Gout is painful swelling in the feet. Medication is the treatment of choice for gout. I would suggest you get in touch with your doctor because there are some newer gout medications on the market that may not interfere with your other medications. The uric acid level in the blood goes up in persons with gout. Alcohol is not recommended for persons with an elevated uric acid because it causes your body to lose water and may increase the uric acid levels in the blood. Also, foods high in purines break down in the body to uric acid. However, food sources of purines account for only about 50 percent of the uric acid produced within the body. The remaining uric acid breaks down from other sources within the body. We used to prescribe a low purine diet, but the diet is not as frequently used. A low purine diet omits organ meats like liver, kidney, sweetbreads, brains, meat gravies and extracts, sardines, anchovies, herring, mackerel, scallops and most wild game. The following foods are allowed once per day: meat; fish; poultry; dried beans and peas; asparagus; mushrooms; cauliflower; spinach.

Gout Greek name: podagra , from pod - foot and agra - trap) is a form of arthritis caused by the accumulation of uric acid crystals (due to hyperuricemia ) in joints . It is an intensely painful disease, which in most cases affects only one joint, most commonly the big toe. The patient usually suffers from two sources of pain. The crystals inside the joint cause intense pain whenever the affected area is moved. The inflammation of the tissues around the joint also causes the skin to be swollen, tender and sore even if it is slightly touched. For example, a blanket draping over the affected area would cause extreme pain.

The classical picture is of excruciating pain of sudden onset in only one joint, usually the big toe (75% of first attacks affect the first metatarsal-phalangeal joint ), associated with swelling and redness.

What is gout? Gout is a painful and potentially disabling rheumatic disease and is one of the most common forms of arthritis. Gout accounts for approximately 5% of all cases of arthritis. In the U.S. it occurs in approximately 840 out of every 100,000 people. Historical records for gout have been found dating back over 2000 years. Gout was first described by the famous greek physician Hippocrates, 'the father of medicine'.
Gout is characterised by high levels of uric acid in the blood and reoccurring 'gout attacks'. Uric acid is eliminated by the kidneys and passed out of the body in urine. If the kidneys are unable to eliminate uric acid efficiently, blood uric acid levels will increase. A hardened form of uric acid ( sodium urate) may then be deposited around joints. These hardened, crystallised deposits can lead to 'gout attacks' which causes joint inflammation (arthritis) and much pain. The skin around an affected joint becomes red and shiny with the affected area being tender and painful to touch. Whilst there are often months or years between initial attacks, gout attacks can become more frequent overtime. If left untreated, acute gout attacks can lead to permanent joint damage and disability.

The association of gout and uric acid has been known since the 19 th (top of page) Asymptomatic Hyperuricemia Incidence of gout (top of page) Acute Gout Attack An acute attack of gout is a highly inflammatory arthritis often with intense swelling, redness and warmth surrounding the joint. The inflammatory component is so intense, an acute attack of gout is often mistaken for a bacterial cellulitis. Even among other types of inflammatory arthritis, gout is considered the most painful with the patient unable to bear weight or put on a shoe over the affected joint and often prompting an off hour trip to the emergency room. Chills, a low grade fever and an elevated white blood cell count can occur, mimicking an infection. Characteristically, early gout attacks resolve spontaneously after 3 to 4 days. The first MTP joint (podagra) is the first joint affected in 50% of cases followed other lower extremity joints. Over 90% of patients who present with acute polyarticular gout have a history of podagra.
The victim goes to bed and sleeps in good health. About 2 o'clock in the morning, he is awakened by a severe pain in the great toe; more rarely in the heel, ankle or instep. This pain is like that of a dislocation, and yet the parts feel as if cold water were poured over them. Then follows chills and shiver and a little fever. The pain which at first moderate becomes more intense. With its intensity the chills and shivers increase. After a time this comes to a full height, accommodating itself to the bones and ligaments of the tarsus and metatarsus. Now it is a violent stretching and tearing of the ligaments now it is a gnawing pain and now a pressure and tightening. So exquisite and lively meanwhile is the feeling of the part affected, that it cannot bear the weight of bedclothes nor the jar of a person walking in the room.

Mike Snaith examines one of the more easily treatable rheumatic diseases. Gout is a 20th century success story. Attacks can be treated and prevented and damage to joints and kidneys can be prevented. However, we can never be complacent, as the following case histories illustrate. Case No 1. John McG. was a heavily built Scotsman, with gout for 15 years. Beer with whisky chasers was his tipple. I told him about his weight, his gout and the poor state of his liver and he promised to stop drinking. We started him on allopurinol (Xyloric) with the usual precautions. Over the next few months his attacks diminished, but not much; his uric acid reduced only a little and his weight remained unchanged. When I asked him about his alcohol, he said: "Of course doc, I've done exactly what you said, I've given up the drink". Me: "Your gout should have improved more than this. So you've given up all your drinking, your beer and your whisky?" To which he replied "Oh! doctor, I didn't think you meant the beer as well, I don't really count that as drinking!" Case No 2. David R. was a bachelor, who limped into the consulting room on a stick in slippers. He could not wear his normal size 91/2 shoes because of the painful swelling due to his gout. He was taking 900mg of allopurinol per day. The usual dose is 300mg per day, rarely 600! And his gout was still not controlled. Like Mr McG, he drank too much, but he also ate liver, bacon, sausage or kidneys at least four days a week, or steak when out in a restaurant. Similarly on his trips abroad his diet would have made a vegetarian weep.

Introduction
Gout has been known for more than 2000 years. It can affect men of any age. It is less common in women and then only occurs after the menopause. Luckily, gout is probably the rheumatic disease for which there is the most satisfactory treatment.
What is gout and what causes it?
People get gout because there is something wrong with the chemical processes of their body. Urate (which we all have in our blood) can build up and form crystals in the joints and cause them to feel intensely painful. This defect is usually inherited from a parent or grandparent. Gout runs in families. The joints are not the only part of the body to be affected. Crystals may appear under the skin, including sometimes on the ear. They look like little white pimples and are called 'tophi'.

What is gout?
Gout is a kind of arthritis caused by too much uric acid in the joints. Uric acid is a substance that forms when your body breaks down a substance called purines. Who can get gout?
If you eat a lot of foods that are rich in purines, you may get gout. Some of these foods are salmon, sardines, liver and herring. Medicines that may cause gout include the following:
  • niacin (a B-complex vitamin) aspirin (taken in low doses) cyclosporine (brand names: Sandimmune, Neoral, SangCya) some drugs used to treat cancer
