Emphysema - an Introduction
E mphysema is a devastating disease caused by destruction of lung tissue. The usual cause is cigarette smoking. As tissue is destroyed in the lung, large airspaces or holes appear in the lung. The elastic properties of the lung are decreased and the airways narrow. This results in overinflation of the lung (lungs too large), large decreases in the patient's ability to move air in and out of the lungs, and decreases in the ability of oxygen to pass into the blood from the lungs. Emphysema, when severe, is as devastating and debilitating as cancer, severe heart failure or any other severe chronic disease. The most troublesome symptom is severe shortness of breath of even the most trivial effort. Patients may develop a whole host of secondary symptoms such as anxiety and depression, severe pulmonary infections, heart failure, and respiratory failure. Mortality is considerably increased in emphysema patients.
Traditional Treatment T raditional medical treatment consists of smoking cessation (the first step), medications to open the airways of the lungs as much as possible (bronchodilators), medications to decrease inflammation in the airways (steroids), oxygen, and antibiotics for pulmonary infections. Inhalers for the administration of bronchodilators and steroids are frequently used.

Signs and symptoms
Emphysema is characterised by loss of elasticity of the lung tissue; destruction of structures supporting the alveoli; and destruction of capillaries feeding the alveoli. The result is that the small airways collapse during expiration, leading to an obstructive form of lung disease (air is trapped in the lungs in obstructive lung diseases). Features are: shortness of breath on exertionparticularly when climbing stairs or inclines (and later at rest), hyperventilation and an expanded chest. As emphysema progresses, clubbing of the fingers may be observed, a feature of longstanding hypoxia Emphysema patients are sometimes referred to as "pink puffers". This is because emphysema sufferers may hyperventilate to maintain adequate blood oxygen levels. Hyperventilation explains why emphysema patients do not appear cyanotic as chronic bronchitis (another COPD disorder) sufferers often do; hence they are "pink" puffers (adequate oxygen levels in the blood) and not "blue" bloaters (cyanosis; inadequate oxygen in the blood).

Emphysema is a disease characterized by dilation of the alveolar spaces and destruction of the alveolar walls. With their loss, much of the elastic recoil of the lung is also lost. Compliance of the lung in emphysema is significantly above normal; the lung becomes easy to distend but empties slowly. This results in a chronically overinflated lung (high total lung capacity, functional residual capacity, and residual volume), which lessens the curvature of the diaphragm, making it less efficient in generating even the small swings in pleural pressure necessary for breathing. Pulmonary function tests on a patient with emphysema will reveal a compromised expiratory flow (due to their low lung recoil), including a low FEV1, FVC, and FEV1/FVC ratio. Emphysema Overview
Emphysema is a long-term, progressive disease of the lung that primarily causes shortness of breath. In people with emphysema, the lung tissues necessary to support the physical shape and function of the lung are destroyed. It is included in a group of diseases called chronic obstructive pulmonary disease or COPD (pulmonary refers to the lungs). Emphysema is called an obstructive lung disease because the destruction of lung tissue around smaller airways, called bronchioles, makes these airways unable to hold their shape properly when you exhale. This group of diseases ranks as the fourth leading cause of death in the United States. Unlike heart disease and other more common causes of death, the death rate for COPD appears to be rising. Emphysema changes the anatomy of the lung in several important ways.
  • Normally, the lungs are very spongy and elastic. When a breath is taken, the chest wall expands, expanding the sponge. Similar to the way a squeezed sponge will draw water into it when released, suction draws air into the lungs when the chest wall expands. Air is brought though the trachea (windpipe) and bronchi (the main air tubes going to right and left lungs). These tubes divide into smaller and smaller tubes, finally ending in alveoli. Alveoli, the tiniest structures in the lung, are very small air sacs that are arranged like a bunch of grapes. The alveoli are at the ends of the smallest tubes called bronchioles. The alveoli and the bronchioles are very important structures for the lungs to function properly. It is these structures that are destroyed by emphysema.

    What Is Emphysema?
    Emphysema is a condition in which there is over-inflation of structures in the lungs known as alveoli or air sacs. This over-inflation results from a breakdown of the walls of the alveoli, which causes a decrease in respiratory function (the way the lungs work) and often, breathlessness. Early symptoms of emphysema include shortness of breath and cough.
    How Serious Is Emphysema?
    Emphysema is a widespread disease of the lungs. In 1993, 1.9 million people in the U.S. had emphysema. It is estimated that 50,000 to 100,000 Americans living today were born with a deficiency of a protein known as alpha 1-antitrypsin (AAT) which can lead to an inherited form of emphysema. Emphysema ranks 15th among chronic conditions that contribute to activity limitations: over 43 percent of individuals with emphysema report that their daily activities have been limited by the disease. Many of the people with emphysema are older men, but the condition is increasing among women. Males with emphysema outnumber females by 22 percent.
