This 28 y/o male presented with lower extremity paresthesias and weakness. There was a history of a recent febrile illness treated with over the counter medications. There is abnormal increased T2 signal in the spinal cord consistent with demyelination. The diagnosis of Acute Disseminated EncephaloMyelitis (ADEM) was considered. The patient was treated with high dose corticosteroids and the symptoms gradually resolved. Followup MRI of the thoracic spine one year later showed complete resolution of the abnormal cord signal.

    ENTEROVIRUS ENCEPHALOMYELITIS
    (previously Teschen/Talfan disease) Article 2.6.3.1. For the purposes of this Terrestrial Code , the incubation period for enterovirus encephalomyelitis shall be 40 days. Standards for diagnostic tests and vaccines are described in the Terrestrial Manual Article 2.6.3.2. Enterovirus encephalomyelitis free country A country may be considered free from enterovirus encephalomyelitis when it has been shown that enterovirus encephalomyelitis has not been present for at least the past 3 years. This period shall be 6 months after the slaughter of the last affected animal for countries in which a stamping-out policy is practised with or without vaccination against enterovirus encephalomyelitis. Article 2.6.3.3.

    Until the late 1980s, myalgic encephalomyelitis was thought to be a distinct, infectious disorder affecting the central, peripheral and autonomic nervous systems and the muscles. Its major symptom was fatigue to the point of extended periods of exhaustion. A group of experts studying the Epstein-Barr virus first published strict criteria for the symptoms and physical signs of chronic fatigue syndrome in 1988. This case definition was further refined in 1994.
    The Fact Sheet for CFS published by the National Institutes of Allergy and Infectious Diseases of the National Institutes of Health states that "[T]oday, CFS is also known as myalgic encephalomyelitis, postviral fatigue syndrome, and chronic fatigue and immune dysfunction syndrome." ME/CFS is now recognized as part of a range of illnesses that have fatigue as a major symptom.

ADEM is more frequently encountered in children than in adults. We had the opportunity to examine and follow-up a rare case of elderly patient with fulminant ADEM treated successfully with a regimen of intravenous immunglobulin (IVIG) followed by intravenous methylprednisolone (IVMP). Follow-up magnetic resonance imagines (MRI) showed striking resolution of abnormalities over 4 months. He had free of symptoms for 3 years. Distinctive clinical and radiological features of ADEM and multiple sclerosis (MS) were also discussed on the basis of our case and recent literature.     View Current Signatures Sign the Petition To: United States Department of Health and Human Services, National Institutes of Health, and Centers for Disease Control We, the undersigned, respectfully demand that Myalgic Encephalomyelitis (ME), an internationally recognized neurological disease, be also formally recognized as a distinct clinical entity in the United States by the National Institutes of Health and the Centers for Disease Control. It is time to make the case definition/diagnostic criteria explicit in the name.
    Currently, most patients are diagnosed with Chronic Fatigue Syndrome, a poorly defined, misunderstood, controversial syndrome which remains underfunded and underresearched. The name Chronic Fatigue Syndrome itself has trivialized the illness and further subjected these patients to incredulity and ridicule. The CFS rubric has not only neglected and failed to help those with ME, it has harmed them. ME patients will not improve with cognitive behavioral therapy (CBT) or graded exercise (in fact, exercise will make them worse) - yet these are considered treatments for CFS. Simply "renaming" CFS will not solve this problem.
    ME should not be considered a subset of CFS, for the hallmarks of ME will not be present in the population of CFS patients once the Ramsay criteria are established in the US. Patients with Myalgic Encephalomyelitis under the CFS criteria must wait for six (6) months before acquiring a diagnosis, a critical period in which possible recovery is diminished by lack of proper treatment.
