What is empty sella syndrome?
Empty sella syndrome is common in women who are overweight or have high blood pressure. Characterized by an enlarged bony structure (sella turcica) that houses the pituitary gland at the base of the brain, the disorders sometimes results in high fluid pressure inside the skull. The pituitary gland is usually normal or small in size.
What are the symptoms of empty sella syndrome?
Each individual may experience symptoms differently. Besides high fluid pressure in the skull, which occurs in about 10 percent of patients, another symptom is a chronically runny nose.

Varanasi Introduction Top Empty sella refers to the radiological appearance of an enlarged or deformed sella turcica that is partially or completely filled with cerebrospinal fluid. In the earlier days, the radiographic appearance of an enlarged sella turcica was frequently equated with the presence of an intra or perisellar neoplasm. It was found, however, in some of these patients on pneumoencephalography, that the enlarged sella was not occupied by a tumour mass, but a significant amount of air was reaching the sella indicating that the subarachnoid space was extending well below the clinoid processes. To describe this finding, several different terms were used such as intrasellar arachnoid diverticulum, intrasellar cysts, deficient sellar diaphragm, intrasellar cistern . Busch , while reporting the presence of empty sella in 5 percent of normal subjects on autopsy studies, used the term "empty sella" to describe this condition. However, the term empty sella in fact, is a misnomer as the sella is not completely empty, but the pituitary is always present both anatomically and functionally, though often it is displaced downwards and compressed by CSF pressure. An empty sella thus is usually an incidental anatomic finding and occasionally it results in abnormal pituitary function. Regardless of the size, the sella is completely or partially filled with CSF. It can occur at any age and in either sex. However, it is more common in women and increases in frequency with age

In most cases, however, the pituitary functions normally as evidenced by normal thyroid functions, normal tests of adrenal function, normal somatomedin-C levels, and regular menses. Some patients have empty sella syndrome as a result of other processes such as neurosarcoidosis pituitary tumors that have degenerated, etc. Rare patients have a congenital empty sella and a coexisting pituitary tumor. Empty Sella Syndrome (ESS) is a disorder that involves the sella turcica , a bony structure at the base of the brain that surrounds and protects the pituitary gland. ESS is a condition that is often discovered during tests for pituitary disorders, when radiological imaging of the pituitary gland reveals a sella turica that appears to be empty. There are two types of ESS: primary and secondary. Primary ESS happens when a small anatomical defect above the pituitary gland increases pressure in the sella turica and causes the gland to flatten out along the interior walls of the sella turica cavity. Primary ESS is associated with obesity and high blood pressure in women. The disorder sometimes results in a build-up of fluid pressure inside the skull and the pituitary gland may be smaller than usual.
