Epididymitis
Definition
Epididymitis is inflammation or infection of the epididymis. In this long coiled tube attached to the upper part of each testicle, sperm mature and are stored before ejaculation. Description
Epididymitis is the most common cause of pain in the scrotum. The acute form is usually associated with the most severe pain and swelling. If symptoms last for more than six weeks after treatment begins, the condition is considered chronic. Epididymitis can occur any time after the onset of puberty but is most common between the ages of 18 and 40. It is especially common among members of the military who exercise for extended periods without emptying their bladders. Factors that increase the risk of developing epididymitis include:
  • Infection of the bladder, kidney, prostate, or urinary tract Other recent illness Narrowing of the urethra (the tube that drains urine from the bladder) Use of a urethral catheter.

Although epididymitis can be caused by the same organisms that cause some sexually transmitted diseases (STDs) or occur after prostate surgery, the condition is generally due to pus-generating bacteria associated with infections in other parts of the body.

Diagnostic Considerations Men who have epididymitis typically have unilateral testicular pain and tenderness; hydrocele and palpable swelling of the epididymis usually are present. Testicular torsion, a surgical emergency, should be considered in all cases but is more frequent among adolescents. Torsion occurs more frequently in patients who do not have evidence of inflammation or infection. Emergency testing for torsion may be indicated when the onset of pain is sudden, pain is severe, or the test results available during the initial examination do not enable a diagnosis of urethritis or urinary tract infection to be made. If the diagnosis is questionable, an expert should be consulted immediately, because testicular viability may be compromised.

Support Organizations Epididymitis Epididymitis is inflammation and infection of the long, tightly coiled tube that lies behind each testicle and collects sperm (epididymis). Epididymitis is usually caused by a bacterial infection but also may occur following a urologic procedure. Sexually transmitted diseases cause most cases of epididymitis in men younger than age 35. Epididymitis usually starts suddenly. Symptoms can include pain, scrotal swelling, painful or frequent urination, and fever or chills. Bacterial epididymitis is treated with antibiotics. Other treatment of epididymitis may include bed rest, ice packs, scrotal support with a jock strap, and pain medication. A person with symptoms of epididymitis should avoid sexual intercourse until he or she sees a health professional. This will reduce the risk of spreading a possible infection to his or her sex partner or partners. It is important for sex partners to be evaluated and treated for a possible infection.

Along with antibiotics, comfort measures, such as warm compresses and nonsteroidal anti-inflammatory medications (e.g., ibuprofen, acetaminophen, naproxen sodium, aspirin), usually clear up the infection and resolve the accompanying pain or discomfort within about 2 - 4 weeks. Chronic epididymitis (lasting longer than 6 weeks) can occur without any bacterial infection present. It sometimes develops after a severe case of epididymitis or after multiple attacks of acute epididymitis. A chronic urinary tract infection sometimes also has been the source of chronic epididymitis.

What is epididymitis?
Epididymitis is an inflammation of the epididymis. The epididymis is the tube through which sperm pass. The tube is located at the back of each testicle. Epididymitis can affect both testicles, but it most often occurs on just one side.
How does it occur?
Epididymitis is caused by bacterial infections, such as:
  • urinary tract infections sexually transmitted diseases such as chlamydia or gonorrhea prostatitis (infection of the prostate).
It may also happen if you have:
  • a fungal infection Henoch-Schoenlein purpura (a disease that causes a rash and swelling of joints) a prostatectomy (removal of the prostate) a medical procedure that affects your urinary tract, such as cystoscopy (looking at the inside of the bladder through a slim, flexible, lighted tube) or a catheter (a small tube put into the bladder to drain the urine from your body).

Epididymitis is an inflammation of the epididymis, the tubular structure that connects the testicle with the vas deferens.
Causes Acute epididymitis causes swelling of the scrotum, pain in the testicles, and sometimes a fever of up to 6 weeks duration or less (usually with a gradual onset over several days). If not treated, or in some other cases, the condition can become chronic. In chronic cases, there is usually no swelling, but simply pain. The incidence is approximately 600,000 cases per year. The highest prevalence is in young men 19 to 35 years of age. The disorder is a major cause of hospital admissions in the military (causing approximately 20% of admissions). Epididymitis is caused by spread of infection from the urethra or the bladder. The most common organisms involved in the condition in young heterosexual men are gonorrhoea and chlamydia.
