When I was eight years of age I was sent to hospital with what doctors described as " a troublesome foot ". The first diagnosis made of me was "Post Polio". Just before I reached the age of ten, I underwent a series of brain operations which many people believe were "experimental" for a condition which medical records call " Dystonia Musculorem Deformens Today my disability is called Generalised Dystonia or Primary Dystonia and is described as a "chronic involuntary movement disorder" for which there is no known cause and no known cure. Finding medical records relating to my time in hospital or records pertaining to surgery performed on me has proven very difficult. Eventually a few pages did surface - one reads as follows: "Admitted for Stereotactic treatment of lower limb disc and dyskenesia. An electrode was inserted into the Globus Pallidum but for technical reasons it was not possible to coagulate it. The Patient appeared to improve following insertion of the electrode" Another medical record reads: Readmitted for possible basal gangloin surgery. A small abscess had formed around one of the markers and therefore no surgery could done, but the marker was removed.

Wat verstaan we onder dystonie? Dystonie is letterlijk vertaald: een stoornis in de spanning van een spier (= tonus). De spierspanning is vaak te hoog, maar op andere momenten weer normaal. De dystonie wordt niet alleen gekenmerkt door onwillekeurige, wisselende spierspanningen, maar ook door een stoornis in het normale samenwerken van de betreffende spieren. Daardoor ontstaan draaiende, wringende bewegingen en ook abnormale houdingen van een of meer lichaamsdelen. De dystone bewegingen kunnen in alle lichaamsdelen optreden, zowel in rust als tijdens een willekeurig uitgevoerde beweging. Het woord dystonie wordt vaak in twee betekenissen gebruikt. Ten eerste ter beschrijving van het symptoom zelf, en om dit te onderscheiden van andere vormen van onwillekeurige spiersamentrekkingen, zoals beven of spasticiteit. Ten tweede ter aanduiding van de ziekte die gekenmerkt wordt door dystonie als enig en op zichzelf staand symptoom: dystonia musculorum deformans. Onderverdeling De dystonie is onder te verdelen naar: beginleeftijd, oorzaak en uitbreiding van de onwillekeurige beweging (tabel 1).

A medical viewpoint by Prof JA Temlett Dystonia was first described at the turn of the century by Qppenheim, and from the outset the condition was surrounded by controversy. Even the name originally proposed - Dystonia musculorum deformans was ambiguous, since it is neither a primary muscle disease nor of necessity deforming. The preferred terms are 'Torsion Dystonia' or simply 'Dystonia'. The Dystonia Medical Research Foundation's ad hoc committee in 1984 defined Dystonia as a' syndrome dominated by sustained muscle contractions, but certain muscle groups are characteristically involved. Dystonia can affect both sexes, and all age and race groups, and is distributed world-wide; the syndrome may sometimes be familial. The cause is presumed to be a neurochemical imbalance of the basal ganglia, but the specific neurochcmicals and the anatomical site involved remain undefined. While Dystonia is almost never 'functional' or hysterical, stress aggravates the condition and rest relieves it. Classification by age of onset, aetiology and body segment involved suggests two ends of a spectrum.
