    7.07 Epididymitis
    Presentation
    An adult male complains of dull to severe scrotal pain developing over a period of hours to a day, and radiating to the ipsilateral lower abdomen or flank. There may be a history of recent urethritis, prostatitis or prostatectomy (allowing ingress to bacteria), straining with lifting a heavy obiect, or sexual activity with a full bladder (allowing reflux of urine). There may be fever, nausea, or urinary urgency or frequency. The epididymis, is tender, swollen, warm, and difficult to separate from the firm, nontender testicle. Increasing inflammation can extend up the spermatic cord and fill the entire scrotum, making examinations more difficult, as well as produce frank prostatitis or cystitis. The rectal exam therefore may reveal a very tender, boggy prostate.
    What to do:
    • Ascertain that the testicle is normal in position and perfusion. Doppler ultrasound may help pick up a drop-off in arterial flow from spermatic cord to testicle in testicular torsion.
    • Palpate and ausculate, the scrotum to rule out a hernia. Gently palpate the prostate once. Culture urine and/or any urethral discharge to identify a bacterial organism.

    Epididymitis refers to inflammation of the epididymis, which is a tube system, lying on top of the testicle. The most common causes of this condition are infection by Chlamydia or Gonorrhoea and it represents a complication of infection from the urethra (and you might consider it the male equivalent to pelvic inflammatory disease ). If the infection involves the epididymis on both sides there is a (small) threat of sterility. The condition can be very painful and can even be a "Waiting Room" diagnosis on seeing a male patient limping and grimacing into the clinic. It may occur in conjunction with a discharge but this is not always the case. The infection may occur as a complication of a urinary tract infection, highlighting that sexual transmission is not always necessary. A rare cause of the problem is the virus that causes mumps, and even then infection of the testicle itself is more likely. Testing The diagnosis is made by clinical examination by the doctor, feeling for tenderness around the epididymis and testicle. In conjunction with this, swabs are taken from the urethra for the routine testing of chlamydia and gonorrhoea. Treatment will often be started on the basis of the clinical examination alone as the infection swabs will take up to a week to process.

    Epididymitis can be hard to distinguish from testicular torsion . Sometimes, both can occur at the same time. Tests are needed to distinguish chronic epididymitis from a range of other disorders that can cause constant scrotal pain. These include: testicular cancer , enlarged scrotal veins ( varicocele ) or a cyst within the epididymis . As well, the nerves in the scrotal area are connected to those of the abdomen, sometimes causing pain similar to a hernia (see referred pain ). Tests may also include a physical examination and ultrasound . A urologist may need to be consulted. Chronic epididymitis is epididymitis which lasts past the first treatment. Typically, a second, longer round of treatment is used. Chronic epididymitis is characterised by

    Epididymitis is an inflammation of the epididymis. The epididymis is the long coiled tube at the top and rear of each testicle. It carries sperm to the vas deferens. The vas deferens is the tube that is cut during a vasectomy to prevent sperm from mixing with the remainder of ejaculatory fluid. See your doctor immediately if you think you have epididymitis. What Causes Epididymitis? Epididymitis can be caused by trauma to the scrotum, chlamydia and an infection with bacteria that are normally present in the intestinal tract .     Epididymitis (EP-ih-DID-ee-MY-tis) is an infection of the epididymis, a tube located behind the testicle. The infection is common in men 19 to 35 years old.
    Causes
    The problem may start with an infection of the bladder or prostate gland. The infection is sometimes contracted through sexual contact.
    Signs/Symptoms
    The usual symptoms are fever, pain, redness, and swelling of the scrotum and painful urination. You also may feel a lump in your scrotum.
    Care
    Your doctor will prescribe an antibiotic to fight the infection. Rest until you feel better, and follow the directions listed below.
    WHAT YOU SHOULD DO
    • Rest in bed until fever, pain, and swelling go down. The testicle may stay swollen and hard for several days or even a few weeks.
    • If your doctor prescribes an antibiotic to fight the infection, take it exactly as directed and finish the entire prescription. If you stop taking the drug too soon, a few germs may survive and re-infect you.

Scrotal disorders may be due to inflammation of the scrotal wall or contents, trauma (see below), neoplasms of the testis or testicular appendages (see Ch. 233 ), or mechanical abnormalities involving the scrotal contents or adjacent structures (see Testicular torsion under Renal and Genitourinary Defects in Ch. 261). Scrotal abscesses may complicate epididymo-orchitis (see under Epididymitis, below), especially when treatment is delayed. Some abscesses drain spontaneously, but most require surgery, usually orchiectomy with drainage. Urethral stricture and diverticulum may be accompanied by abscess formation with swelling of the scrotum, pain, and erythema and extravasation of urine into the scrotum and perineum. Incision and drainage as well as antibiotics are often indicated, but urinary diversion by suprapubic cystostomy is needed only in severe cases with abscess. Persistent strictures can usually be treated successfully by dilation or endoscopy (internal urethrotomy). In difficult or recurrent cases, a permanent stent may be placed endoscopically.     Definition
    Brucella ovis is responsible for the disease known as Contagious Epididymitis in rams. The disease has never been recorded in the United Kingdom, but has been reported from Australia, New Zealand, the United States of America, South Africa and parts of Europe. Prevalence is thought to be high in the Southern European states. Infected flocks may suffer considerable economic loss because of poor fertility. There are no records of human infection, and the disease is not classed as a zoonosis.
    Animals affected
    Under natural conditions, Brucella ovis affects only sheep. Goats may be infected experimentally, but most laboratroy species are resistant.

      • Look for serous urethral discharge (chlamydia) or purulent discharge (gonorrhea)
    • Older than age 35
      • Coliform bacteria usually, but sometimes Staphylococcus aureus or epidermidis
      • Often associated with distal urinary tract obstruction
      • Tuberculosis, if sterile pyuria and nodularity of vas deferens
      • Sterile urine reflux after transurethral prostatectomy
      • Granulomatous reaction following bacillus Calmette-Guerin (BCG) intravesical therapy for superficial bladder cancer
    • Prepubertal boys
      • Usually coliform bacteria
      • Evaluate for underlying congenital abnormalities, such as vesicoureteral reflux or ectopic ureter
