Erythromelalgia is a condition affecting the extremely small blood vessels in the body (micro-circulation). Burning pain of red and warm hands and/or feet are the classical symptoms of erythromelalgia, the feet being predominantly involved. It can occur at any age, being reported in children as young as age five.

General Discussion Erythromelalgia is a rare condition that primarily affects the feet and, less commonly, the hands (extremities). It is characterized by intense, burning pain of affected extremities, severe redness (erythema), and increased skin temperature that may be episodic or almost continuous in nature. (The prefix "erythro-" denotes redness, "mel-" is a combining form meaning limb or limbs, and the suffix "-algia" indicates pain.) Although erythromelalgia typically affects both sides of the body (bilateral), it may sometimes involve only one side (unilateral). In addition, the disease course may be extremely variable from case to case. For example, in some individuals, symptom onset may be gradual (insidious), with the condition potentially remaining relatively mild for years. However, in others, it may have a sudden (acute) onset, possibly spreading and becoming severe over weeks.

Erythromelalgia is a rare syndrome characterized by episodic burning pain, erythema, and elevated temperature of the feet, hands, or both. The syndrome can occur alone or with myeloproliferative or other diseases. Attacks may last from minutes to days and can persist indefinitely in the absence of treatment. The pathophysiology of erythromelalgia is poorly understood. Case reports in the literature have implicated abnormalities of arterioles, platelets, and peripheral nerves. Patients typically obtain relief from elevating and cooling the involved extremity. This relief usually is temporary, with some patients developing immersion injury from prolonged exposure to cold water. Symptomatic responses have been reported to aspirin therapy

Help Center View Basket Checkout Erythromelalgia DESCRIPTION A rare syndrome of paroxysmal vasodilation with burning pain, increased skin temperature, and redness of the feet and, less often, the hands. The early-onset, or primary, form is reported to have a male-to-female ratio of 1:2.5. Secondary erythromelalgia has been reported to have a male-to-female ratio of 3:2 from a review of 60 cases of erythromelalgia due to myeloproliferative disorders. It is not clear what causes erythromelalgia but a common mechanism may be skin microvascular shunting with corresponding hypoxia. Some researchers suggest that Raynaud's and erythromelalgia may result from similar vasomotor abnormalities, with the vasoconstriction phase more prominent in Raynaud's and the hyperemia phase more apparent in erythromelalgia. Other causes may include:

The authors of this study set out to assess the frequency and type of vascular changes and neurologic abnormalities in patients with erythromelalgia. Erythromelalgia is a syndrome characteristic for heat, tingling, burning, erythema and pain of the affected limb. This is a prospective study of patients with no spontaneous symptoms at the time of their visit and with provoked symptoms. There were sixty-seven patients presenting with erythromelalgia at Mayo Clinic, Rochester, Minn, from 1999 through 2001. Vascular studies were perfomed on the test patients; vascular function with and without symptoms was assessed by measurement of local skin temperature, laser Doppler flow, and transcutaneous oximetry. Neurologic assessment included electromyography, nerve conduction studies, and autonomic reflex screening. The authors used the quantitative sudomotor axon reflex test (QSART), adrenergic function testing, heart rate response to deep breathing, and the Valsalva ratio. QSART involved the application of acetylcholine to the suspect area and measuring the response after application.
