What is fifth disease?
Fifth disease is a viral illness that is associated with exanthem. Exanthem is another name for a rash or skin eruption. It is spread from one child to another through direct contact with discharge from the nose and throat. It can also be spread through contact with infected blood. It is moderately contagious and usually does not include a high fever, as seen with some other viral skin conditions.
What causes fifth disease?
Fifth disease is caused the human parvovirus. It is most prevalent in the winter and spring and is usually seen in children 5 to 14 years of age. Outbreaks of the disease frequently occur in school settings.
What are the symptoms of fifth disease?
It may take between four to 14 days for the child to develop symptoms of fifth disease after being exposed to the disease. Children are most contagious before the rash occurs. Therefore, children may be contagious before they even know they have the disease. Also, about 20 percent of people with the virus do not have symptoms but can still spread the disease. The following are the most common symptoms of fifth disease. However, each child may experience symptoms differently. Symptoms may include:

Summary: The "Fifth" disease is so called because its rash is similar to four previously described illnesses; Rubella, measles, scarlet fever, and Filatov-Dukes. Erythema Infectiosum is the clinical manifestation of human parvovirus disease; parvovirus B19. Typically found in elementary school age children, it is transmitted by airborne droplets. Sneezing and coughing is the main mode of transmission. Incubation period is a week. By the time the rash appears, the infectious stage has passed.     What is fifth disease? What does fifth disease cause? Fifth disease is a mild rash illness that occurs most commonly in children. Historically, it was called fifth disease because it was the fifth of a group of once common childhood diseases with similar rashes. It is caused by a parvovirus B19 (not the same one that causes disease in animals). A parvovirus infection can be more serious for pregnant women and for adults with some kinds of anemia or who have a compromised immune system.
    What are the symptoms of fifth disease?
    An ill child may have a low-grade fever, malaise (general ill feeling), or "cold-like" symptoms a few days before the rash breaks out. The ill child typically has a bright-red ("slapped-cheek") rash on the face and a lacy red rash on the trunk and limbs. The rash usually spreads downward, but does not reach the palms or soles. Occasionally, the rash may itch. The child is usually not very ill, and the rash resolves in 7 to 10 days. Sunlight, heat, exercise, and stress can worsen the appearance of the rash. Other symptoms that occasionally occur with fifth disease include swollen glands, red eyes, abdominal pain, nausea, sore throat, diarrhea, and unusual rashes that look like blisters or bruises. In addition, joint pain can occur, but is more common in adults than in children.

Around the turn of the century, doctors began numbering contagious diseases that produce somewhat similar rashes. Over time, most of these numbers fell out of use, replaced by more popular names such as measles (known in the numbering system as first disease), scarlet fever (second disease), and rubella (third disease). The only one of these illnesses to which the number stuck, in fact, was erythema infectiosum, which sometimes still goes by the name fifth disease . Erythema infectiosum is a mild childhood infectionso mild, in fact, that a child can have it without the parents' even noticing. Its hallmarks include a fine, pink rash covering the trunk and limbs and bright, red patches on the cheeks. WHAT CAUSES FIFTH DISEASE?
A highly contagious microorganism called parvovirus B19 has been identified as the source of fifth disease. Symptoms appear between four and 14 days after a child contracts the virus. The illness often occurs in epidemics during late winter and spring. During an epidemic, as many as half of the children in a school may contract erythema infectiosum. Fifth disease, erythema infectiosum or slapped child disease, is a not infrequent childhood infectious disease in which children have fevers, tiredness and extensive red rashes of the cheeks and face. It is caused by parovirus B19 and spreads like chicken pox or measles in preschool and school age children. Every once in awhile, an obstetrician diagnoses a baby who has died before birth with a condition in which there is massive fluid swelling (hydrops) of the fetus. In earlier times this was mostly due to Rh isosensitization in which the mother had an Rh negative blood type and the baby had an Rh positive blood type. In the last 20-30 years, treatment of Rh negative mothers with Rhogam has drastically reduced this immune hydrops occurance. Now the most frequent cause of hydrops is non-immune and is often caused by B19 parovirus infection in pregnant women. Since B19 parvovirus infection in pregnancy has been associated with stillborn babies, the question becomes should pregnant women be excluded from the workplace when there is a epidemic of fifth disease? Another concern would be if any of your family was exposed while you were pregnant, how likely is it that your unborn child would be affected? A recent article by Harger and others

Parvovirus B19 is a virus that commonly infects humans. The most common illness caused by parvovirus B19 is “fifth disease”, a mild rash illness that occurs most often in children. (Dogs and cats may be immunised against “parvovirus”, but these are animal parvoviruses that do not infect humans.) About 20% of infected children will have no symptoms at all. In others, early in the infection there may be mild cold-like symptoms, then two to five days later, the child typically develops a “slapped cheek” rash on the face, and a lacy red rash on the trunk and limbs. The child usually is not very ill, though the rash may occasionally be itchy. The rash disappears after 7 to 10 days, although it may come and go for several weeks, often in response to heat. On recovery, the child develops lasting immunity, and is protected against future infection. Adults may also be infected with parvovirus B19, and may have no symptoms, or may have the typical rash of fifth disease, joint pain or swelling, or both. The joints most frequently affected are the hands, wrists, and knees, usually on both sides of the body, and usually getting better in a week or two, though sometimes the pain and swelling may last several months. At least 50% of adults have previously been infected with parvovirus B19, and have developed immunity, so they will not get the infection again.

Slapped cheek disease or Fifth disease
Slapped cheek disease is a viral disease. It gets this name because, early in the infection, the child's cheeks may be bright red, as if they have been slapped. Other names for this illness are Fifth disease (there used to be six childhood rashes recognised at the turn of the century and this was the number five) and erythema infectiosum (Latin for infectious rash).
What causes it?
The causative virus is called parvovirus B 19. An Australian, Professor Yvonne Cossart first described this virus which is different from the parvovirus of dogs and cats. The virus is transmitted in droplets coughed out by infected children.
Who gets it?
Fifth disease mainly affects school and pre-school aged children, and not uncommonly causes outbreaks in schools. Most adults are immune, having already had the infection when they were children.
What does it cause?
In Fifth disease, children may get a runny nose, fever, aches and pains, and rash. At first the rash may be on the cheeks (slapped cheek appearance). After a few days a rash may be found on the arms, legs or trunk. It is pink, has a lacy appearance and may be itchy. It may fade easily, but re-appear after a bath or exercise. Older children and adults may sometimes get swollen joints (arthritis) which get better after a few days.
