Gallstones
Definition
A gallstone is a solid crystal deposit that forms in the gallbladder, which is a pear-shaped organ that stores bile salts until they are needed to help digest fatty foods. Gallstones can migrate to other parts of the digestive tract and cause severe pain with life-threatening complications. Description
Gallstones vary in size and chemical structure. A gallstone may be as tiny as a grain of sand or as large as a golf ball. Eighty percent of gallstones are composed of cholesterol. They are formed when the liver produces more cholesterol than digestive juices can liquefy. The remaining 20% of gallstones are composed of calcium and an orange-yellow waste product called bilirubin. Bilirubin gives urine its characteristic color and sometimes causes jaundice Gallstones are the most common of all gallbladder problems. They are responsible for 90% of gallbladder and bile duct disease, and are the fifth most common reason for hospitalization of adults in the United States. Gallstones usually develop in adults between the ages of 20 and 50; about 20% of patients with gallstones are over 40. The risk of developing gallstones increases with ageat least 20% of people over 60 have a single large stone or as many as several thousand smaller ones. The gender ratio of gallstone patients changes with age. Young women are between two and six times more likely to develop gallstones than men in the same age group. In patients over 50, the condition affects men and women with equal frequency. Native Americans develop gallstones more often than any other segment of the population; Mexican-Americans have the second-highest incidence of this disease.

What are they?
Gallstones are lumps of solid material that form in the gall bladder. Most are the size of peas, but they can be as large as pebbles. Gallstones can be found in the gall bladder or in the bile duct that connects the gall bladder to the small intestine. Most gallstones are made of cholesterol and bile pigments. Some gallstones, however, are just made of cholesterol and some are made of bile pigment. Often gallstones don't cause symptoms. However, if one partially or completely blocks the normal flow of bile, it may cause an attack of biliary colic with upper abdominal pain, nausea and vomiting. These episodes are normally brief and typically follow a fatty meal, which causes the gall bladder to contract. Stones that continue to block the drainage of bile can cause inflammation or infection of the gall bladder and bile ducts, jaundice and acute

Gallstones also can develop in the common bile duct, the tube that carries bile from the gallbladder and the liver to the small intestine. Gallstones are common, and they develop when there is too much cholesterol in the bile for it to remain dissolved or when the gallbladder does not empty as quickly as it should. Gallstones are most common in women, people who are obese, older people, people with sickle cell disease, people who have lost weight rapidly, and people taking certain medications. Gallstones may be as small as a grain of sand or as big as a large coin. They may irritate the gallbladder and interfere with the way it works.

We always hear about the benefits of maintaining a healthy weight. Here's another. People who maintain a healthy weight are less likely to develop gallstones, according to the National Institute of Diabetes and Digestive and Kidney Diseases (NIDDK). But when it comes to gallstones, there's a catch. Someone who is already overweight and who quickly loses weight, especially through severe calorie restriction, is actually increasing their risk of gallstones. What are gallstones What are gallstones exactly? Basically, they're clusters of solid materials, mostly cholesterol (Read about " Cholesterol "), that form in the gallbladder, an organ that lies beneath the liver and stores a substance called bile that helps with digestion. Bile helps to break down fat and eliminate excess cholesterol, according to NIDDK. But if the bile gets too saturated with cholesterol, gallstones can form. According to the American Gastroenterological Association, gallstone disease affects 10 to 15 percent of the population of the United States or more than 25 million people, although most are unaware of it because they have what's called "silent" gallstones which produce no symptoms. Some people, though, have symptomatic gallstones, which can cause pain, hospitalization, and even be life-threatening. One of the reasons could be that the gallstone is blocking the secretions from the pancreas, causing acute pancreatitis. (Read about "

... Gallstones
Gallstones The gallbladder, a small, pear-shaped organ under the liver, collects and stores bile. Bile is a thick, greenish-yellow, bitter fluid made by the liver. Bile is made up of bile salts, electrolytes, bilirubin, cholesterol and other fats. The salts help stimulate the large intestine to secrete water and other slats, which helps move the contents of the digestive tract out of the body. Bilirubin is a waste product consisting of the remains of worn-out red blood cells. The breakdown products of drugs and wastes processed by the liver are also released in bile. Bile helps the small intestine digest fats and remove waste products. It passes from the liver's bile duct into the duodenum, as needed. It does this through the common bile duct where the liver, gallbladder and pancreatic ducts join. About half the bile secreted between meals flows into the small intestine. The other have is diverted from the common bile duct through the cystic duct, into the gallbladder where it is stored. Once there, nearly 90% of the water in the bile is absorbed by the bloodstream. What is left is a concentrated solution of bile salts, biliary lipids and sodium. Gallstones are like tiny rocks that form in the gallbladder. They are made of cholesterol or bile particles that clump into a solid mass. A stone can grow as liver bile passes through the gallbladder. Most gallstones are small enough to pass out of the body through the intestines without our noticing. However, some do get caught in the narrow outlet of the gallbladder or in the intestines. This can lead to pain, a buildup of bile, pancreatitis and/or infection. Gallstones are common, especially in women, the elderly and overweight individuals.

Gallstones are clusters of solid material that form in the gallbladder. They are made mostly of cholesterol. Gallstones may occur as one large stone or asmany small ones. They vary in size and may be as large as a golf ball or as small as a grain of sand.
Experts estimate that 16 to 22 million people in the United States have gallstones—as many as one in every 12 Americans. Most people with gallstones do not know that they have them and experience no symptoms. Painless gallstones are called silent gallstones. Sometimes gallstones can cause abdominal or back pain. These are called symptomatic gallstones. In rare cases, gallstones can cause serious health problems. Symptomatic gallstones result in about 800,000 hospitalizations and more than 500,000 operations each year in the U.S.

with Rachael Droege The New York Times regional newspaper interviewed me about gallbladders and I thought it would be useful to review this common problem. I have seen many hundreds of patients who have had their gallbladders removed and I don't recall anyone ever telling me that their surgeon advised them to do something to compensate for removing this important organ. Just about every one of them was told they didn't need their gallbladder and that it was perfectly fine to have it removed. This is reprehensible ignorance as it condemns the patient to a lifelong deficiency of essential fatty acids. Signs of Gallbladder Disease If you have any of the following symptoms then your gallbladder may not be functioning properly:
  • Pain when pressing on the gallbladder, which is directly under the last rib on the right on the same plane as one’s nipple. This is usually due to gallbladder "sludge" (thick bile).

Peer Review Status: Internally Peer Reviewed After a meal, while watching television, suddenly you feel a stabbing pain below your rib cage. This isn't the first time. Eventually, you find out your pain was caused by gallstones. Gallstones are a major health problem. About 20 million people in the United States have them and one million new cases are diagnosed each year. "Gallstones are the fifth leading cause of hospitalization," says Dr. Jeffrey Field, professor of internal medicine at the UI College of Medicine and gastroenterologist at UI Hospitals and Clinics. The gallbladder is a small muscular sac located under the liver that stores bile secreted by the liver. Bile helps you digest fats. At the turn of the century, physicians believed gallstones were always the result of a diseased gallbladder. Today they realize that gallstones, most of which are composed mainly of cholesterol, are primarily caused by a disturbance in cholesterol metabolism within the liver. "There is too much cholesterol relative to the amount of other lipids in the bile," Field says. Cholesterol crystals then form in the bile and grow into larger stones. Many people can live for years without knowing they have gallstones. Pain below the right rib cage, called biliary colic, is usually the first symptom. "It's a sharp, boring, intense pain that can radiate around to the back. The gallbladder is trying to contract, but the opening into the cystic duct is blocked," Field says. Nausea, vomiting, and fever can accompany an attack, which can last several hours. Any foodsbut particularly fatty foodscan precipitate an attack. Obesity, pregnancy, diabetes, prolonged fasting, cystic fibrosis, certain medications, or a high-calorie diet can increase your chances of developing gallstones. The condition becomes more common as you age.

Peer Review Status: Internally Peer Reviewed About 500 thousand persons in the United States will require removal of the gallbladder this year, and many aren't sure why this most common of major operations is necessary. Dr. Jeffrey Field, professor of internal medicine at the UI Carver College of Medicine explains, "Almost all the problems result when stones are formed in the gallbladder and block the tiny tube through which the gallbladder fills and empties." Between meals the gallbladder normally collects and stores bile that has been manufactured in the liver. When the nervous and hormonal systems signal the presence of food, the gallbladder contracts, emptying the bile into the intestine to aid the digestion of fats. When the tube is blocked, however, normal emptying is impaired, and stagnant conditions in the gallbladder allow growth of bacteria. The more common type of gallstones consists predominately of cholesterol, the same fatty substance that causes problems in arteries. The cholesterol in bile is kept in solution by the detergent action of bile acids and phospholipids.

Health Problems in American Indian/Alaska Native Women: Gallstones
Gallstones are pieces of solid matter that form in the gallbladder, a part of the digestive system that stores bile, which helps your body digest food. Gallstones form when parts of the bile form hard crystals. There are 2 types of gallstones: pigment (bilirubin) and cholesterol. Most people have cholesterol gallstones. They sometimes form as lots of small stones or may form as one big stone. A lot of times, gallstones don't cause any symptoms. When they do, some common symptoms include:
    • a severe pain in the upper abdomen or right side that sometimes lasts for several hours pain in the right shoulder or between your shoulder blades nausea or throwing up sweating indigestion or stomach bloating after a fatty meal
Some factors that put you more at risk for gallstones include:
    • obesity too much estrogen gender (female) age (over 60) taking drugs that lower cholesterol diabetes rapid weight loss fasting
Because of high levels of cholesterol in their bile, more American Indians have gallstones than other women in the United States. Among the Pima Indians of Arizona, 70 percent of women have gallstones by age 30. Last updated: January 2005
