A diagnostic marker for ET would also help to resolve the question whether site-, position- and task-specific tremors, such as primary handwriting tremor and orthostatic tremor, are distinct entities or whether these tremors represent clinical variants of ET [Rosenbaum and Jankovic, 1988; FitzGerald and Jankovic, 1991; Sander et al, 1998]. Treatment Propranolol (Inderal), a beta-adrenergic blocker, remains the most effective drug for the treatment of ET and enhanced physiologic tremors, but other beta blockers may also ameliorate postural tremor [Koller et al, 2000] [Table 2]. Propranolol is less effective in head than hand tremor. The major side effects of propranolol, and to a lesser degree of other beta blockers, include fatiguability, sedation, depression and sexual impotence. These drugs are contraindicated in the presence of CHF, COPD, asthma and insulin-dependent diabetes. Other treatment modalities for postural tremors include injections of botulinum toxin (BTX) into muscles involved in the production of the oscillatory movement and various surgical approaches. In our initial trial of BTX treatment, 67% of 51 patients with various disabling tremors noted at least some improvement [Jankovic and Schwartz, 1991]. A subsequent double-blind, placebo-controlled trial confirmed that BTX is a safe and effective treatment of hand, and particularly head, tremor [Jankovic et al, 1996].

Essential Tremor What is essential tremor? Essential tremor, sometimes called benign or familial tremor, is an uncontrollable shaking of the hands and forearms. You might notice this tremor when your arms are extended in front of your body. Tremor affects each person differently. Some people hardly notice it, but it embarrasses other people. Who gets essential tremor and why? No one knows why some people get essential tremor. It seems to run in families. You may be more likely to have a tremor if a parent or other close family member has a tremor. Tremor can start at any age but often happens in the teen years or in the 50s. It can get worse as you get older. Are there different kinds of tremor?

Essential Tremor
Essential tremor (ET) is considered the most common neurologic movement disorder. It affects an estimated 10 million people in the United States. ET is a chronic condition characterized by involuntary, rhythmic tremor of a body part, most typically the hands and arms. In most patients, ET is considered a slowly progressive disorder and, in some patients, may eventually involve the head, voice, tongue (with associated dysarthria), legs, and trunk. However, in many people, the disease may be relatively non-progressive and the tremor may be mild throughout life. All tremors usually disappear during sleep. Unlike the resting tremor associated with (during which muscles are not voluntarily activated), the symptoms of essential tremor are either absent or minimal during periods of rest. As the disease progresses, individuals with ET experience varying degrees of functional disability and resultant handicap based on the severity of the tremor.

It may occur in several different neurological disorders which have other associated signs or symptoms. For some people, however, their only problem is the tremor and it is not associated with any other condition. In these cases it is called idiopathic or essential tremor (ET). While essential tremor may involve other parts of the body, the hands and head are most often affected. The shaking often begins in the dominant (writing) hand, and may spread to both hands, affecting both writing and eating. Some affected individuals also develop a quavering intonation to their speech as a result of the disorder. Essential tremor is often slowly progressive and shaking may worsen over time.

Essential tremor
The condition, which affects the muscles of the hands, head, and voice, is often mistaken for Parkinson's patients. But essential tremor is 5-10 times more common than Parkinson's disease. Most people diagnosed with essential tremor have action tremor, which means they shake when they move, and some develop a distinctive nodding or shaking of the head. People with Parkinson's usually experience tremors at rest, along with stiffness and slowness of movement. But there's some overlap between the two conditions. Some studies show that 90% of Parkinson's patients have action tremor and 20% of people with essential tremor have tremor at rest. Both conditions cause a loss of the sense of smell, but in essential tremor it's usually milder. Patients with essential tremor can go on to develop Parkinson's disease, but how many do so isn't known. Katherine Hepburn had essential tremor. Medical historians believe the playwright Eugene O'Neill and the American revolutionary Samuel Adams may have also had the disease. Researchers have diagnosed Adams by analyzing the handwriting in his letters.

This disorder, which is an involuntary shaking, usually of the hands, has no satisfactory treatment. It affects more than one of every 100 people in the general population, with the figure climbing to nearly 4 in every hundred among people over 40 years old. In animal studies, 1-octanol reduced chemically induced tremors in rats. This study will test the effects of the accepted daily intake of 1-octanol (1 milligram per kilogram of body weight) on essential tremor in humans. Patients with essential tremor 21 years old and older who wish to enroll in this study will undergo eligibility screening with a medical history and physical examination that includes tests for thyroid, liver and kidney problems. Participants will be randomly assigned to receive either 1-octanol or a placebo (an inactive substance). Patients in both groups will have an intravenous catheter (a thin, plastic tube) placed in an arm vein for collecting blood samples during the study. Those in the 1-octanol group will be given a 1-octanol capsule; the placebo group will receive a look-alike capsule containing no active ingredient. Neither the patient nor the doctor will know which patients are taking 1-octanol or placebo until the end of the study.

    What is Tremor?
    Tremor refers to rhythmic shaking of a body part. There are more than 20 kinds of tremor. Essential Tremor (ET) is the most common. Although called benign essential tremor in the past, it may be far from benign. It may be frustrating, embarrassing, or disabling. People with ET may avoid dining in restaurants or talking in front of others. They may be tired of being told they're "nervous" because their voice quivers or they fear the ordeal of signing a check in public.
