Painful intercouse (dyspareunia) Pain with intercourse is a signal that something is wrong. There are a variety of things to consider when this symptom occurs – the most common things are included below:
  • first time having sex
  • uncomfortable position (the wrong angle of penetration)
  • too forceful movements
  • inadequate lubrication
  • irritating condom material (e.g. nonoxynol-9)
  • irritating creams, lotions or oils used in the genital area
  • bladder spasm, infection or full bladder
  • vaginal infection
  • STD
  • prolapse (weakened areas in the pelvic floor)
  • hernia
Anytime painful intercourse is a regular problem or if it’s accompanied by bleeding or deep pelvic pain, do not delay in having these symptoms evaluated by your OB/GYN health care provider.

Pain with guys is often caused by a frenulum that is too short or a foreskin that’s too tight , by a penis that is too crooked, by an oversensitive glans or by an infection.
Girls can feel pain if they start intercourse before they are aroused, their vagina being too dry. Other possible causes are a tough hymen , a vaginal infection , an involuntary tightening of the muscles around the vagina vulvar vestibulitis or kamsymfyse . Pain in the abdomen may be caused by PID . If a guy goes into the vagina to deep to fast he’ll hit the cervix , which may also be painful.
Sometimes it’s hard to find out what causes the pain, even for a doctor.
If intercourse is painful for you or your lover don’t continue as if nothing is wrong or as if it will disappear by itself in due time. This might well destroy your (sexual) relationship. Stop having intercourse, make love in other ways by using your hand(s) and/or mouth and seek medical help to find out what’s wrong.

What about more superficial pain?
Pain that is nearer the outside of the vagina can also be caused by vaginismus (see above). However, there can be many other possible causes. Lack of lubrication Menopausal or post-menopausal dryness

Definition
Dyspareunia refers to pain in the pelvic area during or after sexual intercourse. This can occur in both men and women although it is more common in women.
Causes
Women It is believed that dyspareunia is caused by organic (physiological) factors at least 75% to 80% of the time. Pain that occurs initially at entry but decreases over time is commonly caused by inadequate lubrication. This is frequently due to a lack of sexual arousal and effective stimulation, and sometimes due to medication that decreases vaginal lubrication (such as antihistamines). Other causes in women include vaginitis (vaginal infections), endometriosis, cystitis or urethritis, orthopedic problems, retroversion of the uterus, and chronic constipation. Psychological factors are infrequently involved and will most commonly be associated with previous sexual trauma (rape or abuse), feelings of guilt, or negative attitudes toward sex. Men In men, the most common causes of pain that occur at the time of ejaculation are prostatitis and urethritis. This is often associated with a recent reduction in the frequency of sexual activity.

Dyspareunia is genital pain experienced just before, during or after sexual intercourse.(1) Although this condition has historically been classified as a sexual disorder, an integrated and pain-model approach to the problem is gaining support. The current thinking about pain initiation and promulgation suggests an initial instigating factor that is then perpetuated by confounding factors.(2-6) These factors may be physical or psychologic. Patients with dyspareunia may complain of a well-defined and localized pain, or express a general disinterest in and dissatisfaction with intercourse that stems from the associated discomfort. Although dyspareunia is present in both sexes, it is far more common in women, with the pain initiating in several areas, from vulvar surfaces to deep pelvic structures.

Dyspareunia is recurrent genital pain caused by sexual activity. Primary dyspareunia is defined as constant pain during sexual activity, while secondary dyspareunia occurs after a period of pain-free lovemaking. Vaginismus is a conditioned pain caused by involuntary spasm of the muscles around the lower one third of the vagina, resulting from the association of sexual activity with pain and fear. Butcher reviews these two conditions and their respective management approaches and briefly discusses the role of orgasm in women.
