What is essential tremor?
Essential tremor, sometimes called benign or familial tremor, is an uncontrollable shaking in the hands and forearms. It is caused by abnormal communication between certain areas of the brain. You might notice this tremor when your arms are extended in front of your body. Tremor affects each person differently. Some people hardly notice it. Other people may be embarrassed by it. Essential tremor is the most common form of abnormal tremor, which resembles an exaggerated physiologic shaking. Essential tremor is worsened by the same factors that enhance physiologic tremor. Although the cause is unknown, new research shows that one part of the brain called cerebellum does not appear to function properly in patients who have essential tremor. The cerebellum is the part of the brain that coordinates muscle movements and therefore, provides accuracy and harmony to voluntary movement. Nevertheless, no evident brain lesions have been identified in patients with this form of tremor.

Essential Tremor (ET)
Essential Tremor (ET): The diagnosis of ET is based on the presence of bilateral and usually symmetrical postural or action tremor which is visible and persistent, and which involves hands and forearms, (Deuschl, 1998). Essential Tremor can also affect head and voice. Chin or leg involvement is atypical. Some patients, especially female, present with head tremor as an isolated manifestation of ET. Caution is taken to rule out an underlying cervical dystonia with a prominent tremor component. The possibility of underlying symptomatic cause (drugs, toxins, metabolic abnormalities) should also be excluded prior to making a diagnosis of ET. ET is a monosymptomatic disease, meaning that no other abnormal neurological findings are present. ET has strong familial predisposition, with about 50% of patients having a positive family history for tremor. This indicates a likely autosomal dominant inheritance, though the gene has yet to be identified. Lack of family history does not preclude the diagnosis of ET. Symptoms have an insidious onset with a variable rate of progression. The most disabling feature of ET is action/kinetic tremor of the arms, which interferes with the patient's ability to perform daily activities, like eating, drinking, or writing. For people with the common movement disorder known as essential tremor, simple tasks requiring fine motor coordination become increasingly difficult, sometimes even impossible. Three times more prevalent than Parkinson's disease, essential tremor involves uncontrollable shaking of the hands, arms, head or voice. The largely hereditary neurological condition can begin in early adulthood and worsen with age. In an effort to help physicians and patients best cope with essential tremor, the American Academy of Neurology has released the first guidelines for its treatment. The report, posted online, will be published in the June 28 issue of the journal Neurology. Specific drugs commonly used to treat high blood pressure and seizures can be beneficial in the treatment of essential tremor, according to the guidelines. For limb tremors, surgery can be an option if drug therapy is ineffective. "Essential tremor can be quite debilitating and cause embarrassment in social situations. For instance, in a restaurant the person may have difficulty eating when they try to pick up a fork or spoon and bring food to their mouths," said lead guideline author Theresa Zesiewicz, MD, an associate professor of neurology at the University of South Florida (USF)College of Medicine. "Though the tremors do not completely disappear with treatment, they can be managed, making a huge difference in the daily lives of people with essential tremor."
