What is it? Fibromyalgia (fi-bro-my-AL-ja) syndrome (FMS) is a chronic disorder characterized by extreme pain and tenderness in multiple muscles and joints. The term Fibromyalgia means pain in fibrous and muscular tissues. The pain occurs in areas where the muscles attach to bone or ligaments and is similar to the pain of arthritis. The joints are not affected, so they are not deformed, nor do they deteriorate as they may in arthritis. The worst part about fibromyalgia is the pain is not localized to specific joints. The pain is felt "all over," spreading out from the spine over large areas of the body, especially in the lower back and arms. The pain can vary from muscle twitching to a burning sensation. People with fibromyalgia are susceptible to other chronic painful conditions such as osteoarthritis or migraine . The pain can vary, depending on the time of day, weather changes, physical activity, and the presence of stressful situations. Fibromyalgia was originally named fibrositis. Fibromyalgia pain continues throughout a person’s lifetime.

9.08 Fibromyalgia (Trigger Points)
Presentation
What to do:
  • When you find a trigger point, map out its exact location (point of maximum tenderness) and place an X over the site with a marker or ball point pen. If the trigger point is diffuse there is no need to outline its location.
  • Obtain a careful history and perform a general physical exam to help exclude the possibility of a serious underlying disorder such as rheumatoid arthritis or cancer.
  • With any suspicion that an underlying problem exists, obtain an x ray or an erythrocyte sedimentation rate. These studies should both be normal in fibromyalgia.
  • Where trigger points are diffuse, prescribe a nonsteroidal anti-inflammatory such as naprosen (Anaprox) 275mg two tablets stat then one qid or ibuprofen (Motril) 800mg stat then 600mg qid x 5 days. A muscle relaxant like cyclobenzaprine (Flexaril) may also be helpful.
  • When a focal trigger point is present you can suggest to the patient that he may get immediate relief with an injection. Inject 2-5ml of l% xylocaine or loger-acting 0.5% bupivacaine along with 20-40mg of methylprednisolone (Depomedrol) or 2-5mg of triamcinolone (Aristospan) through the mark you placed on the skin, directly into the painful site. Be sure you are not in a vessel and then "fan" the needle in all directions while injecting the trigger point. In addition, to insure total coverage, massage the area after the injection is complete. The patient will often get complete or near-complete pain relief, which helps to confirm the diagnosis of fibromyalgia. The beneficial effect of this injection may last for weeks or months. A supplementary five day course of non-steroidal anti-inflammatories is optional.

F ibromyalgia is a rheumatologic condition characterized by spontaneous, widespread soft tissue pain, sleep disturbance, fatigue and extensively distributed areas of tenderness known as tender points. Estimates of prevalence are 3.4 percent for women and 0.5 percent for men. Fibromyalgia is characterized by widespread soft tissue pain, disturbed sleep, fatigue and diffuse tender points. Fibromyalgia can be perplexing to patients and physicians because of the lack of associated abnormalities on readily available diagnostic tests. Despite this, recent findings about the pathogenesis and pathophysiology of fibromyalgia have dispelled the belief that the disorder is psychosomatic. While no laboratory test can confirm fibromyalgia, most patients present with a history of widespread pain, physical findings and comorbid conditions. With experience, the disorder may be diagnosed with confidence on initial presentation or after a period of observation and minimal diagnostic testing. The family physician is ideally suited to treat fibromyalgia because its management calls for a longitudinal relationship, a willingness to try different therapeutic modalities and an understanding of the interrelationship of the biopsychosocial aspects of health.

Fibromyalgia (FM) is a chronic, complex pain syndrome involving widespread musculoskeletal pain, fatigue, sleep disturbance and other symptoms for which the cause is currently unknown. Recent research has suggested the involvement of the central nervous system in the development of fibromyalgia, along with a suspected genetic predisposition. We are a community based organization dedicated to serving our membership by:
  • providing support to people with fibromyalgia and their families helping to encourage early diagnosis and effective treatment helping educate physicians, paramedical professionals, family
    members and the community at large regarding fibromyalgia and its treatment promoting research aimed at improving treatment and ultimately finding a cure

    Our clinical and research experience suggests a way of unraveling the mysterious discrepancy between patient conviction and neuropsychological findings. Instead of the flaw residing in the patient complaint, it may well be that these patients as a group are being held to the wrong standards. Testing logic often takes the following form: if memory deficits fail to be demonstrated by scores on standard measures of memory that operationally define level of impairment, then fibromyalgia patients are overestimating or overplaying the severity of their memory decline. The logic is flawed because it does not account fully for the many ways memory failures arise and overlooks shortcomings in the testing process. A process prominently linked to memory impairment is dissociation. Dissociation is a failure to integrate experiences that normally go together. Short-term memory complaints of dissociatives are well known to mimic the complaints of patients with fibromyalgia that we now refer to as fibrofog . Level of dissociation fluctuates in intensity throughout the day. Variation is likely to be influenced by daily stress. With rising intensity, sensory experience is transformed into haze like sensations leaving the patients feeling as if enveloped in a fog. At more severe intensity, lapses in memory are likely to occur leaving the individual unable to remember what was said, done or planned. But cognitive testing at levels of dissociative intensity (low) that are less apt to affect memory should yield normal performance.
