Rufus Green Jr., M.D. FACS, welcomes you to his offices on the campuses of RHD Memorial Medical Center, St. Paul Medical Center, and Centennial Medical Center. Doctor Green brings a vast amount of experience to his specialty as well as the belief that every patient is an "individual with unique needs. " Featured Articles: Urologic Advances Female Urology Erectile Dysfunction Erectile Dysfunction: ... Bladder Cancer Erectile Dysfunction Impotence can be defined as the persistent inability to attain and maintain an erection adequate to permit "satisfactory" sexual performance. In the United States 10-20 million men experience complete erectile dysfunction. Another 10 million experience partial erectile dysfunction. The majority of these men are 65 years of age or older. The age-specific prevalence is 5% at age 40 increasing to 15-25% by age 65. Because of such large numbers it is unlikely that any single modality of treatment will be universally accepted as the primary method of impotence management, thus one can expect that a number of treatment options will be available so that the individual needs and demands of each patient can be addressed. Impotence is a frustrating condition that can be caused by either a physical or psychological problem. It affects not only the individual, but the partner, family, and job performance. Over the last 20 years a remarkable amount of significant advances have been made in the field of erectile dysfunction. Research efforts have advanced from the study of the whole organ to the cellular, genetic, and biochemical areas. One of the most significant advances occurred in July 1995 when the FDA approved the use of prostaglandin E1 penile injections for the treatment of impotence.

Impotence/Erectile Dysfunction Introduction
Erectile dysfunction (ED) or male impotence is defined as the inability of a man to achieve and maintain an erection sufficient for mutually satisfactory intercourse with his partner. Sexual health and function are important determinants of quality of life. As Americans age, disorders such as erectile dysfunction (ED) or impotence are becoming increasingly more important. Because this subject is discussed widely in the media, men and women of all ages are seeking guidance in an effort to improve their relationships and experience satisfying sex lives. Sexual dysfunction is often associated with disorders such as diabetes, high blood pressure, heart disease, nervous system disorders, and depression. Erectile dysfunction may also be an unwanted side effect from medication. In some men, sexual dysfunction may be the symptom of such disorders that brings them to the doctor's office. The successful treatment of impotence has been demonstrated to improve intimacy and satisfaction, improve sexual aspects of quality of life as well as overall quality of life, and relieve symptoms of depression.

Impotence/Erectile Dysfunction
What is impotence? Impotence is the failure to achieve an erection, a condition faced by an increasing number of men. In order to diagnose the potential problems that lead to this condition, it is useful to describe what conditions must be present for erection to take place. The first component you need to achieve an erection is a normal, intact male sexual organ. Secondly, a normal amount of circulating hormones is necessary. Third, a man must have an intact nerve supply and blood flow to the male sex organ. Finally, normal erections require a healthy psychological response to the arousing stimuli, coupled with a relaxed state of mind. Impotence is almost always treatable, a resounding majority of patients will respond to one of the available treatments. Unfortunately, many men (some estimates are as high as 1/3) will not seek treatment due to the nature of this problem. Impotence is an extremely common condition and is very treatable. Please check with your doctor to explore treatment options. Another option to try before taking the leap to Viagra, or even along with Viagra is

Three New Drugs

What about Viagra, Levitra, and Cialis? Ouch! Now, we have a choice of three quite effective and well tested oral drugs for erectile dysfunction (often described by the acronym ED). They are not aphrodisiacs, because they will not increase sexual drive. They work by blocking an enzyme called phosphodiesterase-5, or PDE-5. This helps the smooth muscles in the penis to relax, which effectively increases blood flow, allowing an erection. But for them to be effective, sexual stimulation is also required. ED is the consistent or recurrent inability of a man to attain and/or maintain a penile erection sufficient for sexual performance. Obviously an intensely private matter, it is not something that we have begun to talk about in public until quite recently. Consequently it is a common health condition among men that is largely untreated. And it has only since Viagra (sildenafil citrate) came on the market in April 1998 that we have begun to realize how many men suffer from ED. ED is three times more prevalent among men who have diabetes than in the general population. More than half of men with diabetes notice the onset of ED within 10 years of their diagnosis. It occurs in nine percent of men with diabetes between age 20 and 29 years and rises to 95 percent by age 70.

CIALIS IS NOT FOR EVERYONE. Tell your doctor about your medical conditions and all medications, and ask if you're healthy enough for sexual activity. Don't take CIALIS if you take nitrates , often prescribed for chest pain, as this may cause a sudden, unsafe drop in blood pressure. Don't drink alcohol in excess (to a level of intoxication) with CIALIS, as this may increase your chances of getting dizzy or lowering your blood pressure. CIALIS does not protect against sexually transmitted diseases, including HIV.
The most common side effects with CIALIS were headache and upset stomach. Backache and muscle ache were also reported, sometimes with delayed onset. Most men weren't bothered by the side effects enough to stop taking CIALIS. As with any ED tablet, in the rare event of priapism (an erection lasting more than four hours), seek immediate medical help to avoid long-term injury. In rare instances, men taking prescription ED tablets (including CIALIS) reported a sudden decrease or loss of vision. It's not possible to determine if these events are related directly to the ED tablets or to other factors. If you have a sudden decrease or loss of vision, stop taking any ED tablet and call your doctor right away.
Individual results may vary. In clinical trials, CIALIS was shown to improve, up to 36 hours after dosing, the ability of men with ED to have a single successful intercourse attempt. CIALIS has not been studied for multiple sexual attempts per dose.

What is erectile dysfunction?

Erectile dysfunction, sometimes called impotence, is defined as having a consistent problem getting and/or keeping an erection sufficient for you to complete sexual intercourse.
Many men have occasional or temporary erection problems, but that does not always mean erectile dysfunction will become a chronic problem. For the problem to be diagnosed as erectile dysfunction, it must happen on a regular basis. It may not occur every time a man wants to have sex, but it does occur repeatedly, over time. Erectile dysfunction is not a disease itself. It is usually a symptom of some underlying medical condition.

Although there is little research to say that healthy sexual relations can make you live a longer and healthier life, there is plenty of research that shows sexual problems or emotional distancing between couples negatively affects health, particularly for middle-aged and older men. "Sexual dysfunction can be an indictor of other health problems," notes Linda Fisher, associate research director of the AARP. With the assistance of well-known sex researcher John McKinlay, AARP/ Modern Maturity last year released a survey of adults aged 45 and older aimed at discovering their attitudes and sexual activity. That survey, like others, revealed that only a fraction of adults with sexual dysfunction are seeking treatment. Most significantly, heart disease and depression are strongly linked to sexual dysfunction, particularly erectile dysfunction in men. Cardiac problems and depression are two common conditions for middle-aged and older men. And about 25 percent of men by age 50 have at least moderate levels of sexual dysfunction, while half of all men by age 70 have at least moderate degrees of sexual dysfunction, most commonly erectile dysfunction.

    Erectile Dysfunction
    What is erectile dysfunction?
Erectile dysfunction is trouble having an erection or not being able to keep an erection long enough to finish having sex. Another term for this problem is impotence. It is normal for most men to have trouble having an erection sometimes when they are tired or nervous. When erectile dysfunction becomes a pattern or a long-lasting problem, however, it can deeply affect the emotional lives of men and their sex partners. Erectile dysfunction affects older men more than younger men. Midlife and the later years bring changes in circulation that may affect the sex organs. Couples need to be more open and understanding with each other about sexual problems such as erectile dysfunction. Fortunately, there are many effective treatments for erectile dysfunction.
