Table Of Contents: While a handful of sexually transmitted HPVs can cause a variety of conditions that can lead to dangerous cancers if they remain untreated, it is a gross and dangerous exaggeration to typify HPV as a "dreaded virus" and safer-sex, public health messages advocating condom use as a "conspiracy" ("House Approves...", 2000). This fact sheet will give sexually active women and men the facts they need to understand the real nature of HPVs, what conditions HPVs cause, how those conditions can be effectively managed, and how one may reduce one's own risk of becoming infected.

Genital warts are very common. They are caused by a virus, the human papillomavirus (HPV).
  • There are at least 80 different types of HPV; 20 can infect the genital area. At least 75% of sexually active adults have been infected with at least one type of genital HPV at some time in their life. Most do not develop visible warts; the infection may show up on a cervical smear. This is known as subclinical infection. Visible genital warts are often easy to diagnose by their typical appearance. They are usually due to HPV Types 6 and 11.

What are the human papilloma viruses?
The human papilloma viruses (HPVs) are a family of over 60 different viruses responsible for causing warts. The majority of the viruses produce warts in areas other than the genitals. They are responsible for the common warts seen on the hands, fingers, and even the face. Most of these viruses are innocuous, causing nothing more than cosmetic concerns. Several virus "types" are confined primarily to the moist skin of the genitals, producing lesions in this area. The lesions produced by these genital viruses are called genital warts. The virus that causes genital warts is not the same as the virus that causes common warts. You cannot develop genital warts from the virus that causes common warts. Certain types of genital viruses can cause abnormal PAP smears and even cervical cancer. Once contracted, the virus can stay with the infected individual for life. It is estimated that over 50% of sexually active adults are carriers of one of these viruses. Men and women are equally affected. The chance of being a carrier increases with the number of sexual partners. Infection with HPV is now considered to be the most common sexually transmitted disease in the United States. It is also the leading cause of abnormal PAP smears and pre- cancerous changes of the cervix in women. Even though there are reports of pre-cancerous changes occurring on the skin of the male penis, the incidence is insignificant compared to the large number of women who experience pre-cancerous changes of the cervix.

What is human papillomavirus (HPV)?
Human papillomavirus (pronounced pap-ih-lo-ma-vye-rus) is also called HPV. It is a virus that includes more than 100 types, over 30 of which are sexually transmitted. The types of HPV that infect the genital area are known as genital HPV. Most sexually active people will have HPV at some point in their lives, though most will never know it because it usually has no symptoms and goes away on its own. Genital HPV types are either low-risk or high-risk types. This does not have to do with the risk of getting the infection. It is about the risk of getting cervical cancer.

Condyloma acuminata is the official name for a type of wart that appears on the genital area of men and women ( see image ). These warts appear on the penis and around the anus in men, and on any part of the external genitals - vaginal lips, perineum (area between the vagina and anus), and anus in women. They also occur inside the vagina and on the cervix. They may occur singly, or in groups. Sometimes they itch or burn, but often they have no symptoms at all. This type of wart is sexually transmitted. It is highly contagious. The incubation period - the amount of time between contact with a person who has condyloma and the time you notice condyloma - typically ranges from three weeks to eight months, with an average of three months. Condyloma may, however, appear years after a person has been exposed. Transmission
Condyloma are caused by a virus called human papillomavirus (HPV). The virus is generally sexually transmitted in adults. Some people apparently can have the virus, not show any signs of condyloma and still transmit the virus to a partner, who may develop condyloma. Since HPV can have such a long incubation period, it is often not possible to determine how you contracted it. Evaluation
Some strains of HPV seem to be related to the occurrence of genital cancer, especially cancer of the cervix. Therefore, if you have condyloma, it should be thoroughly evaluated and treated.

Abstract
Introduction - A recent study comparing heterosexual men with and without confirmed sexually transmitted diseases (STDs) in an urban STD clinic showed that uncircumcised men were less likely than circumcised men to have genital wart detectable by clinical examination (adjusted odds ratio 0.7, 95% confidence interval 0.4, 0.9). Based on these initial findings we hypothesised that the appearance and anatomic distribution of genital warts, and possibly treatment response, may be different for circumcised and uncircumcised men. Methods - The anatomic location, appearance, number of warts, and response to treatment was investigated through review of medical records of 459 heterosexual men with genital warts detected in 1988.

    Specific descriptions
    • In men, genital warts can infect the urethra, penis, scrotum, and rectal area. The warts can appear as soft, raised masses with a surface that can be smooth (on the penile shaft) or rough with many fingerlike projections (anal warts). Others may appear pearly, cauliflower-like, or rough with a slightly dark surface. Most lesions are raised, but some may be flat with only slight elevation above the skin surface. Infection with HPV may be dormant or undetectable, with some lesions hidden by hair or in the inner aspect of the uncircumcised foreskin in males.
      In women, genital warts have a similar appearance and usually occur in the moist areas of the labia minora and vaginal opening. Lesions visible on the outer genitals require a thorough examination of the vaginal canal, cervix, and anorectal area. Most vaginal warts occur without symptoms. Rarely, women may experience bleeding after sexual intercourse, itching, or vaginal discharge.

The incidence of latent or sub-clinical infections may be as great as 100 times that of clinical disease. Perianal warts may present with or without genital warts. Local spread of infection to the perianal regions is a possible mechanism when there is co-existing of genital lesions. When only perianal warts are present, anal intercourse is a likely route of transmission although such history is rarely volunteered by the patient.

G enital warts are the visible manifestation of infection by one or more of the nearly 100 recognized human papillomaviruses (HPVs). Visible genital warts typically are caused by HPV types 6 and 11, which rarely are associated with invasive squamous cell carcinoma of the external genitalia. HPV types 16, 18, 31, 33, and 35 have been found in genital warts and are associated with squamous intraepithelial neoplasia ; types 16 and 18 are associated most strongly with malignant potential. Genital warts are a sexually transmitted infection caused by a virus called the human papilloma virus (HPV). Genital warts are the most common complaint diagnosed at sexually transmitted diseases clinics in the UK, accounting for around a fifth of all diagnoses in 1999. During the late 1990s, there was a rising incidence of genital warts in the UK, affecting both men and women, and in 1999 there were 70,000 cases. Though younger people tend to be more frequently affected, anyone who is sexually active may be exposed to the genital wart virus.
Some forms of the genital wart virus can lead to cervical or anal cancer. This risk is higher in people with HIV because there is more chance that the wart virus will become re-activated due to loss of immunity. Transmission
The genital wart virus can be contracted during unprotected anal, vaginal or oral sex. It can also be transmitted by close physical contact with the genital warts themselves, as these may shed the wart virus.
