Diagnosis: Acute disseminated encephalomyelitis (presumptive)
The differential diagnosis for diffuse white matter involvement in an adult is broad, including various infectious, inflammatory, and neoplastic processes. Included within the differential are fungal and mycobacterial disease (excluded by culture data), Lyme disease (essentially excluded by negative titers, the rare occurrence of this condition in this part of the country, and the absence of supportive history), and various unusual infections, such as Brucellosis, etc. (excluded by the absence of fever, culture data, and clinical story). CNS lymphoma may present with multifocal white matter involvement, but is unusual in the non-immunocompromised individual (CNS lymphoma typically occurs in a localized region in this population). Metastatic cancers would not have the MRI appearance suggestive of demyelination and would not present with the CSF findings in this case (unless the patient developed meningeal carcinomatosis). The most likely possibility, then, is an inflammatory process affecting the neuraxis at several points (spinal cord, brainstem, and subcortical white matter). The differential diagnosis would then include CNS involvement from systemic lupus erythematosis (or other collagen vascular diseases), CNS vasculitis, sarcoidosis, or inflammatory demyelination following infection (post-infectious demyelinating encephalomyelitis) or associated with multiple sclerosis. The lack of historical or laboratory support for SLE, sarcoid, or other collagen vascular diseases essentially excludes these from consideration. The vigorous inflammatory response would be highly unusual for a CNS vasculitis, leaving the most likely possibility of primary CNS inflammation/demyelination.

Note: In the following report Dr. Yazbak and I have reviewed the case of an infant death which we believe to have been mistakenly diagnosed as shaken baby syndrome, the true cause of death in our opinions having been a vaccine-induced encephalitis. Having carefully followed the case and its developments for nearly a year, the report represents untold numbers of hours of study and investigation into the many technical parameters of the case. From this study we have come to realize that this case is representative of an emerging pattern of increasingly frequent vaccine reactions on the modern scene, which are being overlooked or misdiagnosed by our present health-care system. The "Story of Baby Alan" is a poignant one, all the more so because it is something that could happen to any young couple or parent. Although the story is necessarily technical, nevertheless we urge that you read it. If you are a young person contemplating having a family, it is something that could happen to you. Harold E. Equine encephalomyelitis By Jennifer Kunz, AnimalForum.com staff E quine encephalomyelitis is an inflammation of the brain and spinal cord. This disease not only affects horses, but is also deadly for humans. The virus was isolated, characterized, and vaccines were produced in the 1930s. The two forms currently active in the United States are Eastern equine encephalomyelitis (EEE) and Western equine encephalomyelitis (WEE). The viruses responsible for causing these diseases are members of a family of viruses called the alphavirus. WEE is most active in the western United States and parts of the Midwest. WEE was responsible for the death of 180,000 horses between 1931 and 1938 in the San Joaquin Valley, Calif. There have been isolated cases in every state west of the Appalachians, parts of western Canada, and in Central and South America. EEE has an eastern United States distribution. There have been isolated cases in Michigan and Wisconsin as well as outbreaks in the Caribbean, Dominican Republic, Haiti, Panama, and Central and South America. The mosquito transmits the virus from small infected animals such as birds and rodents to horses. The warm, humid weather of the summer is good for mosquito breeding and this is when outbreaks are more common. Transmission of EEE and WEE is not horse to human, but bird or rodent to human via the mosquito.

Archive Section Mar 1st - 15th 2001 Acute Disseminated Encephalomyelitis (ADEM) (Please wait for the content to download) How did discussion proceed? A two-year old male child presented with high-grade fever, continuous with cough from last two days. There is history of sudden inability to move left side of body from last one day. There is no history of seizure/altered sensorium/speech disturbance. No History of vomiting, loose motions.
On examination child is conscious, irritable, and slightly pale, without cyanosis, icterus, clubbing or lymphadenopathy. Heart rate of child is 90/min, BP - 90/68 mm Hg. All peripheral pulses are well palpable. Child is recognising parents, speech normal for age, no cranial nerve involvement. Tone, bulk, power, DTR and planter of right side are normal. Left side tone is decreased, bulk normal, power 3/5 both limbs, DTR positive. Abdominal, cremastric reflexes are absent and planter upgoing.

HPI: One week later the patient noted decreased vision in OD and for about 2 days she noticed pain in and behind the right eye. As this persisted, the patient was brought again to a local hospital. Hedaches remained unchanged, though the patient did vomit once before presentation to the local hospital. The fundus was examined and an MRI of the head and orbits was obtained. Findings were concerning and the patient was transferred to the University of Iowa. PMH/POH: None Social Hx: Patient lives in an Iowa Amish community

Viral encephalomyelitis of horses is a group of closely related diseases of Equidae which are transmitted by arthropods (insects), and which cause moderate to high mortality after presenting clinical signs of dysfunction of the nervous system. They are caused by Alphavirus (family Togaviridae) , of which three strains are commonly recognized: Eastern Equine Encephalomyelitis (EEE), Western Equine Encephalomyelitis (WEE) and Venezuelan Equine Encephalomyelitis (VEE) , Human infection can occur, and the disease is of significant public health importance. The viruses are restricted to the Americas, with cases reported from the USA, Canada, Venezuela, Brazil and Argentina. Outbreaks have occurred in Central America. In North America the disease tends to be seasonal due to the changes in mosquito and other insect populations. Spread of the disease between areas by migratory birds may be possible, and encephalitis has been detected in infected birds. In general, mortality is lower with WEE infection (20-30 %) than with EEE or VEE (40-80 %). Wild birds act as a reservoir for the virus, and spread is by insects. Mosquitoes appear the main vector, but mites, ticks and lice may also be implicated. The movement of infected mosquioes between distant areas by high winds has been recorded. Rodents and other small mammals may serve as viral reservoirs for VEE.

What is Venezuelan equine encephalomyelitis?
Venezuelan equine encephalomyelitis ( VEE ) is a mosquito-borne viral disease, affecting all equine related species, such as horses, mules, donkeys, and zebras. Humans can also contract this disease. In horses, the disease typically starts as a mild fever that progresses into severe and often fatal encephalomyelitis (inflammation of the brain and spinal cord). In some cases it can produce an acute disease causing sudden death. In humans, the virus usually causes flu-like symptoms such as fever and headaches.

(1) reports successful treatment of human herpesvirus 6 (HHV-6) encephalomyelitis. The patient was an immuno-competent young woman whose symptoms were fever, urinary retention, blurred vision, quadriparesis, bilateral papillitis, and optic neuritis. Magnetic resonance imaging (MRI) showed multiple lesions on the spinal cord white matter and the left thalamus, and the cerebral spinal fluid (CSF) showed inflammation.
