consideration is given to therapy with plasmapheresis or intravenous immunoglobulin.
Clinical Bottom Lines
  • Intravenous immunoglobulin (0.4 g/kg daily for 5 days) is equivalent to plasma exchange (five 50 ml/kg exchanges over 8-13 days) for treatment of Guillain-Barre syndrome in adults. Therapy with both plasma exchange and immunoglobulin conferred no additional benefit. Intravenous immunoglobulin therapy is less expensive and is easier to administer.

Inclusion criteria were severe disease (aid needed for walking), age over 16 years, and onset of disease within the past 14 days. Patients were followed up for 48 weeks. The major outcome measure was improvement on a seven-point disability scale (0=healthy without signs of disease to 6=dead). Secondary outcomes included: time to recovery of unaided walking, time to discontinuation of ventilation, and the trend in improvement of the disability over the follow-up period.
