Physicians call for change in flu vaccine recommendations

Cutter (WebMD) As another flu season approaches, a leading physicians' organization is recommending everyone 50 years and older receive a vaccination against influenza. That represents a much lower age than the previous recommendation, which called for people 65 years and older to get flu shots, as well as people of all ages with special health conditions. Officials at the American Academy of Family Physicians called for the change during a September meeting in Orlando, Florida, because the rate of flu-related deaths rises once people pass age 50. "We're not talking about a small public health problem; we're talking about a major problem," said Dr. Richard Zimmerman of the University of Pittsburgh and a member of the academy's Commission on Clinical Policies and Research.

"If you're healthy, you have two options: Please take the option that the other folks can't have," said Dr. William Schaffner of the National Foundation for Infectious Diseases. The nasal spray is recommended only for healthy people ages 5-49, and doctors began pushing it as an alternative to the traditional vaccine Friday, after the two makers of flu shots in the United States announced they have run out and will not be able to meet a surge in demand resulting from fears of a particularly bad season.

*Since live vaccine is licensed for use only in healthy persons 5 to 49 years of age, it should be used primarily for the vaccination of healthy persons in that age group who are health care workers or household contacts of infants younger than six months of age. Nonpriority groups The state and local jurisdictions may choose to broaden the groups for whom influenza vaccination is recommended on the basis of local vaccine supply situations. Where supply is ample, jurisdictions may choose to expand vaccination beyond the priority groups named above. Where supply of influenza vaccine is still short, persons who are not included in one of the priority groups or additional priority groups for vaccination should be informed about the vaccine supply situation and asked to forego or defer vaccination with inactivated influenza vaccine.

The bird flu has forced the slaughter of some 50 million poultry across Asia, and killed at least 19 people 14 in Vietnam and five in Thailand. "It can be anticipated that human cases will also be detected in other countries where outbreaks in poultry are spreading," the WHO said Tuesday in a statement posted on its website. Eight other governments in Asia also have reported bird flu outbreaks Cambodia, China, Indonesia, Japan, Laos, Pakistan, South Korea and Taiwan.

What is Flu?
Even if you're not among the 20,000 Americans flu kills in an average year, the bug's a bummer. Between 35 million and 50 million Americans get its chills, fevers, aches and coughs. The direct medical cost of flu is guesstimated at $4.6 billion annually (not counting lost work time). Many deaths are caused by the complication of pneumonia, a lung infection that can be caused by a virus or a bacterium. Others result from exacerbations to pre-existing conditions like diabetes, heart or lung disease. To understand flu, you've got to know something about viruses. First off, they're weird. Unlike all other forms of life, they don't have cells they're basically chains of genetic material lumped inside a protein coat. Viruses can multiply and evolve, but some experts think they're not really alive, since they can't reproduce without a host cell. Host cells are innocent victims that allow the virus to enter and commandeer its machinery to make new viral particles. To some observers, viruses are the ultimate guerilla fighters. Other virologists disagree, arguing that there really is life inside that protein coat. University of Wisconsin flu expert Virginia Hinshaw, for example, says, "To have an infectious agent that has adapted to use our machinery to reproduce, to me, that's living."

(CBS) Some public health officials believe a deadly worldwide flu pandemic is inevitable. They say it would likely involve a new strain of the virus that spreads easily from person-to-person.
And, while admitting it can't be ruled out, Health and Human Services Secretary Mike Leavitt says precautions are being taken to try to keep officials ahead of the curve.
"We've had three pandemics of different proportion in the last century," Leavitt told co-anchor Harry Smith on The Early Show Monday. "These things do happen. They're like biologic earthquakes. They will occur. The important thing is that we're prepared when they do and find ways to minimize them."
The so-called Asian bird flu is "the pandemic we're worried about currently," Leavitt continued. "Since 1997, when it first began to appear, we've seen it appear in some 10 or more countries. Since 2004, we've been able to identify 109 cases where it has actually transmitted to people. About half of those died. So it is of concern.

Cat 'flu is caused by either feline calicivirus (FCV) or feline herpesvirus (FHV - also known as feline viral rhinotracheitis virus - FVR), or, uncommonly, a combination of both. The most serious cases are caused by FHV. How is cat 'flu spread?
The main method of spread of the cat 'flu viruses is by direct contact of one cat with another. Large amounts of virus are present in saliva, tears and nasal secretions (FCV may also be shed in urine and faeces). Thus it can be spread by cats sniffing each other, mutual grooming and sharing feeding bowls. Sneezed droplets may travel 1-2 metres and cat 'flu can spread rapidly through a cattery unless cats are housed individually with sneeze barriers. Both of the cat 'flu viruses are relatively fragile: FCV lasts about a week outside the cat and FHV lasts a day. Plainly, however, these viruses can be spread within a cattery on cages, food dishes, litter trays, people, etc. so good hygiene precautions should be taken.
