Fibrocystic changes in the breast refer to a variety of irregularities in the breast tissue. These changes are benign, or noncancerous.
What is going on in the body?
Fibrocystic changes occur when there is an imbalance of the female hormones produced by the ovaries. These hormones are estrogen and progesterone. The imbalance changes breast tissue to produce lumps. This condition is very common and is not cancer.
What are the signs and symptoms of the condition?
The classic symptom of fibrocystic changes is pain in both breasts. The pain is usually worse right before menstruation. The woman may have other symptoms, including:
  • breast tenderness cysts, which are fluid-filled sacs within the breast tissue generalized lumpiness of the breasts nipple discharge pain in the shoulders and upper arms
What are the causes and risks of the condition?
Fibrocystic changes are caused by a hormonal imbalance. This imbalance usually involves estrogen and progesterone. An increase in another hormone, prolactin, may also be responsible for some of the changes. Over half of all women experience fibrocystic changes at some time in their lives. The changes are most common in women between the ages of 20 and 50. The condition is uncommon after menopause unless the woman is taking hormone replacement therapy.

Fibrocystic Breasts with Mastalgia and/or Suspicious Lumps
Most women at some time or another have painful or tender breasts. It is the most common breast condition among women. Although mastalgia (painful breasts) rarely signals a serious problem, for those women who have it, this may be small comfort. Among premenopausal women, most breast pain is caused by breast changes associated with fluctuations in hormonal levels that are linked to the menstrual cycle. The term given commonly to this occurrence is fibrocystic breast changes and can cause breasts to feel tender, sore and swollen. Lumps fluid-filled sacs surrounded by fibrous tissue or areas of thickening may also be felt and are called cysts. These changes peak about a week to 10 days before a menstrual period. For years, fibrocystic breast changes were diagnosed as a condition called fibrocystic breast disease. Because about half of all women have this condition, it is now recognized as a common condition. The cause of fibrocystic breast changes is not understood. Some studies have linked diets high in caffeine, cola, or chocolate with increased risk of pain associated with fibrocystic breasts.

If you have fibrocystic breasts it does not mean that you are at greater risk of developing breast cancer. It is true however, that women with fibrocystic breasts may have more difficult to interpret mammograms. If this is the case, then breast ultrasound is often utilized along with screening mammography to further evaluate areas on the mammogram that may be too dense for accurate assessment. Fibrocystic breast changes do not necessarily require any treatment. Women with symptomatic fibrocystic changes particularly women with breast pain may be counseled to limit their caffeine intake or to undergo drainage of large cysts if they are present.

(WASHINGTON, DC—3/21/02) - At least half of all women experience lumpy, painful, swollen breasts at some point in their lives, according to the American Cancer Society. This benign condition is properly called “fibrocystic breasts” or “fibrocystic change”; it is sometimes mistakenly referred to as “fibrocystic disease. In 95 percent of cases, fibrocystic breast changes do not increase a woman's risk of breast cancer. They do, however, lead to thicker and denser breast tissue, which can mask cancerous changes or make them more difficult to detect. This condition can make breast exams and mammograms difficult to interpret, and make early breast cancer detection more challenging. Fibrocystic breasts are most common in women ages 30 to 50, but these changes can occur at any age. In fact, fibrocystic changes occur in varying degrees in almost all women over 30; they are just clinically apparent in about half of them.  Fibrocystic breast changes include variations in tissue that occur during the menstrual cycle, as well as benign lumps and fluid filled sacs or cysts.  "Eight out of 10 lumps I evaluate are benign and most of these are due to fibrocystic breast changes," said George N. Peters, MD, Executive Director of the UT Southwestern Center for Breast Care in Dallas. “This does not mean women who know or suspect they have fibrocystic breasts should ignore lumps. Instead, if they are premenopausal and they find a lump, they should wait one menstrual cycle; in 60-70 percent of cases, the lump will go away. If it doesn't go away, she should see her doctor immediately,” said Dr. Peters, who notes that women have to learn to recognize changes via consistent monthly breast self exams (BSE).

I recently had a stereotactic biopsy done on my right breast after my routine annual mammogram found a spot. It was diagnosed as being fibrocystic, not malignant. I was told by my doctor to come back in 6 months for another mammogram, but I was not told whether or not I needed to do anything beyond that. I had read that fibrocystic breasts can be caused by consuming caffeine, but my doctor didn’t tell me to stop. Should I? Can fibrocystic masses turn into cancer? I have been taking estrogen since menopause (approximately 15 years). First I took Premarin, then Ogen, and finally Cenestin for the past several years. I stopped taking it when I was told that a spot was found, because I was worried about cancer, but my doctor didn’t tell me whether or not I should resume taking estrogen. I’m taking it for my high cholesterol and also for osteoporosis protection. Will I be in greater danger from breast cancer now? I would really appreciate hearing from you. I read your column in First for Women magazine all the time.

The Treatment of Fibrocystic Breasts
The term fibrocystic/cystic breast disease is not a distinct disease, but rather a term used to represent a group of breast tissue abnormalities that may occur separately or together. While we associate this "disease" with the menstrual cycle, it is important to remember that women can experience palpable (able to be felt) breast irregularities regardless of menstruation. Pathologic descriptions of the disease were recorded as early as the 1880's, with the term Chronic Cystic Mastitis identified a decade later. While the past 100 years has introduced a variety of terms to describe the abnormality, a consensus for treatment has developed to treat this significant health problem. Any treatment of fibrocystic breast problem is designed to:
  • alleviate breast pain
  • reduce or remove irregularity
  • rule out the possibility of breast cancer
Nonsurgical treatment can include:
  • Use of sex hormones (estrogens, progestins, androgens)

Last week, got an abnormal reading on a screening mammogram indicating increased breast density in the right breast. I realize that this could be due to a fold during the original picture. Had a physician conducted breast exam approximately three weeks ago which was normal. I am 45 and have 2 kids. During my first pregnancy, had a nodule on my left breast, that was biopsied and benign. At that time I was diagnosed with fibrocystic breasts. There is no hx of breast cancer in my family, although my mother had fibrocystic breasts and, I believe, a number of abnormal mammograms.
Since this discovery on Friday, I have naturally been quite anxious and realized some additional symptoms, which I neglected to tell my physician about during my physical. One-two months ago I had a clear spontaneous discharge from my right breast (and perhaps my left...I can't remember). It was watery, but a small amount. Both breasts were sort of chapped. Neither symptom has returned.
Also periodically I am having breast pain....in my right breast, but also in my left. I was attributing this to my fibrocystic breasts and aging. I also think I may be entering menopause. How alarming are my symptoms? Have already scheduled a follow-up.
