Erythema multiforme (EM)
Synonym: Erythema exudativum multiforme Epidemiology Fairly common An acute self-limiting skin reaction triggered by a number of stimuli e.g. infections, drugs. It is most commonly recognised by the characteristic "target" or "iris" lesions, but it may present in a wide range of different severities from a mild localised eruption to severe life threatening mucosal disease ( Stevens-Johnson syndrome (SJS)). Commonest cause of EM is herpes simplex infection. Also other infections including mycoplasma, orf and viral hepatitis. Non infectious causes - connective tissue disease, pregnancy, malignancy. In 50% of cases no cause if found. Drugs can trigger EM but more usually the two more serious variants. Drugs associated with EM and its serious variants: Antibiotics Sulphonamides, co-trimoxazole, penicillin, cephalosporins, fluoroqinolones, vancomycin

Many vaccinees develop skin rashes after vaccination, almost all of which are benign, if occasionally frightening in appearance. Some are local skin reactions, while others appear to be hypersensitivity reactions, like erythema multiforme. These rashes require only symptomatic therapy. Rarely, a more serious eruption, Stevens Johnson Syndrome may occur, requiring more aggressive therapy which may include steroids. If possible, a dermatology consultation should be obtained before initiating steroid therapy.

Erythema multiforme
Definition: Erythema multiforme is a skin disorder resulting from an allergic reaction

Causes, incidence, and risk factors: Erythema multiforme is a type of hypersensitivity (allergic) reaction that occurs in response to medications, infections, or illness. Medications associated with erythema multiforme include sulfonamides, penicillins, barbiturates, and phenytoin. Associated infections include herpes simplex and mycoplasma infections.
The exact cause is unknown. The disorder is believed to involve damage to the blood vessels of the skin with subsequent damage to skin tissues. Approximately 90% of erythema multiforme cases are associated with herpes simplex or mycoplasma infections. The disorder occurs primarily in children and young adults.
Erythema multiforme may become noticeable with a classic skin lesion , with or without systemic (whole body) symptoms. In Stevens-Johnson syndrome, the systemic symptoms are severe and the lesions are extensive, involving multiple body areas, especially the mucous membranes. Toxic epidermal necrolysis (TEN syndrome, or Lyell's syndrome) involves multiple large blisters (

Erythema multiforme is a relatively common cutaneous reaction pattern that usually presents as well-demarcated plaques on the trunk and extremities, often with target, or "iris," lesions. Lesions may also occur as widespread plaques or erosions on the oral mucosa, conjunctiva and genitalia. Erythema multiforme can have a varied clinical presentation, which can make diagnosis difficult. Most clinicians separate erythema multiforme into two clinical subsets in the mild cutaneous syndrome (erythema multiforme minor) and the more severe syndrome with marked mucosal damage (erythema multiforme major or Stevens-Johnson syndrome). The two types of erythema multiforme are distinguished by the duration of disease and the degree of mucosal damage. The Stevens-Johnson subtype is characterized by hemorrhagic oral mucosal and conjunctival lesions that can cause severe discomfort and disability.

Erythema Multiforme Information
Erythema multiforme is an acute inflammatory skin disease. Erythema multiforme is divided clinically into minor and major types based on the clinical findings. Approximately 90% of cases of erythema multiforme minor follow outbreaks of herpes simplex. Erythema multiforme may also present as recurring oral ulceration, with skin lesions present in only half of the cases, and is diagnosed by oral biopsy. Since erythema multiforme may have its own prodrome, many medications taken for such symptoms have been implicated in its pathogenesis without definitive proof.

Erythema multiforme major and minor (Stevens-Johnson syndrome) is marked by toxicity and involvement of two or more mucosal surfaces (often oral and conjunctival) and is most often caused by drugs, especially sulfonamides, nonsteroidal anti-inflammatory drugs, and anticonvulsants such as phenytoin .
