    EPIDIDYMITIS Common Name: None Specified Description: Infection or inflammation of the epididymis, which is a tubular structure attached to the upper posterior part of each testicle, and results in scrotal pain. Causes: Causes depend on the age of the patient: In younger men ( less than 35 )- Sexually transmitted disease e.g.. Chlamydia or Gonorrhea and E. coli infection, associated with anogenital sex- can be seen in gay men. Older than age 35- Non-sexually transmitted disease associated with prostatitis, post prostatectomy, and benign prostatic hypertrophy (obstruction leads to high voiding bladder pressures and causing reflux into the epididymis). Infection is usually due to gram-negative bacteria ,namely Eschericia coli (E. coli) and Pseudomonas aeruginosa. In children , the infection is due to E. coli . Children presenting with epididymitis should always be investigated for underling congenital abnormalities, such as vesicouretral reflux ( reflux of urine from the bladder into the ureter) or an ectopic ureter (abnormal position of ureter). Also,epididymitis may occur in patient on treatment with a drug called Amiodarone (this medication is used in treatment of an irregular heart beat). This is a non-infectious epididymitis, that resolves on decreasing the drug dose.

    EPIDIDYMITIS
    Among sexually active men aged less than 35 years, epididymitis is most often caused by C. trachomatis or N. gonorrhoeae. Epididymitis caused by sexually transmitted E. coli infection also occurs among homosexual men who are the insertive partners during anal intercourse. Sexually transmitted epididymitis usually is accompanied by urethritis, which often is asymptomatic. Nonsexually transmitted epididymitis associated with urinary tract infections caused by Gram-negative enteric organisms occurs more frequently among men aged greater than 35 years, men who have recently undergone urinary tract instrumentation or surgery, and men who have anatomical abnormalities. Although most patients can be treated on an outpatient basis, hospitalization should be considered when severe pain suggests other diagnoses (e.g., torsion, testicular infarction, and abscess) or when patients are febrile or might be noncompliant with an antimicrobial regimen.

It presents as sudden painful scrotal swelling. There usually is pus in the urine. The germ that causes it is usually bacterial; we are not always successful in figuring out exactly which germ it is. The major problem with epididymitis lies in the fact that differentiation from torsion of the testicle can be very difficult. Surgical exploration is often the only way to be sure, and must be undertaken very quickly to save the testicle if torsion is present. If the problem is just infection, bed rest and antibiotics are curative.

    How to Treat Epididymitis in the Wilderness
    The epididymis is a tube in the scrotum. Epididymitis is an inflammation of this tube, causing great discomfort. Steps: Identify the signs and symptoms of epididymitis: reddening and swelling of the scrotum, pain or discomfort in the scrotum, and possible fever. These signs and symptoms will increase gradually. Support the scrotum with a jock strap or an improvised jock strap made from a triangular bandage or a bandanna. Confine the patient to bedrest - any activity will be very painful. Apply cold packs to the scrotum. Administer aspirin, ibuprofen or acetaminophen to help with the fever and pain. Evacuate to a hospital immediately - antibiotics will be necessary for recovery.
