Dystonia musculorum deformans: implications of therapeutic response to levodopa and carbamazepine

Sporadic, autosomal recessive and dominant forms of dystonia musculorum deformans have been recognized. This communication reports the results of treatment of six patients with this condition. Two patients with the recessive form responded to levodopa therapy. Three patients who responded to carbamazepine therapy probably have the dominant form. Nocturnal EEG, electro-oculograms, and electromyograms were studied in nine patients with dystonia musculorum deformans and in nine healthy controls. Electrodes were placed over frontal, central, and occipital regions in accordance with the international 10-20 system of electrode placement. A standard bipolar montage was used for the recordings, and records were scored independently in accordance with the manual of Rechtschaffen and Kales. All patients were found to sleep poorly. What is dystonia? Dystonia is a general term used to describe failure of regulation of muscle tone, usually a sustained increase in muscle tone. When opposing muscles of an extremity have increased tone, there is a limitation of voluntary activity. When the increased tone of opposing muscles is not symmetrical, the extremity or trunk may be drawn into a distorted posture that cannot be overcome voluntarily. The most common form of dystonia is dystonia musculorum deformans. What is dystonia musculorum deformans and what are the symptoms? In the usual history, the first symptoms begin just prior to puberty, but they may occur anywhere between the ages of 3 and 15 or not until early adulthood. When the disease begins early, it usually involves the lower extremities first, and gait (walking) disturbance may be the only symptom for some time. When the disease begins later in life trunk and neck involvement is more likely to be the initial manifestation. The rate of progression of the symptoms is variable, but appears to be more rapid in patients whose symptoms appear early. The dystonia may progress to the point of almost complete immobility and death from secondary complications, usually pneumonia. What causes dystonia musculorum deformans?

What is dystonia?
Dystonia is a state of abnormal (either excessive or inadequate) muscle tone. There are many forms of dystonia. Dystonia disorders cause involuntary movements and prolonged muscle contraction, resulting in twisting body motions, tremor , and abnormal posture. These movements may involve the entire body, or only an isolated area. There are several situations that are associated with dystonia, including medication-induced and dystonia resulting from a specific form of lung cancer . Symptoms may even be "task specific," such as writer's cramp. A form of dystonia known as early-onset torsion dystonia (also called idiopathic or generalized torsion dystonia) begins in childhood around the age of 12. Symptoms typically start in one part of the body, usually in an arm or leg, and eventually spread to the rest of the body within about 5 years. Early-onset torsion dystonia is not fatal, but it can be severely debilitating. Most children with the disorder are unable to perform the simplest of motor tasks and are confined to a wheelchair by the time they reach adulthood. What are patterns of dystonia?

What is it?
Early-onset generalized dystonia, the most common hereditary form of dystonia, is characterized by the twisting of the limbs, specifically the foot/leg or hand/arm. The spasms may spread to involve twisting contractions of other parts of the body.

Symptoms
Symptoms in early-onset generalized dystonia can range from twisted postures, turning in of the foot or arm, muscle spasms, unusual walking with bending and twisting of the torso, rapid, sometimes rhythmic, jerking movements; and progression of symptoms leading to sustained or fixed postures. Because the legs and trunk are so commonly affected in early-onset generalized dystonia, abnormal gait may be common. Factors such as age and site play a significant role in the progression of early-onset generalized dystonia. The younger the age of onset, the more likely the dystonic symptoms will begin in one of the legs, spread upward to other areas, and possibly become generalized. Symptoms commonly begin with a specific action, that is, the abnormal movements appear with a specific action, and are not present at rest. For example, if it begins in one leg, the symptoms may only be present when walking and disappear when the child runs or walks backwards.
