What is Erectile Dysfunction (ED)? Erectile dysfunction (ED) is defined as the consistent inability to obtain or maintain an erection for satisfactory sexual relations. Data from the Massachusetts Male Aging Study has indicated that the prevalence of erectile dysfunction of any degree is 39% in men 40 years old, and 67% in those aged 70 years. This extrapolates to over twenty million men in the United States alone with ED. Erection problems do not indicate a lack of sexual interest or desire, and the man may or may not be able to have orgasms or to ejaculate. Erection problems can occur at any age but are more common as men age. Occasional episodes are considered normal and often do not indicate a serious problem. The cause of erection problems may be physical (such as injury to the nerves or blood flow going to the penis) or psychological (such as anxiety or depression). Erection problems also can be caused by medications, tobacco use, alcohol consumption, or drug abuse. Several treatments are available, including oral medications, a vacuum device, medications injected into the penis, and penile implant surgery. Counseling can help improve psychological factors.

July 2003 Erectile dysfunction (ED), or impotence, occurs when a man has difficulty with either getting an erection or with keeping an erection for long enough to allow satisfactory sexual activity. It is one of the most common sexual problems and affects around half of all men over 40 at some point. It becomes more common and severe as men get older. However, only a fraction of affected men seek help.
Erections
The shaft of the penis has two chambers that fill up with blood during sexual arousal. These are known as the corpora cavernosa. When a man has sexual thoughts or feelings, the brain communicates this sexual excitement through the nerves and these eventually reach the genitals. In turn, the nerves bring about a relaxation of the muscle cells in the walls of the blood vessels entering the penis. This increased flow of blood fills up the two corpora cavernosa resulting in an erection.
Cause of erection problems
There are a number of causes of ED. In around two thirds of cases, there are physical problems affecting the blood supply. However, there is a complex relationship between physical and psychological aspects of sexual function. For instance, physical health problems can cause psychological distress that can have an additional effect on erection problems.

Many men have this health problem as stated earlier. Studies show that about half of the men between the ages of 40 and 70 have ED to some degree.

Including less severe degrees of erectile dysfunction this number swells to nearly 30 million! Erectile dysfunction typically appears in males in their fifth decade of life. It is estimated that approximately 50% of all males between the ages of 50 and 70 exhibit some degree of erectile dysfunction. By stark contrast, at 40 the incidence of erectile dysfunction is on average considerably less, about 5% ( ten-time less! ). Furthermore, environmental factors such as drug abuse, stress, anabolic steroids, poor diet and bad health, can accelerate the expression of erectile dysfunction causing it to manifest at an earlier age. Sadly, despite the high incidence of erectile dysfunction in older males, less than 5% ever seek treatment. Erectile dysfunction IS NOT an inevitable consequence of aging!

    • Erectile dysfunction (ED) is a common condition and can usually be managed pharmacologically, with drugs delivered by intracavernosal injection (ICI), transurethrally or orally.
    • The cardiovascular status of the patient and his overall fitness for renewed sexual activity must be assessed before treatment for ED is initiated.
    • The efficacy of sildenafil is related to the extent and severity of ED, and is significantly reduced in patients with severe vasculogenic ED, ED associated with diabetes and after radical prostatectomy.
    • Alprostadil (prostaglandin E ) is the drug of first choice in patients treated with ICI; it is effective in 72.6% of men with ED and is associated with a low risk of priapism and cavernosal fibrosis.
    • Transurethral alprostadil is significantly less effective than alprostadil ICI, producing improved erections in 30%-40%, but rigid erections in only 10%, of men with ED.

      • alprostadil ICI is an effective treatment for ED
