The key to understanding refractive surgery is to understand how the eye works. Normal Vision Sharp, clear vision is the result of light rays passing through the cornea, pupil and lens of the eye and focusing directly on the retina. If the cornea is not round, or is too steep or too flat in relation to the length of the eye, light rays focus either in front of or behind the retina. This results in "refractive errors" such as nearsightedness, farsightedness, and astigmatism. Laser vision correction improves the way the eye focuses light by modifying the curvature of the cornea and re-focusing the light directly on the retina. Nearsightedness Nearsightedness, or myopia, occurs when the curvature of the cornea is too steep or the eyeball is too long. With nearsightedness, the eye's strong refractive power forces the image to be focused in front of the retina, rather than on it.   • Corrective Lenses (glasses): Eyeglasses are, of course, the classic way to correct vision problems. They've been helping those problems since the 13th century. Contact Lenses : Contact lenses are delicately crafted, very thin optical discs generally smaller than a dime, worn directly on the eye. They are comfortably held in place by a natural layer of tears present between the contact lens and the cornea. Contacts eliminate the barriers encountered with spectacles that interfere with the line of sight above, below and to the sides of the eye, offering outstanding peripheral vision. Farsightedness is the result of the visual image being focused behind the retina rather than directly on it. It may be caused by the eyeball being too small or the focusing power being too weak. Farsightedness is often present from birth, but children can often tolerate moderate amounts without difficulty and most outgrow the condition. As aging occurs, glasses or contact lenses may be required to correct the vision. A family history of farsightedness is a risk factor.

What is farsightedness?
Farsightedness (hyperopia) results when structural defects in the eye cause your vision to be blurry. If you are farsighted, you see distant objects more clearly than close objects, though both near and distant vision may be affected, and you may have trouble focusing when performing tasks such as reading or sewing. Although these defects (such as a shorter eyeball or a flat cornea) are often present early in life, normal development and lengthening of the eyeball during early childhood usually corrects the condition. As you age, your eyes lose the ability to change the shape of the lens to focus on near objects ( accommodation ). Farsightedness is often first noticed after age 40 when the eyes begin to lose their ability to accommodate. The age-related decline in focusing power, called presbyopia , makes farsightedness more apparent.

Laser used to treat nearsightedness can now help the farsighted Portland, Ore. - Patricia Aieollo has worn two pairs of glasses for more than 18 years, one for driving and golf, the other to read and watch television. Aiello is farsighted, or hyperopic, meaning she has a problem seeing things close-up. Now, with the help of an excimer laser, she hopes to eliminate the need for her glasses all together. Aiello had her right eye treated with the laser in May, now she's getting her other eye treated on Fri., June 18 at 10:45 a.m. at the Casey Eye Institute at Oregon Health Sciences University. So far she's pleased with the results of the first procedure. "It's absolutely wonderful. I'm so excited to be getting the other one done, so I don't have to wear my glasses anymore. On the golf course the glasses were especially annoying because they kept sliding down," says Aiello. Last November, the Food and Drug Administration approved the use of the first laser surgery system to treat farsightedness, the Visx laser. Two lasers have been used to treat nearsightedness, or myopia, and myopia with astigmatism for many years. Photorefractive keratotomy (PRK) and laser in-situ keratomileusis (Lasik) both reshape cornea so light focuses more precisely on the retina. PRK shapes the cornea's surface; Lasik, a midcorneal method, reshapes the middle of the cornea rather than the front to improve focus.

LASIK for Farsightedness (Hyperopia) In farsightedness (or hyperopia), light that enters the eye focuses at a point behind the retina because the eye is too short, or the cornea is too flat. To correct hyperopia, the surgeon reshapes the sides (mid-periphery) of the cornea in order to steepen the central region. This permits the light entering the eye to focus on the retina, instead of behind it. Eye doctors use the term hyperopia for people who need reading glasses (and occasionally distance glasses) before the age of about 40 years old. This type of farsightedness can be treated with the laser to give most patients excellent distance and reading vision without glasses. Why is "farsightedness" so confusing? Hyperopia is often confused with presbyopia. Presbyopia occurs as a natural aging process. As we get older, we lose the flexibility of the lens inside the eye, which makes it difficult to focus up close. This is the reason people start to need reading glasses over the age of about 40 years old. Because the changes happening with presbyopia are occurring inside the eye, reshaping the cornea with laser surgery will not solve the problem. Excimer laser treatment cannot eliminate the need for reading glasses unless one eye is made nearsighted to improve its reading ability. This would blur the distance vision somewhat in that eye. We call this option monovision Laser Vision Correction of farsightedness has been FDA-approved since 1998. We have been performing this procedure prior to its approval by special permission from the FDA on an investigational basis. Treating farsightedness has now become a routine procedure similar to treating nearsightedness. Farsighted (or hyperopic) patients under 40 can generally achieve both excellent distant and near vision after laser vision correction. For those farsighted patients over 40 we can usually give them excellent distance vision and markedly improved reading vision, but they will still need reading glasses for some activities as they get older. One option for both farsighted and nearsighted patients over 40 who want to reduce their dependence on reading glasses is

Causes
Both of these conditions are caused by structural distortions within the eye.
Myopia
In myopia, your eyes focus ahead of the retina. This usually occurs with an eyeball that is longer or a cornea that is steeper than normal.
Hyperopia
In hyperopia, your eyes focus past the retina. This usually occurs if your eyeball is too short or the cornea has too little curvature. With aging, the lens of the eye becomes less flexible and cannot focus as well on close objects. This condition, called presbyopia, accounts for the development of farsightedness in most people after the age of 40.
Risk Factors
A risk factor is something that increases your chance of getting a disease or condition. Risk factors are slightly different for the two types of vision disorders.

Being farsighted makes it difficult to read things at a close distance, such as a computer screen, magazine, or restaurant menu. Many people use reading glasses to correct this common eye deformity, which is also called hyperopia
Correcting the Cornea
The usual cause of hyperopia is a cornea that is too flat. The cornea is a transparent coat at the front of the eye through which the iris can be seen. CK is a method of reshaping the cornea to increase the curvature and make it more "plump." This procedure involves a small probe about the length of a pen and as thin as a human hair. The probe is applied in a circular motion along the outer cornea to heat and shrink small areas of corneal tissue, which then form a constrictive band. This process increases the overall curvature of the cornea. What differentiates CK from the already available procedures” LASIK and noncontact laser thermal keratoplasty (LTK)”is that CK uses radiofrequency energy rather than a laser. Radiofrequency energy has been used since the 1950s in other types of surgery. This method is slightly less invasive than laser therapy, which actually removes a small portion of the corneal tissue.

In H-LASIK, a thin flap is made on the cornea using a blade or laser. The flap is lifted, and a laser is applied to the central corneal tissue. The laser makes contact with the cornea in a circular pattern around the central optical zone, which changes the profile of the cornea, making it steeper. The laser removes tissue from the cornea very precisely without damaging nearby tissues. The flap is then replaced, allowing for rapid healing. H-LASIK is performed in a surgeon's office or same-day surgery center and does not require a hospital stay.

Now a study of 5,000 healthy volunteers has shown that farsightedness is a risk too. As part of the Beaver Dam Eye Study in Wisconsin, the mostly white participants, ages 43-84, had eye exams, including measurements of pressure within the eye. Most of them returned five years later for follow-up examinations. Those who were farsighted when the study began were 40 percent more likely to later have incident ocular hypertension a condition of abnormally high pressure in the eye but with no damage to the optic nerve. However, eventually the high pressure can injure the nerve and lead to glaucoma. "If confirmed by future studies, it may suggest that people with refractive errors should be monitored more closely for glaucoma," says Tien Yin Wong, the lead author and assistant professor of ophthalmology at the National University of Singapore. The study was published in the January issue of Ophthalmology.
