Stevens-Johnson syndrome is a severe and sometimes fatal form of erythema multiforme, a disorder that occurs in response to medications, illness, or infections. Erythema multiforme is believed to be an allergic reaction resulting in damage to blood vessels of the skin followed by infection of the skin tissue. More than half of the cases are caused by herpes simplex, and virtually any drug , including penicillins and barbiturates, may cause the viral infection. Herb reactions and Stevens-Johnson syndrome have also been linked.

Erythema multiforme is an unusual allergic reaction which results in red rashes, blisters and ulcers of the mouth membranes and the skin, either together or separately. Some cases are very severe and may be fatal, but most persons have a much more mild, self-limiting form of the disease. Recurring examples do occur, especially when recurring herpes virus infection (fever blisters on the lip) is the triggering event. Erythema multiforme has an abrupt onset and typically resolves without scarring in 2-6 weeks. Systemic or topical corticosteroids are usually effective. Erythema multiforme is an acute, self-limiting, blistering and ulcerative allergic response of the skin and mucous membranes. Some authorities separate the disease into minor and major (Stevens-Johnson syndrome) subtypes, and an extreme subtype referred to as toxic epidermal necrolysis (Lyell's disease), first described in 1956. Most cases are minor, but death has resulted from severe involvement. Infection appears to be the triggering event in at least half of the cases, usually with Herpes simplex virus or Mycoplasma pneumoniae. Medications, especially sulfonamides, and foods may also initiate an attack, and those associated with medications tend to be among the most severe cases.

This leaflet has been written to help you understand more about erythema multiforme. It tells you what it is, what causes it, what can be done about it, and where you can find out more about it. What is erythema multiforme?
The name erythema multiforme tells you a bit about what the rash looks like. It means a redness ( erythema ) that is of many ( multi -) shapes (- forme ). In fact, the rash of erythema multiforme can be recognised by the presence of spots that look like small targets (‘target lesions’). These have a dusky red centre, a paler area around this, and then a dark red ring round the edge. Erythema multiforme is usually mild ( erythema multiforme minor ) - with only a few spots, causing little trouble and clearing up quickly – but there is also a rare but much more severe type ( erythema multiforme major ) that can be life threatening. Beyond this, the classification of erythema multiforme is still not finally settled. The Stevens-Johnson syndrome - in which the brunt of the trouble is borne by the moist areas (mucous membranes) inside the mouth, in the genital area, and on the conjunctiva of the eyes – is a good example of this. For a long time it was thought to be a type of erythema multiforme major; but it may really be closer to another serious skin disorder (toxic epidermal necrolysis) in which the skin strips off from large areas of the body.

What are erythema multiforme and Stevens-Johnson syndrome?
Erythema multiforme (E. multiforme) is a rash that can range from spots to sores. When severe, the condition is called Stevens-Johnson syndrome. In this severe form you have sores over much of your body and you feel sick.
How does erythema multiforme occur?
E. multiforme usually occurs as a reaction to:
  • infections, such as infections caused by the herpes simplex virus some medicines, such as sulfa drugs and the seizure medicine phenytoin (Dilantin).
What are the symptoms?
The first symptom is red spots on the skin, especially on the backs of the arms and the fronts of the legs. The hands and feet also usually have spots. Sometimes the spots are flat, red patches, and sometimes they are raised bumps. They may look like targets with a red center, a lighter area around the red center, and then a red outer ring. The rash can also be on the inside of the mouth and in genital areas. Usually the rash does not itch.

    Erythema multiforme is a skin disease which causes lesions and redness around the lesions. Description
    Erythema multiforme appears on the skin and the mucous membranes (the lining of the mouth, digestive tract, vagina, and other organs). Large, symmetrical red blotches appear all over the skin in a circular pattern. On mucous membranes, it begins as blisters and progresses to ulcers. A more advanced form, called Stevens-Johnson syndrome, can be severe and even fatal.
    Erythema multiforme has many causes, most commonly are drugs. Penicillin, sulfonamides , certain epilepsy drugs, aspirin , and acetaminophen are the most likely medication-induced causes. Erythema multiforme can also be caused by certain diseases. Herpes virus and mycoplasma pneumonia are likely infectious causes. Diagnosis
    The appearance of the rash is sufficiently unique to identify it on sight. Having identified it, the physician will determine the underlying cause. Treatment
    Erythema multiforme is inadvertently treated when the causative agent, whether it be a drug or a disease, is treated. In severe cases, cortisone-like medication is often used along with general supportive measures and prevention of infection.

    General Discussion Erythema multiforme (EM) is the name applied to a group of hypersensitivity disorders, affecting mostly children and young adults, and characterized by symmetric red, patchy lesions, primarily on the arms and legs. The cause is unknown, but EM frequently occurs in association with herpes simplex virus, suggesting an immunologic process initiated by the virus. In half of the cases, the triggering agents appear to be medications, including anticonvulsants, sulfonamides, nonsteroidal anti-inflammatory drugs, and other antibiotics. In addition, some cases appear to be associated with infectious organisms such as Mycoplasma pneumoniae and many viral agents.

    Erythema multiforme
    Definition: Erythema multiforme is a skin disorder resulting from an allergic reaction
    Alternative Names: Lyell's syndrome; Stevens-Johnson syndrome; Toxic epidermal necrolysis
    Causes, incidence, and risk factors: Erythema multiforme is a type of hypersensitivity (allergic) reaction that occurs in response to medications, infections, or illness. Medications associated with erythema multiforme include sulfonamides, penicillins, barbiturates, and phenytoin. Associated infections include herpes simplex and mycoplasma infections.
    The exact cause is unknown. The disorder is believed to involve damage to the blood vessels of the skin with subsequent damage to skin tissues. Approximately 90% of erythema multiforme cases are associated with herpes simplex or mycoplasma infections. The disorder occurs primarily in children and young adults.
    Erythema multiforme may become noticeable with a classic skin lesion , with or without systemic (whole body) symptoms. In Stevens-Johnson syndrome, the systemic symptoms are severe and the lesions are extensive, involving multiple body areas, especially the mucous membranes. Toxic epidermal necrolysis (TEN syndrome, or Lyell's syndrome) involves multiple large blisters (
