The treatment for acute epididymitis is well accepted and effective. It includes antibiotic therapy, bed rest, scrotal support ("supporter"), and oral anti-inflammatory drugs (such as Ibuprofen). Each of these modes of treatment is important. Chronic epididymitis is more of a problem (though less severe) because it's symptoms seem to persist even after the initial treatment. In these cases a second round of therapy may be helpful. Beyond this, longer term anti-inflammatory medication is recommended. Surgical treatment for chronic epididymitis is an uncommon last resort.

General Discussion
Epididymitis is inflammation of the long, tightly coiled tube behind each testicle (epididymis) that carries sperm from the testicle to the spermatic duct. Affected individuals usually have painful swelling of the one epididymitis and the associated testicle. In some cases, the second testicle may also be tender. In addition, affected individuals have fever, painful swelling and redness (erythema) of the scrotum, and/or inflammation of the tube from which urine is carried from the bladder (urethritis). The two main forms of epididymitis are the sexally-transmitted form and the nonspecific bacterial form.

I am a white male, 39 years old, and have good health. I was diagnosed with epididymitis on Friday, 5/6/05. Rx was for Cipro, 750 mg 2X daily, and Vicoden for pain. I've had this one time before about 20 years ago, which was a milder case then.
Symptoms were: A really swollen left testicle including the respective epididymis, extreme pain producing nausea and vomitting (one time), chills, etc. I started the Cipro late Friday evening, and things are changing.
First, the pain is reduced. I do not need the Vicodin anymore. The testicle is still swollen but is now getting hard. I assume this has something to do with the fluid/infection coming out. So I have a few questions:
1. Is it normal for the testicle to go from really swollen with fluid around it to just plain swollen and a hard feel?
2. I can feel a pain up my left/front groin area. I think this is coming from the sperm cord. Plus, I have a slight ridge by the scrotum where this cord seems to exit. The sperm cord is a guess, so it could be something different. Anyway, is it normal for that to hurt for a while? It seems to be worse when I stand up.

GENERAL INFORMATION Epididymitis is an inflammation of the epididymis, an oblong structure attached to the upper part of each testis. The inflammation may be so great that the testicle itself feels greatly enlarged and hardened. However, this does not mean that the testicle itself is infected.
Some rarer types of bacteria create a different picture altogether. Tuberculosis, for instance, is a bacteria that can spread to the epididymis in unusual circumstances. Tuberculosis epididymitis develops very slowly over days and weeks. Non bacterial epididymitis simply means an inflammation of the epididymis not caused by a bacteria. The causes are many and include viruses, trauma, and unknown causes. The unknown causes are commonly referred to in medicine as 'idiopathic' - an unknown cause. Non-bacterial epididymitis can be just as painful and with a very quick onset. The viral types may be associated with fevers and generalized weakness and ill feeling. Epididymitis caused by trauma or the 'idiopathic' types are not usually associated with the fevers and chills and generalized symptoms. Many believe that these types of epididymitis are caused by a rupture or blow-out of the sperm ducts causing a leakage of sperm fluid into the tissue. The body tissues can be quite irritated by this leakage and hence the redness and swelling. The treatment for these types of epididymitis may include antibiotics just to prevent bacteria from infecting the area whose resistance has been weakened.

The epididymis is a soft, coiled tubular structure on the back of the testicle. What is going on in the body? Infections generally spread to the epididymis from the bladder or the urethra. The urethra is the tube that allows urine to pass from the bladder to the outside of the body. In young children or older men, bacteria that cause urinary tract infections are often the cause. Sexually transmitted diseases are more likely to be the cause in sexually active young men. Infection in the epididymis causes severe pain and swelling. The infection may spread to the testicle. When both structures are inflamed, the condition is known as epididymo-orchitis. It is rare that an infection will occur in the testicle alone. The bacteria will usually infect the epididymis first.
