General Discussion
Erythromelalgia is a rare condition that primarily affects the feet and, less commonly, the hands (extremities). It is characterized by intense, burning pain of affected extremities, severe redness (erythema), and increased skin temperature that may be episodic or almost continuous in nature. (The prefix "erythro-" denotes redness, "mel-" is a combining form meaning limb or limbs, and the suffix "-algia" indicates pain.) Although erythromelalgia typically affects both sides of the body (bilateral), it may sometimes involve only one side (unilateral). In addition, the disease course may be extremely variable from case to case. For example, in some individuals, symptom onset may be gradual (insidious), with the condition potentially remaining relatively mild for years. However, in others, it may have a sudden (acute) onset, possibly spreading and becoming severe over weeks.

Erythromelalgia , also known as Mitchell's disease (after Silas Weir Mitchell ) and red neuralgia , is a rare disorder in which blood vessels , usually in the extremeties and especially in the feet, are blocked and inflamed, causing a painful burning and throbbing sensation and red skin. The attacks come and go, and tend to be worse in summer. Erythromelalgia is often associated with vascular diseases such as polycythaemia vera, thrombocytosis or with heavy metal poisoning. Erythromelalgia responds to cooling, and often disappears completely on taking non-steroidal anti-inflammatory drugs , such as aspirin A primary (or familial) form is termed erythermalgia . This occurs in patients who are younger. The disease is very rare. It has been linked to a mutation of the SCN9A (a sodium channel), the gene of which is located on the long arm of the second chromosome (2q24). It inherits in an

Synonyms: Erythromelalgia, erythermalgia Classification and Types: Primary: sometimes reported as an autosomal dominant but most likely idiopathic Secondary: associated with other disorders such as myeloproliferative disorders and thrombocytosis. Smith and Allen in 1938 used the term erythermalgia to place an emphasis on the warmth noted with this disorder. They were the first to subdivide the disorder into primary and secondary. Two differing systems of classification exist today. Drenth and Michiels (1994, 1995) proposed using the terms erythromelalgia for a platelet-mediated form that is responsive to aspirin and use the terms primary and secondary erythermalgia for non-aspirin sensitive forms of the disorder. Kurzrock and Cohen (1991) subdivide the disorder into adult-onset idiopathic and adult-onset secondary along with early-onset idiopathic and early-onset childhood forms. The early onset form has a male to female ratio of 1:2.5. Secondary erythromelalgia caused by myeloproliferative disorders seem to occur in a male to female ratio of 3:2.

can't go in the sun in moderate heat with out being miserable. My doctors
ran some tests, Its not diabetes, thyroid disorder, blood pressure, or
glandular related, so they say. My allergies are, sulfa, shellfish, codiene
are amoxicillian (for acne) 1 and 1/2 yrs. Celexa 30 mg for 8 months. If
the temp decreases I am not as miserable, but still not normal. This is getting very frustrating and the doctors think I'm looney . I'm getting scared, It's very uncomfortable and is keeping me from being in a lot of situations. I don't even know for sure if this is the correct forum, my searches at many sites come up empty, but since Erythromelalgia is connected to problems with the vascular system, I decided to try here. I'm trying to find GOOD, basic information/explanations for Erythromelalgia. I have been diagnosed but without any clear understanding as to what this will mean - prognosis. It is secondary to peripheral neuropathy, but doesn't seem to be a neurological disorder, although many of the symptoms overlap. I'm confused! I'll appreciate any help you can give me. Thanks!

According to a well known textbook of medicine, Erythromelalgia is a disorder
characterized by burning pain and erythema of the extremities. The symptoms are
precipitated by exposure to a warm environment, aggravated by the dependent
position, and relieved with exposure of the extremity to cool air, water, or elevation.

    To the Editor: Erythromelalgia is a rare disease of unknown cause, characterized by redness of the skin, increased temperature, and pain in the extremities. Erythromelalgia is often primary, but it may also be secondary to disorders such as myeloproliferative diseases.     Erythromelalgia is a rare disorder that includes enlargement of the blood vessels in the feet.
    The blood vessels are open or dilated and the oxygen and heat of the blood is discharged into the tissues, making them turn red and feel hot all the time. Treatment includes using drugs that induce restriction or opening of the blood vessels.
    People with constricted blood vessels or related circulatory problems should wear socks and well insulated shoes to maintain heat. Some may be more comfortable in sandals, going barefoot, or certainly using a light shoe that can `breath` to allow heat to escape. In severe conditions, pain medications can be helpful. Consult us before taking any medication.
