Organizations Definition Dyspareunia refers to pain in the pelvic area during or after sexual intercourse. This can occur in both men and women although it is more common in women. Causes Women It is believed that dyspareunia is caused by organic (physiological) factors at least 75% to 80% of the time. Pain that occurs initially at entry but decreases over time is commonly caused by inadequate lubrication. This is frequently due to a lack of sexual arousal and effective stimulation, and sometimes due to medication that decreases vaginal lubrication (such as antihistamines). Other causes in women include vaginitis (vaginal infections), endometriosis, cystitis or urethritis, orthopedic problems, retroversion of the uterus, and chronic constipation. Psychological factors are infrequently involved and will most commonly be associated with previous sexual trauma (rape or abuse), feelings of guilt, or negative attitudes toward sex. The current thinking about pain initiation and promulgation suggests an initial instigating factor that is then perpetuated by confounding factors.(2-6) These factors may be physical or psychologic. Patients with dyspareunia may complain of a well-defined and localized pain, or express a general disinterest in and dissatisfaction with intercourse that stems from the associated discomfort. Although dyspareunia is present in both sexes, it is far more common in women, with the pain initiating in several areas, from vulvar surfaces to deep pelvic structures.
