9.10 Acute Gouty Arthritis
Presentation
Usually a male patient over 50 years old with an established diagnosis of gout or hyperuricemia rapidly develops an intensely painful monarticular arthritis, often in the middle of the night, but sometimes a few hours following a minor trauma. Any joint may be affected, but most common is the metatarsophalangeal joint of the great toe (podagra). The joint is red, hot, swollen, and intensely tender to touch or movement. There is usually no fever, rash, or other sign of systemic illness. The patient may have predisposing factors that increase his risk of developing gout, like obesity, moderate to heavy alcohol intake, high blood pressure, diabetes and abnormal kidney function, or he may be taking certan drugs including thiazide diuretics, low-dose aspirin, and tuberculosis medications (pyrazinamide and ethambutol).
What to do:
  • If the patient has not been previously diagnosed by arthocentesis that showed crystals, then tap the involved joint as described in acute monarticular arthritis . In addition to ruling out infection, look under the microscope for crystals in the joint fluid. Urate crystals look like needles, and may be in white cells. Polarizing filters above and below the sample can help disclose and differentiate crystals.

G out (hyperuricemia or elevated uric acid) is a systematic disorder that affects the big toe joint. Gout is the end result of a build-up of uric acid in the blood. The uric acid crystallizes and settles in the joints in the body, most often in the big-toe joint. When it does so, it produces an excruciating pain. The onset of an attack of gout can be sudden and vicious. It is not uncommon for the victim to be awakened by this excruciating pain in the middle of the night. (Gout is often confused with Osteoarthritis, a condition that is characterized by a much slower onset of pain, and seems worse after a person has been walking or running.)

Gout has been recognized for more than 2,000 years, making it one of humankind's oldest known diseases. In the past, gout was often known as "the disease of kings" because it was associated with wealthy men who overindulged in rich food and drink. Today, it's known that gout is a complex disorder that can affect anyone. In fact, it's a painful problem for more than 2 million Americans. It's true that men are more likely to get gout, but women become increasingly susceptible to it after menopause. Fortunately, gout is treatable, and there are ways to keep it from recurring. Causes of Gout Almost all people who have gout have too much uric acid in their blood, a condition called hyperuricemia. However, there are many people who have hyperuricemia but not gout.

Gout
A recurrent acute or chronic arthritis of peripheral joints that results from deposition in and about the joints and tendons of monosodium urate crystals from supersaturated hyperuricemic body fluids.
Pathophysiology
Tophi are MSU crystal aggregates. They are large enough to first be seen on x-rays of the joints as punched-out lesions and later seen or felt as subcutaneous nodules. At the acid pH of urine, uric acid itself is precipitated readily as small platelike crystals that may aggregate to form gravel or stones, which may cause obstructive uropathy. Sustained hyperuricemia is usually caused by decreased renal clearance of urate, especially in patients receiving chronic diuretic therapy and in patients with primary renal diseases that decrease GFR. The greater the degree and duration of hyperuricemia, the greater the chance of crystal deposition and of acute attacks of gout. However, many hyperuricemic persons never develop gout. The serum urate level reflects the size of the extracellular miscible urate pool, which normally turns over once every 24 h; 1/3 of the urate is excreted in the feces, and 2/3 in the urine. Normal 24-h urinary uric acid excretion is 300 to 600 mg after 3 days on a low-purine diet or about 600 to 900 mg on a regular diet. Therefore, food intake accounts for about 450 mg of uric acid daily. Hyperuricemia and gout are common complications in patients receiving cyclosporine for organ transplants. Mean urate levels in premenopausal women are about 1 mg/dL (0.6 mmol/L) lower than those in men, but after menopause the levels approach those of men.

Gout is a syndrome caused by an inflammatory response to the formation of monosodium urate monohydrate crystals which develop secondary to hyperuricemia. Acute and chronic forms are recognized. Hyperuricemia may be due to environmental and/or genetic factors.

    Gout is a form of acute arthritis that causes severe pain Description
    Uric acid, which is found naturally in the blood stream, is formed as the body breaks down waste products, mainly those containing purine, a substance that is produced by the body and is also found in high concentrations in some foods, including brains, liver, sardines, anchovies, and dried peas and beans. Normally, the kidneys filter uric acid out of the blood and excrete it in the urine. Sometimes, however, the body produces too much uric acid or the kidneys aren't efficient enough at filtering it from the blood, and it builds up in the blood stream, a condition known as hyperuricemia. A person's susceptibility to gout may increase because of the inheritance of certain genes or from being overweight and eating a rich diet. In some cases, another disease (such as lymphoma, leukemia, or hemolytic anemia ) may be the underlying cause of the uric acid buildup that results in gout.
