Ehrlichiosis
Human Ehrlichiosis is an illness transmitted to humans by tick bites. It is caused by two different species of rickettsial bacteria: E. chaffeensis and E. phagocytophilia . Many individuals infected with Ehrlichiosis are also infected with Lyme disease or babesiosis. Most people infected with Ehrlichiosis experience fever, persistent headache, chills, muscle aches, and fatigue. Some might also experience abdominal pain, nausea, vomiting, diarrhea, cough, and joint aches. It is usually treated with the antibiotic doxycycline. If not treated early, Ehrlichiosis is potentially fatal, especially in the elderly, those with compromised immune systems, people with diabetes, and those with collagen vascular disease. Ehrlichiosis is a disease that affects a variety of animals including human beings; caused by obligatorily intracellular bacteria belonging to the genus Ehrlichia (Classification in Fig. 1 ). There are several species assigned to this genus and those for which the vectors are known are transmitted by ixodid ticks. Ehrlichial organisms infect predominantly white blood cells of their vertebrate hosts; one exception is Ehrlichia platys which infects dog platelets. The organisms appear in clusters known as morulae in the cytoplasm of infected cells. Ehrlichial organisms are classified as agranulocytic ( Fig.2 ) or granulocytic ( Fig.3 ) based on the cells they infect and one that infect platelets ( Fig.4 ). The species currently assigned to the genus Ehrlichia are as follows:

The etiologic agent of canine monocytic ehrlichiosis (CME), the rickettsia Ehrlichia canis E. canis ), is a small pleomorphic gram-negative coccoid bacteria that parasitizes circulating monocytes intracytoplasmically in clusters of organisms called morulae. 4. EHRLICHIOSIS
Two previously unknown tick-borne diseases have been discovered in the past decade: Human monocytic erhlichiosis (HME) and human granulocytic ehrlichiosis (HGE) . The agents are bacteria that live inside monocytes and granulocytes, two types of circulating white blood cells. Since 1964, more than 400 confirmed cases of HME, and 170 cases of HGE have been reported, including some from northern California. Many were first thought to be Rocky Mountain Spotted Fever. Ehrlichia body inside a stained monocyte HME is caused by a bacteria named Ehrlichia chaffeensis , and HGE by another bacteria closely related to Ehrlichia equi . Similar agents cause severe disease in animal populations. The still undiscovered source of the human infections may involve domestic (HGE) and wild mammals (HME). Many cases of E. equi infection have been recorded from horses in Marin and Sonoma counties, but there is no formal reporting system so the actual numbers are not known. In one study site in the Sonoma Valley, workers found E. equi

Ehrlichiosis Human Monocytic Ehrlichiosis (HME) is caused by E. chaffeensis, which was discovered in 1991. They recognized Human Granulocytic Ehrlichiosis (HGE) as a new disease in 1993. HME and HGE have been referred to as spotless Rocky Mountain spotted fever. Having similar clinical symptoms, ranging from a mild illness, to severe life threatening conditions, both types are tick borne diseases. Other species of Ehrlichiae, E. ewingii, and E. phagocytophila are also capable of causing disease in humans. The bacteria that cause ehrlichiosis are transmitted to humans through the bite of infected ticks, which acquire the bacteria after feeding on infected animal reservoirs. The public health importance of ehrlichioses has not been well defined, largely because these diseases are newly recognized. A national ehrlichiosis surveillance program currently does not exist. The emerging tick-borne zoonoses human monocytic ehrlichiosis (HME) and human granulocytic ehrlichiosis (HGE) is under reported in the United States. What Is Ehrlichiosis?
Ehrlichiosis is caused by several bacterial species in the genus Ehrlichia that affect humans and animals. Currently there are four ehrlichial species that are known to cause disease in humans. Human monocytic ehrlichiosis (HME) from infection with Ehrlichia chaffeensis is the ehrlichial disease most often reported in Kentucky and other southeastern and midwestern states. Six or fewer confirmed cases per year have been reported in Kentucky since 1991.
How Ehrlichiosis Is Spread?
Human monocytic ehrlichiosis is spread by the bite of an infected tick. The lone star tick, Amblyomma americanum is the primary vector for HME. Other known vectors of ehrlichiosis in the United States are the blacklegged tick (Ixodes scapularis) and the western blacklegged tick (Ixodes pacificus). Ticks become infected with ehrlichiae while feeding on blood from the host animal in their larval or nymphal state. Whitetail deer are the major host of the lone star ticks. The tick develops into the next stage and the ehrlichiae are then transmitted to the following host during the feeding process. Both the nymphal and adult stages are probably capable of transmitting the ehrlichiae to the next host.
