Support Organizations Familial dysautonomia Familial dysautonomia (FD) is a rare genetic disease caused by abnormal development of the nervous system. People with FD have problems feeling pain and heat, may have problems with balance and swallowing, and are unable to regulate their body temperature and blood pressure. Symptoms of FD include severe eye problems, feeding difficulties, and poor weight gain because of a weak suck reflex. Pneumonia may occur when uncoordinated swallowing allows breast milk or formula into the lungs. Other common symptoms include lack of coordination, inability to feel pain, excessive sweating, and developmental delays in speech and motor skills. Many children have stunted growth and curvature of the spine (scoliosis). FD is treated with surgery to help maintain nutrition and prevent pneumonia. Medications are used to control vomiting, eye dryness, and blood pressure.

Diseases: Familial Dysautonomia amilial dysautonomia (FD) is a rare genetic disease that results from the abnormal development of the nervous system, particularly the sensory and autonomic systems. Dys-auto-no-mia literally means the dysfunction of the autonomic nervous system. The autonomic nervous system controls involuntary functions, such as swallowing, temperature and blood pressure regulation. Individuals with FD cannot regulate these autonomic functions. In addition, they have problems in perceiving various sensations, such as pain and heat. Treatment has had a dramatic impact on improving the prognosis of this disorder. Prior to 1960, approximately 50% of patients died before five years of age. Currently, approximately 50% of patients reach 30 years of age. The greatest impact on treatment has been the increased use of gastrostomy (surgical incision into the stomach) and fundoplication (mobilization of the lower end of the esophagus and subsequent folding of a portion of the stomach around it) to avoid aspiration pneumonia and to maintain adequate nutrition and hydration. Some other important treatment methods have been the use of diazepam and chloral hydrate to control intractable vomiting attacks, and artificial tears to supplement decreased eye moisture. In addition, blood pressure regulation is enhanced by giving extra fluid and salt, physical therapy and an anti-inflammatory medication known as fludrocortisone. FD is inherited in an autosomal recessive manner and affects boys and girls equally. It only occurs in families of Ashkenazi Jewish descent. In 1993 researchers established that the

Familial Dysautonomia and the Brain Behavior Enigma

An understanding of the brain is essential to developing safe and effective treatments for disorders of the nervous system and for advancing our understanding of the human experience. Nervous system disorders cost the US more than $400 billion in medical expenses each year . In addition to having great clinical significance, such research offers a valuable perspective into the nature of the brain-behavior relationship. The extent to which the brain is organized in terms of overt behavior remains an open question. Because it is difficult to ethically manipulate the neural composition of humans in the laboratory setting, our understanding of the biological and neurophysiological influences on behavior is limited. In the spirit of this class, I have decided to take an alternate route to untangling the connections between brain and behavior. In this paper, I will discuss Familial Dysautonomia, a neurological disease that encapsulates the relationship between sensation, perception, emotion, physiological response and the nervous system.

  • What Is It? Familial dysautonomia (FD), also called Riley-Day syndrome, is an inherited disorder that affects the nervous system. People who are born with FD have fewer nerve fibers for carrying sensations. For this reason, they have trouble feeling pain, temperature, skin pressure and the position of their arms and legs. They can't experience taste fully. They also have abnormally low levels of a natural chemical called norepinephrine that helps to relay messages between nerve cells.

    ABSTRACT
    The orthotic management of Familial Dysautonomia presents many challenges to the clinic team. The clinical manifestations of this disorder are many and cause accepted orthotic management regimes to be modified. This article presents the characteristics of this disorder as well as the orthotic management of the condition as prescribed at the Hospital for Special Surgery Familial Dysautonomia Clinic. Modifications to the standard TLSO and CTLSO designs that have shown promise will be detailed. Disorders accompanied by similar clinical features also may benefit from the orthotic changes described.     Introduction
    Familial Dysautonomia, a rare autosomal recessive disorder (1) first described by Riley and Day in 1949 (2), is the most extensively described congenital sensory neuropathy (3). The disorder reduces the number of nerve cells destined to control certain sensations and autonomic functions (4). Studies suggest as few as 1 in 50 to as many as 1 in 30 Jews of Ashkenazi origin are possible carriers of the recessive gene (3,4). To date, as many as 500 cases of Familial Dysautonomia have been diagnosed around the world (5).
