What is Essential Tremor?
Essential tremor is a very common but complex neurologic movement disorder. It's called "essential" because in the past, it had no known cause. It's not caused by another neurological condition or the side effect of a medication. ET usually affects the hands, but it may also affect the head and neck (causing shaking), face, jaw, tongue, voice (causing a shaking or quivering sound), the trunk and, rarely, the legs and feet. The tremor may be a rhythmic "back-and-forth" or "to-and-fro" movement produced by involuntary (unintentional) contractions of the muscle. Severity of the tremors can vary greatly from hour to hour and day to day.

Essential tremor (ET) is considered the most common neurologic movement disorder affecting nearly 10 million people in the United States. ET is a chronic condition characterized by involuntary, rhythmic tremor of a body part, most typically the hands and arms, often the head and voice, but rarely the legs. ET is generally considered a slowly progressive disorder, although many individuals may have a mild form of ET throughout life that never requires treatment. The most common form of ET affects the arms and hands, usually bilaterally, and is most prominent with the arms held against gravity (postural tremor) or in action (kinetic tremor) such as when writing or drinking from a cup. Unlike patients with Parkinson's disease, patients with ET rarely exhibit a tremor when the arm is at rest. [return to top]

Diagnosis
Although commonly seen in family medical practice, essential tremor may be difficult to diagnose and equally challenging to treat. Some patients may wait up to 20 years before seeking appropriate medical evaluation for their worsening symptoms. ET is a clinical diagnosis; there is no definitive test for essential tremor. Since most people with ET benefit somewhat from drug therapy, early diagnosis and prompt, appropriate treatment may delay or eliminate functional disability. Essential tremor is best diagnosed by a physician who has experience with tremor and movement disorders. It is possible to measure tremor and assess its impacts on patients in a variety of ways. These include electrophysiologic techniques (e.g., EMG, accelerometer, spectral analysis, and weighting of the body part), clinical rating scales (e.g., tremor scale [see below]), and simple objective tests of hand and arm functions. The results of patient interviews and responses to questionnaires help to measure total disability, resultant handicap, and impact on quality of life. Several tremor classification scales have been proposed to assist physicians to distinguish between the different types of tremor, such as action and resting tremors and other variants and to appropriately diagnosis the type of tremor (e.g., probable or possible ET, classic ET, etc.). These proposed classifications include Collaborative Clinical Classification of Tremor (which includes distinction between tremor types, data from medical history, and results of neurologic evaluation) and the Tremor Research Investigation Group (TRIG) Classification of Essential Tremor (which has inclusion and exclusion criteria for definite, probable, and possible ET).

However, many people with tremor have what is called idiopathic or essential tremor. In these cases, which number 3-4 million people in the US, the tremor itself is the only symptom of the disorder. While essential tremor may involve other parts of the body, the hands and head are most often affected. In more than half of cases, essential tremor is inherited as an autosomal dominant trait, which means that children of an affected individual will have a 50 percent chance of also developing the disorder. In 1997, the ETM1 gene (also called FET1) was mapped to chromosome 3 in a study of Icelandic families, while another gene, called ETM2, was mapped to chromosome 2 in a large American family of Czech descent. That two genes for essential tremor have been found on two different chromosomes demonstrates that mutations in a variety of genes may lead to essential tremor. While the mainstays of treatment are drugs such as propranolol and primidone, alternative drugs and surgical treatments are also available. Further understanding of the molecular mechanism behind the disease awaits the discovery and cloning of an essential tremor gene.
