Commonly Treated Conditions The most common conditions treated by a pedorthist include:
  • Plantar Fasciitis (pain in heel or arch) - Patients suffer from exquisite pain at the heel or along the arch of the foot, especially first thing in the morning. Plantar fasciitis is a common (and often chronic) overuse condition. It requires a controlling orthosis to maintain foot position and reduce tension on the injured tissue. Metatarsalgia (pain in the joint of the ball of the foot)- Patients with metatarsalgia describe a persisting pain in the ball of the foot that feels like "walking with a pebble in your shoe". It can also be accompanied by burning, tingling or numbness into the forefoot and toes. Pedorthist commonly treat metatarsalgia with two devices: an orthosis contoured to unload painful areas, and a rocker sole to reduce painful toe extention. Diabetes - Diabetics have decrease blood circulation and loss of sensation in their feet, which makes them high-risk patients. Diabetics are particularly susceptible to skin breakdown and ulceration, so it is critical that they have optimal-fitting footwear and accommodative orthoses. Arthritis - Activities such as standing and walking aggravate the continual pain and degeneration of the joint caused by arthritis. Patients benefit tremendously from additional-depth footwear and footwear modification that align, cushion, and stabilize feet.

Nail and Hyperkeratotic Problems in the Elderly Foot Subungual hemorrhage, onychauxis, onychogryphosis, mycotic infections, onychia and paronychia are common toenail disorders in the elderly. The many contributing factors include impaired vision, changes in gait, past foot conditions and neurovascular disease. Hyperkeratotic disorders are secondary to external or internal pressure. Many of these foot problems are chronic conditions and require continuing management for relief. If improperly treated, some conditions may lead to amputation. Foot health and the ability to remain ambulatory are major concerns for the elderly. In these patients, the foot can be a focus for morbidity, as well as a cause of disability and limitation of activity. The loss of a limb or the ability to ambulate produces significant medical and social problems for the elderly patient, the family and society.

Important Note
Each benefit plan defines which services are covered, which are excluded, and which are subject to dollar caps or other limits. Members and their providers will need to consult the member's benefit plan to determine if there are any exclusions or other benefit limitations applicable to this service or supply. The conclusion that a particular service or supply is medically necessary does not constitute a representation or warranty that this service or supply is covered (i.e., will be paid for by Aetna) for a particular member. The member's benefit plan determines coverage. Some plans exclude coverage for services or supplies that Aetna considers medically necessary. If there is a discrepancy between this policy and a member's plan of benefits, the benefits plan will govern. In addition, coverage may be mandated by applicable legal requirements of a State, the Federal government or CMS for Medicare and Medicaid members. CMS's Coverage Issues Manual can be found on the following website: http://cms.hhs.gov/manuals/pub06pdf/pub06pdf.asp
