To all current and future members:
At the 56th meeting of the Japanese Society for Esophageal Diseases (held in Hiroshima), the Board of Directors, Board of Caretakers, and Board of Representatives decided that the 1st meeting of the Japan Esophageal Society should be held instead of the 57th meeting of the Japanese Society for Esophageal Diseases.
The Japanese Society for Esophageal Diseases was established in 1965. Dr. Kohmei Nakayama had an instrumental role in its founding. During the next approximately 37 years, the Japanese Society for Esophageal Diseases contributed substantially to improving the outcome of treatment for esophageal diseases. Such improvement was enabled by the continuous, sincere efforts of many of our senior colleagues. The history of our society is intertwined with many precious memories and anecdotes.
Esophageal diseases are currently treated at nearly all hospitals. The outcomes of treatment are uniformly excellent. Many experts in surgery, radiology, immunochemotherapy, and basic research have played important parts in the improvement of treatment outcomes. Recent studies have shown that the outcomes of radiotherapy and chemotherapy are comparable to that of surgery in patients with esophageal cancer. Cytological and genetic research will gradually play increasingly important roles in the future.

In this issue of American Family Physician, Shalauta and Saad (1) describe endoscopic screening for Barrett's esophagus and esophageal adenocarcinoma in patients with reflux symptoms. This type of screening has been proposed in an effort to decrease the rate of death from esophageal adenocarcinoma. The concept is intuitively appealingwe know that gastroesophageal reflux disease (GERD) is associated with cancer, and we think that most cancers arise from Barrett's esophagus. Why not use endoscopy for patients with GERD to find those who have Barrett's esophagus, and then to follow those patients with Barrett's esophagus to catch curable cancers? Unfortunately, as with most issues in medicine, the application of this simple concept is fraught with problems. Several barriers exist that make endoscopic screening, as currently practiced, a flawed concept.

Associate Professor of Medicine and Epidemiology
Nicholas Shaheen is Associate Professor of Medicine and Epidemiology at the University of North Carolina (UNC) School of Medicine and School of Public Health, and Director of the Center for Esophageal Diseases and Swallowing at UNC.
Dr. Shaheen attended college at Harvard University and earned his medical degree at the University of Chicago, Pritzker School of Medicine, where he fulfilled his internship and residency requirements. He completed his clinical fellowship training at UNC, where he also earned his masters degree at the School of Public Health. He then completed a National Institutes of Health fellowship in Epidemiology. For the last seven years, he has been a faculty member at UNC.
His research interest is in the epidemiology and management of esophageal diseases. He is the author of numerous journal articles and book chapters related to reflux disease, motility disorders, Barrett’s esophagus, and esophageal cancer. He is currently on the editorial board of Gastroenterology and Evidence-Based Gastroenterology, and is a reviewer for numerous journals. Dr. Shaheen receives research funding from the National Institutes of Health and multiple private foundations and corporations to pursue clinical and translational research in esophageal diseases. He is a member of several professional societies, including the American Gastroenterological Association, the American Society of Gastrointestinal Endoscopy, the American Medical Society, and the North Carolina Medical Society. He is a Fellow of the American College of Gastroenterology and the American College of Physicians.

      Gastrointestinal and Hepatobiliary Surgery General Information The Gastrointestinal Surgical Service has been active within the Department of Surgery and provides comprehensive care for patients that are suffering from surgical diseases of the gastrointestinal tract. This group works in close cooperation with members of the Cancer Center, the Digestive Health Center, the Women's Center as well as other Surgical Services including the Laparoscopy Center. Specifically, patients are seen and evaluated in clinic, receive either surgical or non-surgical evaluation and are evaluated postoperatively on an acute or chronic basis. Diagnostic and therapeutic services for patients with a wide spectrum of gastrointestinal disease including benign or malignant diagnosis are seen and evaluated by the Gastrointestinal Surgical Group. Additionally, this service provides complete diagnostic and therapeutic use of laparoscopy for diverse problems such as gastrointestinal bleeding, polyps, and cancer. The Gastrointestinal Surgery Group also works closely with many members of the Colo-rectal Surgery Group.

Definition: Pathological processes in the ESOPHAGUS. Notes: Esophageal Diseases Categories. Diverticulosis, Esophageal - A pathological condition characterized by the presence of a number of ESOPHAGEAL DIVERTICULA in the ESOPHAGUS. Esophageal and Gastric Varices - Dilated blood vessels in the ESOPHAGUS or GASTRIC FUNDUS that shunt blood from the portal circulation (PORTAL SYSTEM) to the systemic venous circulation. Often they are observed in individuals with portal hypertension (HYPERTENSION, PORTAL). Esophageal Motility Disorders - Disorders affecting the motor function of the upper or lower ESOPHAGEAL SPHINCTERS, the ESOPHAGUS body, or a combination of these parts. The failure of the sphincters to maintain a tonic pressure may result in gastric reflux of food and acid into the esophagus (GASTROESOPHAGEAL REFLUX). Other disorders include hypermotility (spastic disorders) and markedly increased amplitude in contraction (nutcracker esophagus). Esophageal Perforation - An opening or hole in the esophagus as the result of an injury or pathological process. (From Dorland, 27th ed)
