Complications of LM tension: the Plummer-Vinson-Paterson-Brown-Kjelberg Syndrome (PVS)
The etiology of the PV syndrome is unclear. Does the anaemia cause the dysphagia? Does it cause the post-cricoid web? Is the dysphagia hysterical? Does the web cause the dysphagia? Is there an ectodermal defect causing the nail changes and loss of teeth. Are the labial fissures due to staph, monilia or drooling? Answer: None of the above. I will discuss the complications of hiatal transtraction and gastroesophageal reflux concurrently with Plummer-Vinson syndrome (PVS) because they are identical. The various features of PVS are merely selections from the extensive menu of pathologic events that are directly or indirectly related to excessive LM tone and/or the reflux it produces.
At present, PVS is widely regarded as due to an iron deficiency, perhaps complicated by a vitamin B complex deficiency. The several findings are, like the anaemia, attributed to iron deficiency or epithelial dysplasia.
PVS is rarely seen as a full-fledged syndrome.

Esophageal Disease Profiled AIDS Clinical Care (09/95) Vol. 7, No. 9, P. 77 A prospective study in Atlanta cataloged the etiology and course of esophageal ulceration in HIV-positive individuals who were diagnosed between 1990 and 1994. All 100 subjects had symptomatic esophageal ulcer identified on endoscopy, and the majority had longstanding HIV infection, with an average of 15 CD4 cells. The researchers tabulated syndromes including cytomegalovirus esophagitis in 50 subjects, herpes simplex esophagitis in nine, idiopathic esophageal ulceration in 41, and gastroesophageal reflux-associated ulceration in four. For the most part, clinical and endoscopic response to therapy was good, although 27 patients relapsed. These findings demonstrate the shifting microbial etiologies of HIV- related disorders and the need for specific tissue diagnoses.

WASHINGTON - Carbonated drinks may raise the risk of esophageal cancer, a usually fatal disease, researchers reported on Monday. Several studies presented at a meeting of cancer and gastrointestinal experts in New Orleans showed that what people eat and drink could affect a range of cancers. "This research supports the widespread medical recommendations for healthy eating," said Dr. Lee Kaplan of Massachusetts General Hospital and Harvard Medical School in Boston. A team at Tata Memorial Hospital in India found a strong correlation between the rise in per capita consumption of carbonated soft drinks in the past 50 years and a documented increase in rates of esophageal cancer in the United States. Team members studied U.S. Department of Agriculture data to find that per capita consumption of carbonated drinks rose by more than 450 percent, from 10.8 gallons (49 litres) on average in 1946 to 49.2 gallons (224 litres) in 2000. And over the past 25 years, the incidence rates of esophageal cancer have risen by more than 570 percent in white American men. Esophageal cancer affected 13,900 U.S. men and women in 2003 more than 10,000 men and killed almost all of them, according to the American Cancer Society.

Color Video Camera in a Pill Helps Doctors Evaluate Severity of GERD and Pre-Cancerous Conditions Every year millions of Americans undergo procedures to examine the esophagus. The most common procedure involves advancing a long, flexible tube (endoscope) into the mouth, down through the throat into the esophagus. The procedure, known as traditional endoscopy, requires sedation and hours of recovery. "This is a technology that could revolutionize how esophageal disease is identified and evaluated," said Blair Lewis, M.D., a gastroenterologist at the Mount Sinai School of Medicine in New York City. "More patients are willing to swallow a pill than go through a traditional endoscopy." According to health experts, most of the 19 million GERD sufferers do not monitor their condition despite the fact that five percent to 15 percent of GERD patients may have Barrett's Esophagus, a condition that increases the risk of developing esophageal cancer, the fastest growing type of cancer in America. About 700,000 people have Barrett's Esophagus.

The esophagus is the muscular tube which propels food from the mouth into the stomach. In the dog it is made up primarily of skeletal muscle. The esophagus normally has tone like other muscles in the body and is closed when it is not being used. Megaesophagus however denotes an esophageal disease where there is no tone to the esophagus and it becomes flaccid and enlarged. What are the symptoms?
Typically patients with megaesophagus will regurgitate their food. This is different from vomiting in that the food passively comes up from the esophagus versus the active abdominal compression that is involved with vomiting. The constant regurgitation of food and liquids predisposes these patients to aspiration of stomach contents into the airways and lungs. They then develop pneumonia so coughing, lethargy, labored breathing or loss of appetite may also be among the first signs noted in these patients. How is megaesophagus diagnosed?
